The original descriptions of polymyalgia rheumatica (PMR) and
giant cell arteritis
(
GCA
) in the medical literature date back to 1888 and 1890, respectively. Classification criteria for PMR and
GCA
are not standardized since most authors used subjective criteria based on their personal experience. Only one study has evaluated criteria for PMR and has found seven variables with high discriminant value. Criteria for
GCA
are less varied because a positive biopsy of the temporal artery is diagnostic. However, combinations of different clinical and laboratory features have been used for diagnosis when biopsy is negative or missing. Assessment of PMR/
GCA
is based on the serial determination of markers of acute phase such as ESR, CRP, or plasma viscosity. However, their value in predicting recurrence of the diseases is poor. New immunological factors including soluble interleukin-2 receptors, interleukin-6, serum soluble CD8, and serum soluble
intercellular adhesion molecule-1
are presently under investigation.

The pathophysiology of cluster headache (CH) is supposed to involve the lower posterior part of the hypothalamus, the trigeminal nerve, autonomic nerves and vessels in the orbital/retro-orbital region. The exact connection of this hypothalamic-trigemino-autonomic-vascular axis is not fully understood. The presence of inflammation in the perivascular tissue of the retro-orbital region has been presented as a possible mechanism behind the pain and the sympatheticoplegia sometimes observed during headache attacks. In a previous study we found neither increased levels of erythrocyte sedimentation rate, C-reactive protein or acute-phase reactants nor clinical signs of a generalized inflammatory disorder. However, these tests may not be sensitive enough to detect a focal inflammatory process in the retro-orbital region. In the present study, we analysed serum levels of three soluble adhesion molecules; soluble
intercellular adhesion molecule-1
(sICAM-1), soluble vascular cell adhesion molecule-1 (sVCAM-1) and soluble E-selectin (sE-selectin) in patients with episodic CH and in patients with biopsy-positive
giant cell arteritis
(
GCA
), a known vasculitic disorder of large and medium-sized arteries. A control group of healthy volunteers was also included. Within the CH group, sICAM-1, sVCAM-1 and sE-selectin showed an increasing trend in remission compared with the active CH period, but the difference was statistically significant for sE-selectin only. The mean sICAM-1 value was higher in patients with active
GCA
than in CH patients during the active cluster period. Compared with the healthy control group, the mean levels of soluble adhesion molecules in CH patients also tended to be higher, but statistically significantly so only for sVCAM-1. We hypothesize that CH is not a vasculitic disorder of the medium-sized arteries, but CH patients may have an immune response that reacts differently from that of healthy volunteers.

The aim of this study was to determine whether
intercellular adhesion molecule-1
(
ICAM-1
) polymorphisms are associated with susceptibility to vasculitis or rheumatoid arthritis (RA). Meta-analyses were performed to assess the associations between K469E and G241R polymorphisms of
ICAM-1
and vasculitis or RA. A total of 12 studies on 1,368 patients and 1,922 controls, which comprised 8 vasculitis studies and 4 RA studies, were included in the meta-analysis. We found no significant association between vasculitis and K469E E allele among the various subjects (OR = 1.238, 95% CI = 0.9781-1.566, p = 0.076). However, an association between vasculitis and K469E polymorphism was observed under homozygote contrast (OR = 1.443, 95% CI = 1.084-1.920, p = 0.012). Stratification by ethnicity and vasculitis type showed no association between vasculitis and 1 K469E polymorphism under heterozygote contrast. In addition, the meta-analysis revealed a significant association between the
ICAM-1
G241R R allele and Behcet's disease (BD) (OR = 3.261, 95% CI = 1.653-6.434, p = 0.001), but not
giant cell arteritis
. Moreover, the meta-analysis indicated an association between RA and the R allele and RR+ RG genotype of the
ICAM-1
G241R polymorphism (OR = 2.014, 95% CI = 1.215-3.339, p = 0.007; OR = 2.394, 95% CI = 1.354-4.235, p = 0.003). This meta-analysis suggests that the K469E polymorphism is associated with susceptibility to vasculitis, and that the G241R polymorphism is associated with susceptibility to BD and RA.
