Although cardiac papillary fibroelastomas are benign tumors, they have the potential to cause life-threatening complications such as stroke and myocardial infarction and may even lead to sudden cardiac death. We report a case of surgical treatment for a papillary fibroelastoma of the aortic valve. The patient was a 62-year-old woman who was asymptomatic for cardiac papillary fibroelastoma. Echocardiography demonstrated a mobile, rounded mass on the left coronary cusp of the aortic valve. The patient underwent surgical excision, had an uneventful recovery, and was discharged 10 days after the operation. Pathology examination confirmed papillary fibroelastoma. Surgical excision of the tumor is recommended for patients with symptoms, but the treatment of asymptomatic patients is controversial. We believe that surgical treatment should be performed if the tumor is mobile.

1. Stimulation causes Antedon to swim by means of alternate oral bending and dorsal stroke of the arms. Two arms of a given ray move alternately so that while one is executing the aboral stroke its mate is flexing ventrally. This implies reciprocal inhibition. 2. Recriprocal inhibition between the two arms of an isolated ray can be abolished by the use of either strychnine or nicotine. 3. Coordination between the rays is referable to the conducting properties of the nervous pentagon which connects the five rays. In this system an impulse loses in effectiveness as it passes from the point of origin. 4. When Antedon is made to rest oral face down on the floor of an aquarium, oral flexion of all the rays, swimming movements, and righting result. Antedon is therefore negatively stereotropic with reference to its ventral side. 5. Excitation of the dorsal cirri results in aboral bending of all the rays. Stimulation of the cirri inhibits ventral flexion to the extent of preventing righting movements while on the other hand stimulation of the ventral surface inhibits the grasp reflex of the cirri. Thus oral and aboral sides of Antedon exhibit dynamic symmetry although structurally dissimilar.

1. At constant temperatures, and within physiological limits, changes of pH in either direction from the neutral point result in immediate increases in speed of movement of Paramecium. 2. These increases are temporary. In 30 to 45 minutes a minimum of speed is reached. This is followed by a period of recovery lasting about an hour. Finally an equilibrium is found. With inorganic acids (HCl or H(2)SO(4)) the final speed after 3 or 4 hours is that characteristic of prolonged exposure to pH 7.0; on the other hand, 3 or 4 hours after the application of either valeric or carbonic acid, speed is proportional to the [H;prime;] of the external and, probably, of the internal medium. 3. These facts become explicable if we assume that the ionization of an ampholyte superficially localized is essential for the execution of the ciliary stroke. Valeric and carbonic acid, in time, demonstrably penetrate the cell. As a working hypothesis we postulate that internal increase of the [H'] accelerates the rate at which this ampholyte is synthetized; but without actually penetrating the cell, hydrogen or hydroxyl ions in the external medium could also increase the degree to which this ampholyte dissociates. 4. Increased ionization of a fixed quantity of ampholyte and an increase in the rate of its production are in these experiments practically indistinguishable. Hence we assume that immediate and temporary increases of speed resulting from any change of pH, as well as final and permanently higher speed levels manifest only after prolonged exposures to organic acids, involve essentially the same mechanism.

More than 50% of cerebrovascular accidents are caused by atherosclerotic disease at the carotid artery.1 Carotid artery stenosis may be detected by the standard method of duplex ultrasound (DUS), while calcifications can be detected by panoramic radiographs.2-4 Panoramic radiographs may be used as a selective tool, reducing health costs by identifying individuals at risk of stroke or myocardial infarction.5-7 The present study sought a correlation between laboratory findings/behaviors and panoramic radiographs and DUS images, thus determining the value of the panoramic radiographs' findings.

Although the causes of stroke are diverse, thromboembolism due to a mobile aortic thrombus is rare. We describe a surgical case of acute massive pulmonary embolism after critical cerebral infarction associated with a mobile ascending aortic thrombus in a 52-year-old woman. Concomitant surgical removal of the aortic thrombus and pulmonary embolectomy was performed successfully, and the patient has been stable without recurrent thromboembolic complications after 18 months of follow-up.

Ischemic/reperfusion brain injury (IRI) is a very severe event with the multiple etiopathogenesis. Ischemic preconditioning (IPC) is an important phenomenon of adaptation of CNS to subsequent ischemia. An altered cross-talk between intracellular calcium stores is presumed in the mechanisms of ischemic damage/protection. We show here that IRI leads to the inhibition of mitochondrial respiratory complexes I and IV, however due to the excess of their capacities, the mitochondrial Ca(2+) uptake rate is not significantly depressed. IPC acts at the level of both initiation and execution of IRI-induced mitochondrial apoptosis and protects from IRI-associated changes in integrity of mitochondrial membranes. IPC also activates inhibition of p53 translocation to mitochondria. Inhibition of the mitochondrial p53 pathway might thus provide a potentially important mechanism of neuronal survival after ischemic brain damage. In addition, IRI initiates a time dependent differences in endoplasmic reticular (ER) gene expression of the key UPR proteins at both the mRNA and protein levels. Moreover, gene expression of the UPR proteins is affected by preischemic treatment by the increased expression of Ca(2+) binding protein: GRP 78 and transcriptional factor ATF6 in reperfusion times. Thus, IPC exerts a role in the attenuation of ER stress response, which might be involved in the neuroprotective phenomenon of ischemic tolerance. Hippocampal cells respond to the IRI by the specific expression pattern of the secretory pathways Ca(2+) pump (SPCA1) and this pattern is affected by preischemic challenge. IPC also incompletely suppresses lipo- and protein oxidation of hippocampal membranes and leads to partial recovery of the ischemic-induced depression of SPCA activity. The data suggests the correlation of SPCA function with the role of secretory pathways (Golgi apparatus) in response to preischemic challenge. Documented functional alterations of mitochondria, ER and Golgi apparatus put light into the understanding of cross-talk between intracellular Ca(2+) stores in cerebral ischemia and ischemic tolerance and might suggest for possible targets of future therapeutic interventions to enhance recovery after stroke.

This article presents the case of a 50-year-old asymptomatic man whose panoramic radiograph revealed calcium deposits within the left internal carotid bifurcation region. Subsequent duplex ultrasonic examination indicated unilateral low-grade carotid arterial stenosis, a condition associated with a significant risk of stroke, which had not been identified previously. The findings on the panoramic radiograph prompted appropriate and potentially lifesaving treatment. Dentists who are well-versed in diagnosing calcified plaques on panoramic radiographs can play a major role in the early referral and treatment of undiagnosed asymptomatic patients.

We recruited 560 unrelated patients with ischemic stroke and 153 unrelated controls to undertake a genetic analysis for association between the NOS3 gene and ischemic stroke. All the subjects were Chinese of Han descent. Because the NOS3 gene spans about 21 kb of DNA and contains 26 exons, we selected a single nucleotide polymorphism (SNP) rs3918181, an A to G base change located in intron 14 of the gene, as a DNA marker. PCR-based restriction fragment length polymorphism analysis was applied to genotype rs3918181 (RsaI site). The chi-square (chi(2)) goodness-of-fit test showed that the genotypic distributions of the marker were not deviated from Hardy-Weinberg equilibrium in both the patient group (chi(2) = 0.166, p = 0.684) and the control group (chi(2) = 0.421, p = 0.517). The cocaphase analysis showed allelic association of rs3918181 with ischemic stroke in males (chi(2) = 4.04, p = 0.044, OR = 1.43, 95% CI 1.01 approximately 2.02) and frequency of allele A was significantly higher in male patients than male control subjects. The chi(2) test revealed genotypic association between rs3918181 and ischemic stroke in males (chi(2) = 4.26, df = 1, p = 0.039, OR = 1.61, 95% CI 1.02 approximately 2.53) but not in females. The present work suggests that rs3918181 is associated with ischemic stroke in male patients. This finding gives further evidence in support of the eNOS association with ischemic stroke in the Chinese population.

Democrats and Republicans have turned to the concept of "high-risk pools" to provide health care for those Americans who face the dual challenge of uninsurance and serious health difficulties. Under the Patient Protection and Affordable Care Act (PPACA), these "high-risk" individuals will receive extensive help and regulatory protections, in concert with a new system of health insurance exchanges. However, these federal provisions do not become operational until 2014. As an interim measure, PPACA provides $5 billion for temporary, federally funded high-risk pools, now known as the Pre-Existing Condition Insurance Plan (PCIP). This analysis explores the adequacy of such funding. Using 2005/06 data from the National Health and Nutrition Examination Survey (NHANES), we find that approximately 4 million uninsured Americans have been diagnosed with emphysema, diabetes, stroke, cancer, congestive heart failure, angina, or a heart attack. To provide adequate health care for uninsured individuals with chronic diseases, the federal PCIP appropriations would need to be many times higher than either Democrats or Republicans have proposed.
