The hemorheologic properties are obviously abnormal in patients with cor pulmonale (CP), presenting as high hematocrit (HCT), hyperviscosity and hyper-viscoelasticity. Twenty patients suffering from CP with polycythemia were randomized into either an isovolemic hemodilution (IHD) group or control group. In the IHD group, 10 patients were treated by routine therapy combined with IHD. The other 10 patients in the control group were treated by routine therapy only. The results demonstrated that after treatment in IHD group, all viscoelasticity property parameters except eta p were decreased significantly. Oxygen delivery, oxygen transport capacity, and stroke volume and cardiac output were increased. Mean pulmonary artery pressure and total pulmonary resistance were decreased. However, in the control group only HCT and eta 0.512 were decreased after treatment. The viscosity and eta' of the whole blood in IHD group were decreased more markedly than those in the control group. IHD was well tolerated in most of the patients. There were no complications or side effects. Thus, IHD is a safe, effective and acceptable therapy, and it may play a role in the treatment of cor pulmonale.

After making theoretical analysis, we detected the relationship among the maximal tension of ventricular contraction, left ventricular end-systolic pressure and stroke volume, and then derived a new and simple method of evaluating left ventricular contractility, the slope of the left ventricular end-systolic pressure-volume relation (Emax). Twelve mongrel male dogs were used to test this method. The results show that the value of Emax (0.60 +/- 0.16 kPa/ml) coming from this simple method is very close to that from complicated method; this method has good repetitiveness and is very sensitive to the variation of myocardial contraction. Using the method for evaluating left ventricular contraction after ischemia, we found the Emax more cogent than other hemodynamic parameters. Therefore, this method is an efficient method of evaluating left ventricular contraction.

Few papers on alteration of whole blood viscoelastic properties in patients with congestive heart failure (CHF) have been reported. In this study viscoelastic property parameters were measured in 63 consecutive patients with rheumatic heart disease and CHF. All subjects were divided into four groups according to NYHA classification of heart function. There were 14 class I, 16 class II 20 class III and 13 class IV. Contraves low-shear-30 rheometer was used for measuring the viscoelastic parameters. The results demonstrated that there was no significant difference of these parameters between class I and class II, that the apparent viscosity, AI and eta' in class III were higher than those in class I, and that eta 0.512, eta 5.96 AI, eta', eta" and G' in class III were higher than those in class II. All viscoelastic parameters but eta p in class IV were markedly higher than those in other groups. However, there were no statistically significant difference of eta p among all subjects and nor significant difference of these parameters between male and female were noted. There was a negative correlation between viscoelastic parameters and heart function parameters by pulse Doppler echocardiography (peak flow velocity, stroke volume and cardiac output). The results also showed a significant direct correlation between eta 0.512 AI, and eta" and the heart function parameters mentioned above.

We studied the positive inotropic effect of the novel cardiotonic agent, vesnarinone on open chest and intact anaesthetized dogs hearts. The results indicate that vesnarinone has a dose-effect relationship on increasing the contractile force, LV dp/dt max and stroke volume, and it shortens the ejection time 21%. No effect on blood pressure and heart rate was noted. Vesnarione is more effective than amrinone in terms of positive inotropic effect.

Adherence of erythrocytes to human umbilical vein endothelial cells (HUVEC) in patients with cerebral thrombosis and transient ischemic attack (TIA) were quantitatively studied in the flow chamber system, compared with that in healthy subjects. We found: (a) The adherence of erythrocytes to HUVEC in patients with cerebral thrombosis and TIA increased, compared with that in healthy subjects; (b) The adhesion of erythrocytes to HUVEC in patients with TIA was not significantly different from that of patients with cerebral thrombosis. These suggested that the increased adhesion of erythrocytes to endothelial cells might play an important role in the pathologic process of ischemic stroke.

For further understanding the changes in immune function of the patients with hemorrhagic stroke, we investigated the activity of interleukin-2 (IL-2) in the peripheral blood T lymphocytes from 32 patients. The results showed that the levels of IL-2 in the patients were lower than those of the healthy controls, but no significant difference was observed between the cerebral hemorrhage group and subarachnoid hemorrhage group. It suggests the cellular immune function of patients at the acute period is low. We consider that IL-2 might be of use as a kind of therapeutic measure to modulate patients' immune function and treat the diseases.

This study sought to understand the mechanism for the increased adhesion of leucocytes and endothelial cells in ischemic stroke. 20 patients with acute cerebral thrombosis and 20 healthy subjects as controls for expression of CD11a and CD11b (adhesion molecules on surface of leucocytes) were tested in vitro by flow cytometry (FCM) method. The results showed that compared with the control group, the patient group had significantly higher rates for expression of CD11a on monocytes, granulocytes and lymphocytes (P < 0.05). The CD11b expression in the patient group was positively elevated on monocytes and granulocytes (P < 0.05), but it was of lower positive rate on lymphocytes and no statistical difference was noted between the patient and control groups. These indicate that the expression of CD11a and CD11b on leucocytes increases in cerebral ischemic damage; thus adhesion of leucocytes and endothelial cells obviously increases. This change may aggravate post-ischemic delayed neuronal death.

Male SD rats(n = 29) were subjected to 2 hours' middle cerebral artery occlusion and were killed at various times of reperfusion (0.5-48 hours). The histological features of neuronal changes, the macrophagic response and the relationship between them were investigated. The neurons underwent acute changes(shrinking or swollen), and the pyknotic apoptotic neurons were occasionally seen within 3 hours. The number of apoptotic neurons increased with recirculation time. The scattered red neurons and ghost cells were first found at 6 hours of reperfusion. Many apoptotic neurons, apoptotic bodies were detected in the penumbra of ischemic core at 48 hours. Lectin histochemistry with GSI-B4 was performed to observe the macrophage. The results showed the active microglia and macrophage from meninges presented at 12 hours. The macrophages from parenchyma vessels appeared at 24 hours. Reactive microglia and extrinsic macrophage aggregated into the band of macrophage which phagocytized the apoptotic neurons. The results indicate that, firstly, it is important to prevent the neuronal apoptosis and reduce the volume of infarct in stroke patients; secondly, irreversible neuronal lesions simultaneously stimulate microglial activation and the presence of extrinsic macrophages from meninges was earlier than the appearance of the macrophages from parenchyma blood-monocytes. The authors also suggest that the data on the neuronal changes over time after ischemia and reperfusion be useful reference materials for selecting the most timely medication to treat cerebral infarction.

The objective of this study was to examine the efficacy of milrinone in chronic congestive heart failure. In this randomized, double-blind, placebo-controlled trial, 30 patients with class III or IV chronic congestive heart failure were randomly assigned to receive milrinone (n = 15) or placebo (n = 15). The results showed that in the milrinone group, stroke volume increased from 38.50 +/- 5.34 to 64.93 +/- 6.46 ml (P < 0.001), cardiac output increased from 3.39 +/- 0.42 to 5.28 +/- 0.55 L/min (P < 0.001), cardiac index increased from 2.01 +/- 0.42 to 3.21 +/- 0.57/min/m2 (P < 0.001) and ejection fraction increased from 0.23 +/- 0.05 to 0.35 +/- 0.06(P < 0.001). In the placebo group, no hemodynamic effects were found. Milrinone favourably influenced the symptoms and heart function in 11/15(73.3%) patients. In the placebo group, only 4/15(26.7%) patients had such response (P < 0.05). These data indicate that milrinone is effective for improving the hemodynamics status and heart function in patients with chronic heart failure.
