Though major differences exist in subcategory mortality levels, cardiovascular disease remains a leading cause of death among both Asian Chinese and Westerners. This paper examines the possible relationship between cardiovascular mortality and biochemical, diet and lifestyle factors based on two surveys in China. Statistically significant associations indicate five variables negatively correlated: molybdenum, oleic acid, liquor consumption (males), legumes, and age at first pregnancy with ischemic heart disease; molybdenum, oleic acid (females) and age at first pregnancy with hypertensive heart disease; and legumes and age at first pregnancy with
stroke
. Five variables were positively correlated: triglycerides and herpes antibodies with ischemic heart disease; salt and phosphorus (females) with hypertensive heart disease; and only albumin (males) with
stroke
. Some findings confirm those observed in the West (salt, triglycerides, herpes, legumes, oleic acid, and liquor), but molybdenum and age at first pregnancy have not been emphasized previously. Still others significant in the West have not been observed here, such as cholesterol and smoking.

No standardized assessment of aphasic symptoms for
stroke
patients in the acute phase has as yet been published in the German language. Current test batteries such as the
AAT
are too time-consuming and cannot be performed on severely impaired patients. A short test for the examination of aphasic and dysarthric symptoms has been developed which contains 7 subtests in three main areas: spontaneous speech, comprehension and planning of movements, speech and language abilities. Since abilities in acute cases may only be functionally impaired and often only detectable after stimulation, standardized stimulation is applied in most of the subtests. In this way it is possible to test acute
stroke
patients for aphasic and dysarthric symptoms. The course of recovery in a typical case is described.

Researchers analyzed 1983 vital statistics for Taiwan using the life table analysis and calculating the potential years of life lost (PYLL) to identify life shortening features of several major causes of death and examine the preventability of premature mortality. Life expectancy for males was 69.9 years whereas for females it was 75.1 years. these corresponding life expectancies for Japan were 73.8 years and 79.6 years and 71.7 years and 78.7 years for the US. Between 1950-1983, life expectancy at birth in Taiwan increased 17.5 years for males and 19 years for females. In 1983, the cumulative total of PYLL before age 70 for males was 737,205 compared to 353,780 years for females. Thus loss of productivity of males was 2 times that of females. Most of this loss was a result of accidents. In fact, they contributed to 37.2% of PYLL for males and 24.9% of PYLL for females. Even though accidents were also the leading cause for the most PYLL in Japan and the US (17.4% and 8.7% respectively in Japan and 25% for both sexes combined in the US), the magnitude was considerably lower than that of Taiwan. Further the type of accidents males were more likely to die from were motor vehicle accidents (18,8% vs. 18.4% for home and workplace accidents). On the other hand, women were more likely to die from home and workplace accidents(14% vs. 10%). The 2nd major cause for the most PYLL was all cancers, except liver cancer, (15.9% for males and 20.4% for females). The 3rd major cause for the most PYLL for males was liver disease (liver cancer and cirrhosis of the liver) (9.6%) while for females it was
stroke
(8.7%). Further suicides contributed to 6.5% of PYLL for females. IN conclusion, Taiwan should place accident prevention as a high priority since it needs limited resources and is more achievable than that of cancer elimination. The next level of preventive efforts should include
stroke
and suicide.

The purpose of this study was to determine whether segment lengths measured from the right ventricular inflow and outflow tract regions of the right ventricle would accurately reflect true volume changes of the right ventricle and to determine the response of the right ventricle to afterload increases induced by both constricting the pulmonary artery (
PAC
) and embolizing the pulmonary circulation with glass beads (GBE). Three excised hearts were instrumented with segment-length crystals attached to the inflow and outflow tract regions, and saline was instilled into a balloon implanted inside the right ventricular cavity. The experiments showed a high correlation (r greater than or equal to 0.90 in all cases) between static segment lengths and volume instilled. In open chest, open pericardial canine experiments, vena caval occlusion reduced end-diastolic segments lengths and right ventricular systolic pressure consistent with a reduction in right ventricular end-diastolic volume. In a separate group of animals, volume loading with dextran increased inflow and outflow end-diastolic segment lengths and increased cardiac output. In two further groups of animals, one of which was pretreated intravenously with propranolol (Inderal), both forms of pressure overload increased end-diastolic lengths in both regions. However, GBE increased right ventricular
stroke
work compared with
PAC
. We conclude that end-diastolic segment lengths reflect true volume changes of the right ventricle. Furthermore, during acute pressure overload, the right ventricle dilates to compensate for the afterload change. However, ventricular function is better maintained after GBE.

To obtain further insights into the rehabilitation outlook in different baseline conditions, a comparison was made of data and treatment outcomes in 92 patients having aphasias of different etiology and disease duration. Rating as "improved" was contingent on significant changes in at least 2 subtests of the
AAT
(Aachener Aphasietest), which is a fairly stringent requirement. This criterion was met by some 35% of the patients overall, independent of their age; the percentage increased to 45% in case of treatment onset during the first year, while in later onset, significant improvement was achieved in 15% only. Aphasic syndromes secondary to hemorrhage, trauma, tumors, or infections, usually are less severe than the predominant classical syndromes, i.e. Global, Broca and Wernicke aphasia, caused by ischemic
stroke
. CT lesion size did not yield any definite information. Minor improvements below the required significance levels, and, above all, gains in communication performance, i.e. further consolidation, could be achieved by appropriate management even after the crucial initial year, which, however, does not imply any fundamental changes in the rehabilitation prognosis as such.

The crumbling of the nation's public health care system is nowhere better illustrated than in St. Louis, Mo., where the city administration recently removed its ailing municipal hospital and clinics from public control in one quick, autocratic
stroke
. Helping the mayor "rescue" St. Louis "out of the health care business" were the city's business elite and the highly paid managers of National Medical Enterprises, one of the nation's largest for-profit health care corporations. The St. Louis story is just one local chapter of an unfolding tragedy which finds the poor and uninsured increasingly shut out and shunted aside as the nation's public and voluntary health sectors fall prey to what is euphemistically called "privatization."

This review elucidates the importance of healthy dietary and lifestyle habits to reduce morbidity and mortality associated with coronary heart disease (CHD),
stroke
and cardiovascular diseases. Given published evidence of the poor compliance, increased cost, and decreased benefit/risk ratios of medical therapies, individuals (and populations) are encouraged to adopt healthy life habits. The three most atherogenic dietary risk factors are saturated fat, cholesterol and obesity. Dietary patterns associated with the consumption of grains and grain-based foods predict risk of CHD independently of other life habits. Epidemiological and intervention studies elucidating the strong protective associations of grains, cereal fibers and anti-oxidant vitamins on CHD are reviewed. In summary, the consumption of grains and grain-based cereals is repeatedly associated with the ingestion of many nutrients, e.g., dietary fiber and anti-oxidants, that alter energy balance and nutrient intakes to positively affect cardiovascular health, especially when combined with healthy life habits,

Cardiovascular disease is a major cause of morbidity and mortality. Epidemiological studies indicate fish eaters are less likely to die prematurely compared with non-fish eaters. The protective properties in fish are likely to be related to its concentration of omega 3 fatty acids, calcium, selenium, vitamin D, taurine and coenzyme Q10. A high consumption of fruits and vegetables has been shown to protect against
stroke
and coronary heart disease. The presence of vitamins and minerals, as well as the complex array of non-nutrient compounds, found in fruits and vegetables would play an important role in this protection.

By 2020, non-communicable diseases including cardiovascular diseases (CVD) are expected to account for seven out of every 10 deaths in the developing countries compared with less than half this value today. As a proportion of total deaths from all-causes, CVD in the Asia Pacific region ranges from less than 20% in countries such as Thailand, Philippines and Indonesia to 20-30% in urban China, Hong Kong, Japan, Korea and Malaysia. Countries such as New Zealand, Australia and Singapore have relatively high rates that exceed 30-35%. The latter countries also rank high for coronary heart disease (CHD) mortality rate (more than 150 deaths per 100,000). In contrast, death from cerebrovascular disease is higher among East Asian countries including Japan, China and Taiwan (more than 100 per 100,000). It is worth noting that a number of countries in the region with high proportions of deaths from CVD have undergone marked declining rates in recent decades. For example, in Australia, between 1986 and 1996, mortality from CHD in men and women aged 30-69 years declined by 46 and 51%, respectively. In Japan.
stroke
mortality dropped from a high level of 150 per 100,000 during the 1920s-1940s to the present level of approximately 100 per 100,000. Nonetheless, CVD mortality rate is reportedly on the rise in several countries in the region, including urban China, Malaysia, Korea and Taiwan. In China, CVD mortality increased as a proportion of total deaths from 12.8% in 1957 to 35.8% in 1990. The region is undergoing a rapid pace of urbanization, industrialization and major technological and lifestyle changes. Thus, monitoring the impact of these changes on cardiovascular risks is essential to enable the implementation of appropriate strategies towards countering the rise of CVD mortality.

Accumulated evidence from experimental and epidemiological studies indicates that there is a low risk of degenerative diseases, cardiovascular disease, hypertension, cataract,
stroke
and, in particular, cancers in people with a high intake of fruit and vegetables. This protective effect is assumed to be associated mainly with the antioxidant activities of either individual or interacting bioactive components present in the fruits and vegetables, and with other biochemical and physical characteristics of the identified and unknown bioactive components. The implicated bioactive components present in citrus fruits include vitamin C, beta-carotene, flavonoids, limonoids, folic acid, and dietary fibre. A high intake of citrus fruits may reduce the risk of degenerative diseases.
