The study was designed to compare the frequency and severity of postoperative vomiting in paediatric out-patients receiving controlled ventilation (IPPV) or breathing spontaneously (SV) during anaesthesia for
strabismus
repair. One hundred and twenty unpremedicated children (ages 2-12 years) were studied in a randomized fashion. After intravenous induction of anaesthesia and tracheal intubation, patients breathed halothane 1-1.5 per cent inspired and N2O 66 per cent in O2 spontaneously (n = 60), or received IPPV, halothane 0.5-1 per cent, N2O 66 per cent, and pancuronium 0.05 mg.kg-1, which was reversed with neostigmine and atropine (n = 60). The incidence of vomiting with SV was 50 per cent (95 per cent confidence limits: 34.5-65.5 per cent) compared with 40 per cent (24.5-55.5 per cent) with IPPV (p greater than 0.25). Patients in the SV group experiencing emesis had longer operations than those not vomiting (mean +/-
SEM
= 1.5 +/- 0.1 vs 1.2 +/- 0.1 hours, p less than 0.005). This was not the case with IPPV. There was no correlation between age, sex, duration of surgery, or number of extraocular muscles repaired, and frequency or severity of vomiting or time to discharge. No significant advantage was afforded by IPPV over SV in the present study.

. METHODS: We measured the near
heterophoria
of 22 young adults before and after 30 minutes of reading. The reading task comprised a column of local English newsletter studied monocularly at 33 cm. One of three addition lenses (that is, 0.00 D, +1.50 D and +3.00 D) was randomly incorporated into the optical prescription. The difference in near
heterophoria
between the pre- and post-reading task was recorded. The experiment was completed on separate days for the other lens powers. RESULTS: Reading for 30 minutes with a plano lens addition (control) increased the near
heterophoria
by 3.81 +/- 0.95 prism dioptres (
SEM
) toward exo-deviation (p < 0.002). Addition of a +3.00 D lens significantly decreased the reading-induced exophoric shift to 1.36 +/- 0.55 prism dioptres (
SEM
). Similarly, a +1.50 D lens reduced the exophoric shift to 3.14 +/- 0.85 prism dioptres (
SEM
) but the difference was not statistically significant when compared with the control. CONCLUSIONS: The results showed that close work might cause eye strain via the extraocular muscles. Incorporation of plus lens into the optical correction caused a power-dependent reduction in the stress, that is, smaller exophoric shift. Whether binocular stress contributes to myopia and its response to addition lens therapy deserve further investigation.

The purpose of this study is to compose between hand scaling with abd without the calculus solvent gel (sofscale) and ultrasonic instrumentation at clinical and

level. 30 patients belonging to the age group of 17-50 year were selected. Patients selected were subjected to three different scaling modalities namely hand scaling (control), hand scaling using sofscale (Experimental quadrant A) and ultrasonic scaling (Experimental quadrant B), in three different quadrants. Case report forms were used to document the tooth sensitivity, soft tissue pain after scaling, patient preference of instrumentation, ease of calculus removal, patient comfort, soft tissue irritation, time taken for scaling, Bleeding while scaling, pre and post operative sulcus bleeding index. In addition to the clinical criteria, the teeth treated were extracted and evaluated using the scanning electron microscope to show potential effects on cemntal surfaces. No difference in tooth sensitivity was appreciated between control and experimental quadrant A. There was a higher degree of tooth sensitivity when treated with ultrasonic. Patients in control group appreciated a higher degree of soft tissue pain. Hand scaling using softscale produced a lesser amount of pain and treatment with ultrasoincs was the least painful. Most of the patients preferred ultrasonic scaling (70%) Calculus removal was easier. Hand scaling using sofscale gel results in more patient comfort when compared to hand scaling alone. There was no significant difference in patient comfort between handscaling using sofscale and ultrasonic scaling. The percentage of reduction of sulcus bleeding index showed no difference between the 3 scaling modalities
