A three and one-half year-old female who presented with orbital cellulitis and leukocoria is described. Enucleation was performed as the eye was microophthalmic, blind, and painful. Histopathologic study revealed uveitis with total retinal detachment. The etiology of the retinal detachment was indeterminable. The differential diagnosis of orbital cellulitis in children is reviewed.

Pediatric orbital pseudotumor may be associated with iritis, unlike the adult form of this disorder. However, orbital pseudotumor is seldom considered in the differential diagnosis of childhood uveitis. We report two cases of children with uveitis who were ultimately diagnosed as having orbital pseudotumor. No proptosis was noted in either child. For those pediatric patients with a persistent or recurrent uveitis and a previous negative diagnostic workup, the possibility of an orbital pseudotumor should be considered and ultrasonography, magnetic resonance imaging, and/or computed tomography performed.

Two cases of chorioretinitis were caused by polymer material from the suture (Dexon) used in squint surgery. The lesions were macula edema, granulomatous uveitis and capillary occlusions on the ocular fundus located in the operated area. To confirm the cause of this injury, an immunological investigation was performed. Macrophage block and an incomplete adjuvant were necessary to lead delayed hypersensitivity against the synthetic polymer polyglycolic acid (PGA). The mice showed a strong reaction as hyperemia and swelling on their experimental footpads after the injection of PGA. Histologically the footpad accumulated a large quantity of lymphocytes in the swollen tissue but the control footpad had no reaction.

Disorders of pigmentation can result from either an abnormal number of melanocytes, as in nevus of Ota and vitiligo, or an abnormal amount of melanin production, as in albinism. Melanin-producing cells are found in the skin, mucous membranes, uveal tract, and retinal pigment epithelium of the eye and the stria vascularis of the inner ear. Thus, many of the hereditary or congenital pigmentary disorders of the skin are associated with similar pigmentary abnormalities in the eye, such as iris heterochromia or changes in pigmentation of the fundus; however, more commonly, the associated eye finding is a defect in ocular motility, i.e., strabismus and nystagmus, suggesting a concomitant defect in neurologic development. In albinos, the observed neurologic abnormality in the visual pathway and foveal hypoplasia are hypothesized to be related directly to the lack of melanin in the pigment epithelium during development. In acquired disorders of pigmentation, in particular, vitiligo, Vogt-Koyanagi-Harada syndrome, and onchocerciasis, there is a frequent association with uveitis, suggesting an inflammatory cause for the cutaneous pigmentary changes.

A 3-year-old boy underwent secondary implantation of an iris supported intraocular lens following extraction of a unilateral congenital cataract. Eight months later, he developed a bilateral plastic uveitis which, in spite of intensive medical treatment, resulted in NPL (no perception of light) in the implanted eye and 2/60 vision in the fellow eye. Sympathetic ophthalmia possibly played a role in the pathogenesis of the uveitis.

The authors evaluated complications which appeared in 113 eyes in 66 patients treated for congenital glaucoma. The most frequent early complication connected with the surgical procedure was an anterior chamber haemorrhage (in 22 eyes). Individual cases exhibited intravitreal haemorrhage (5 eyes) and uveitis (2 eyes). Among the late complications prevailed complicated cataract (in 30 eyes), less frequently nystagmus (11 eyes), intraocular infection (9 eyes), squint (17 eyes) and internal hemophthalmos.

Clinical findings, course, treatment, and complications of intermediate uveitis in children is analyzed in a series of 60 patients. While generally a chronic indolent disease, in the pediatric age group acute anterior uveitis is the presenting symptom rather than the milder symptomatology characterized in the adult with a similar syndrome. The etiologic evaluation of these patients was unrewarding. The great majority of patients improved following the use of periocular corticosteroid injections and retinal cryopexy, and immunosuppressive agents were infrequently employed in the management of the disorder. Cataract formation, secondary glaucoma, band keratopathy, vitreous hemorrhage, and papillitis were seen as complications. A treatment algorithm is presented.

BTX injection has been used for 11 years by 292 ophthalmologists in 8,854 patients aged three months to 90 years in a variety of eye muscle and eyelid disorders. No systemic toxic reaction has occurred, local complications are few, and visual loss has not occurred in any case. In blepharospasm and hemifacial spasm BTX appears to fill an important need, since no other drug is reliably effective and since surgical interventions have substantial side effects. Strabismus cases with active uveitis, hypotony, previous detachment surgery, active thyroid eye disease, and recent paralytic strabismus are often poor candidates for surgical intervention. Some patients in each of these categories were treated effectively and safely by BTX injection. Surgery is clearly the preferred treatment modality in large angle deviations, in chronic paralytic strabismus, in cases where diplopia for a month or two from injection would incapacitate the patient, in nystagmus, in oblique muscle disorders and A-V patterns, where muscles have been misplaced and where restrictions to alignment have been created by disease or prior surgery. Side by side comparisons of surgery and injection in congenital esotropia and in concomitant strabismus of 50 PD or less should result in further clarification of treatment choices as to effectiveness, side effects and cost. BTX is presently available only to clinical investigators using the drug under research protocols.

A 13-year-old black female was referred with unilateral granulomatous uveitis and orbital inflammation of sudden onset. Hematologic, serologic, bacteriologic, and ultrasonographic studies along with high resolution CT scans of the orbits were employed to determine the diagnosis and appropriate treatment. Subconjunctival corticosteroid injection containing air prior to referral obfuscated the ultimate diagnosis of pseudotumor. The Pediatric Infectious Disease service delayed definitive treatment with systemic steroids. Differential diagnosis of granulomatous uveitis with orbital inflammation are discussed. CT scan has significantly advanced the diagnosis and management of orbital pseudotumor.
