The vascularization of the anterior section of the bulb depends partially on the anterior ciliary vessels. Circulatory disorders of the anterior
uvea
have been detected following conventional
squint
surgery in which several of these vessels were severed, even though there were no clinically detectable symptoms. It therefore makes good sense to spare the muscular and anterior ciliary vessels as far as possible, in order to preserve the muscles themselves and the anterior section of the bulb (a) by careful surgery and (b) by adopting surgical procedures in which the vessels are not sacrificed. In the final analysis, complete preservation of the vessels can only be achieved using the operating microscope.

Amniocentesis has been previously reported to cause damage to the anterior segment of the eye with subsequent loss of vision. We report the first case of amniocentesis in which the needle penetrated the globe and punctured the retina and the first case in which vision was retained. At four months of age the injured eye was found to have a perilimbal conjunctival bleb, a scarred retinal hole, unilateral myopia, and mild amblyopia. A prospective ophthalmological study was done in 86 out of a total of 100 Swedes with established thalidomide embryopathy. Forty-six (54%) of all examined individuals had eye findings, which made the eye the second most commonly affected organ in thalidomide embryopathy only surpassed by upper limbs (81%). Forty-three patients (50%) had ocular motility defects, mostly incomitant

. Facial palsy and abnormal lacrimation each occurred in 17 (20%) individuals. One patient had coloboma of the
uvea
and optic disc and another two had coloboma of the optic disc. Infrequent anomalies were microphthalmos, congenital glaucoma, lipodermoid, and large refractive errors. The observed ocular motility defects, facial palsy and abnormal lacrimation occurred with early induced defects in thalidomide embryopathy, but not with isolated late occurring anomalies. This suggests that thalidomide exerts its effects on the development of these structures early in the teratogenic period, probably mainly during the fourth week of development.

Between 1982-1996 nineteen cases with retinoblastoma were hospitalized. Thirteen of them were one to three years old. The study of their origin shows fourteen cases by rural medium and five cases by city medium, seventeen cases had an unilateral evolution and two cases a bilateral evolution. Clinical manifestations which requested ophthalmological exam were: secondary glaucoma in twelve cases, leukochoria in ten cases, decreased of visual acuity in eight cases, inflammation of
uvea
in three cases,
strabismus
in two cases, buphthalmia in one case. On insist belated (100 late) presentation by the physician. Computer tomography shows the area of tumour and its extension at the neighbour structures.

Intraocular tumours may be benign or malignant. The latter are more numerous, and endanger not only vision but life as well. Two of them deserve special attention: melanoma malignum oculi in adults and retinoblastoma in children. Melanoma malignum may arise from all three areas of the
uvea
: the iris, the ciliary body and the choroid. The more malignant growths are those which are situated closer to the posterior pole. Histologically the epitheloid cell-type of melanoma is more malignant than those containing only spindle cells. Their treatment depends on the size: in the case of large tumours enucleation is required, while for the smaller ones, radiation therapy can be applied. Retinoblastoma is most common in children of 1-2 years of age. It has familial and sporadic forms. Sixty-seven percent of the inherited-type cases are bilateral. An early symptom in small children is
strabismus
. A white tissue mass growing into the vitreous is seen on the fundus. A diagnostic feature that can be detected by ultrasound examination is calcification. The tumour may also present intracranially, therefore CT of the skull should be performed in each case. Histologically the tumour contains malignant neuroepithelial cells, which may form a rosette. In the case of large tumours the treatment is enucleation; in bilateral processes the bulbus with the larger mass is removed and the other eye is treated with radiation therapy. In both cases chemotherapy is used according to a prescribed schedule. Metastases to the eye occur most frequently from carcinomas of the breast, lungs or gastrointestinal tract. These are treated with radiotherapy, chemotherapy and hormone therapy. Primary intraocular lymphoma often occurs bilaterally, and may be accompanied by primary lymphoma of the central nervous system (CNS). Some benign tumours are found by chance on routine eye examinations, others due to subjective and objective symptoms.
