Anterior segment ischemia changes can occur without detachment of any muscles. The most common cause of such ischemic changes of the anterior segment is the removal of too many rectus muscles in one operation. Twenty dog eyes and eight monkey eyes were subjected to the disinsertion and detachment of various combinations of extraocular muscles. They were sacrificed at intervals from 30 to 90 days. During the observation period, they were observed for gross and slit lamp changes. The enucleated eyes were studied microscopically for signs of ischemic and necrotic changes. Two patients who were studied, observed, and treated for anterior segment ischemia following muscle surgery are described. The changes which occur after muscle surgery are extensive and include
corneal edema
, cataract, chemosis, corneal changes, decreases in intraocular pressure, decreases in outflow or glaucoma and frank necrosis. The variables which lead to this reaction is described in detail. Also, some unanswered queries, such as the duration of the reaction and the time interval of the reaction after multiple muscle surgeries, are discussed.

Sixty-eight soft cataracts were removed utilizing the O'Gawa double-bore aspirating-irrigating cannula. The postoperative visual acuity was found to be 20/100 or better in 83% and 20/40 or better in 59% of the 64 cases in which visual acuity was obtainable. In congenital cataracts, associated ocular defects such as
strabismus
or nystagmus reduced the visual prognosis. All cases of traumatic cataract had associated defects, usually a corneal laceration, however, visual improvement was noted in 81% of the cases. Significant postoperative
corneal edema
was not seen. Complications attributable to the operation included: rupture of the posterior capsule, insufficient removal of lens cortex, and pupullary block glaucoma. Discission of the posterior capsule was necessary in 26% of the eyes with an intact capsule. Retinal detachment followed the operation in three instances. The O'Gawa double-bore cannula is recommended for the removal of soft cataracts.

In a study of over 2,000 consecutive live births, 243 newborns (12%) suffered birth trauma to the eye and its adnexa. Two hundred twenty-one cases (11%) had multiple retinal hemorrhages. Severe ocular accidents by forceps delivery were seen in the form of hyphema, Purtscher's retinopathy,
corneal edema
, facial palsy, and corneal abscess. Purtscher's retinopathy and corneal abscess are rare birth injuries that have not been reported so far in newborn babies.

We report a case of traumatic left sixth nerve palsy in a 32-year-old man who developed anterior segment ischemia after left medial rectus muscle recession and partial tendon transfer of the vertical rectus muscles to the lateral rectus, augmented by lateral fixation sutures. The
corneal edema
and anterior chamber reaction cleared upon removal of the lateral fixation sutures and institution of steroids and cycloplegics. The possible role of lateral fixation sutures in anterior segment ischemia is discussed.

In children with congenital glaucoma the functional long-term result is often disappointing even if the intraocular pressure is well controlled. The reason for this discrepancy is attributed to amblyogenic factors responsible for interfering with normal visual development. These amblyogenic factors are

A 46-year-old male patient was referred to our clinic with complaints of diplopia and esotropia in his right eye that developed after a car accident. The patient had right esotropia in primary position and abduction of the right eye was totally limited. Primary deviation was over 40 prism diopters at near and distance. The patient was diagnosed with sixth nerve palsy and 18 months after trauma, he underwent right medial rectus muscle recession. Ten months after the first operation, full-thickness tendon transposition of the superior and inferior rectus muscles (with Foster suture) was performed. On the first postoperative day, slit-lamp examination revealed
corneal edema
, 3+ cells in the anterior chamber and an irregular pupil. According to these findings, the diagnosis was anterior segment ischemia. Treatment with 0.1/5 mL topical dexamethasone drops (16 times/day), cyclopentolate hydrochloride drops (3 times/day) and 20 mg oral fluocortolone (3 times/day) was initiated. After 1 week of treatment,
corneal edema
regressed and the anterior chamber was clean. Topical and systemic steroid treatment was gradually discontinued. At postoperative 1 month, the patient was orthophoric and there were no pathologic symptoms besides the irregular pupil. Anterior segment ischemia is one of the most serious complications of
strabismus
surgery. Despite the fact that in most cases the only remaining sequel is an irregular pupil, serious circulation deficits could lead to phthisis bulbi. Clinical properties of anterior segment ischemia should be well recognized and in especially risky cases, preventative measures should be taken.
