Currently used cardiovascular drugs such as
nicotinamide
, strophanthin, corglycon, curantyl, cavinton, papaverin hydrochloride, nicotinic acid, xantinole nicotinate, isoptin, parmidin and halidor were studied by the program of antiviral drug screening. The majority of them (9 out of 11) were shown to have antiviral activity which was rather individual by its specificity and level. The three drugs papaverin hydrochloride, strophanthin and corglycon proved to be the most promising. Their antiviral activity was confirmed on a model of herpes infection in mice. The paper discusses whether the phenomenon discovered in the official drugs is important in the therapy of somatic patients.

Vitamin B
(12) deficiency is a well-known cause of recurrent aphthous stomatitis (RAS). However, the mechanism by which this deficiency causes the
stomatitis
is not well understood. Imerslund-Grasbeck syndrome (IGS) causes vitamin B(12) deficiency and proteinuria due to a defect in the vitamin B(12) receptor. We sought to determine whether the RAS observed in IGS patients is associated with neutrophil dysfunction. We report 3 infants with vitamin B(12) deficiency due to IGS, who presented with borderline or normal hemoglobin concentrations, RAS, and a neutrophil function defect. All 3 patients were homozygous for a splice site mutation affecting exon 4 of the AMN gene. A direct correlation was observed between low serum vitamin B12 levels and defective neutrophil function (low chemotaxis and elevated superoxide production) in the patients.
Vitamin B
(12) therapy led to an immediate resolution of aphthous stomatitis and full correction of neutrophil function. We demonstrated that serum vitamin B(12) deficiency is associated with a neutrophil chemotactic defect and RAS in IGS patients. We suggest that the RAS observed in these patients is due to this defect.

A 67-year-old woman with chronic alcoholism was referred to our hospital with a 3-month history of progressive weakness in all four extremities and a 5-day history of hoarseness. On admission, she presented with glossitis and laryngitis. Neurological examination revealed weakness in all extremities and nerve conduction studies showed polyneuropathy. She was diagnosed with thiamine deficiency and administered intravenous multivitamins, including thiamine, pyridoxine and mecobalamin. Although the weakness in her extremities was ameliorated after treatment, her glossitis and laryngitis did not improve and psychosis subsequently occurred. After administration of
nicotinamide
, her psychosis and laryngitis dramatically improved. She was diagnosed with pellagra after detection of decreased serum levels of niacin and thiamine. Weakness may have been due to thiamine-deficient neuropathy or multiple vitamin deficiencies, as it improved with thiamine, pyridoxine and mecobalamin therapy. Because of lack of response to thiamine therapy, we concluded that the psychosis and laryngitis were caused by pellagra. Although

and glossitis in pellagra has been well documented, laryngitis and hoarseness appear to be very uncommon conditions. Differences in the turnover rates or energy requirements between the gastrointestinal tract and larynges may be a reason for the rarity of laryngeal involvement in pellagra. We should be vigilant to hoarseness, caused by laryngitis, which can be the predominant symptom in pellagra.
