The present study summarizes information on toothpaste composition as supplied by the manufacturers. The survey covered 48 items, virtually all toothpastes offered for sale in Finland. It was concluded that the toothpastes are not entirely safe to use, because almost 50% of the products studied contained a total of some 30 compounds widely recognized as allergens. According to the literature, the most common allergens in toothpastes are flavours (e.g., cinnamic aldehyde, cinnamon oil and peppermint) and preservatives. Symptoms include

Occupational allergic contact dermatitis caused by (meth)acrylates is common in dental personnel, whereas dental acrylic fillings and crowns have rarely been reported to cause problems in dental patients. Here we report on a 48-year-old woman who developed gingivitis,
stomatitis
, and
perioral dermatitis
after a temporary crown made of restorative, two-component material had been inserted. The manufacturer stated that the temporary crown base paste and catalyst contained three (meth)acrylates, namely, a proacrylate, which is a modification of 2,2-bis[4-(2-hydroxy-3-methacryloxypropoxy)phenyl]propane (BIS-GMA); a tricyclate, which is a saturated, aliphatic, tricyclic methacrylate; and urethane methacrylate. The manufacturer refused to give more exact information on the (meth)acrylates. Patch testing revealed that the patient was highly allergic to BIS-GMA, other epoxy diacrylates, and (meth)acrylates, as well as to the base paste and catalyst of the temporary crown. Accordingly, it was concluded that the allergic reaction was caused by BIS-GMA, or a cross-reacting (meth)acrylate, or other (meth)acrylates in the temporary crown.

Delayed-type hypersensitivity reaction or type IV allergic reaction can cause different oral manifestations. They can be localized or diffusely visible on oral mucosa and usually appear 24-72 hours after antigen input. The antigens that cause this type of reaction are mostly external, such as contact allergens (particularly metals) and drugs. , burning mouth syndrome, lichenoid reaction and orofacial granulomatosis. The most important part of diagnosis is the use of patch testing that indicates contact allergic reaction to an allergen. The results of patch testing have shown that the most common proven allergens are gold, nickel, mercury, palladium, cobalt, acrylate, etc. Although connection between specific clinical manifestations and positive patch test results was not always found, patch testing is necessary to prove contact hypersensitivity. Therefore, in patients with oral symptoms, allergic hypersensitivity to dental components has to be considered.
