Fourteen cases of gastric cancer, 9 cases of gastric polyp and 4 cases of
gastric ulcer
were studied on their tissues lipid-chemically with the aim to clarify the biochemical differences between malignant and benign growth of the human gastric tissue. Tissues were collected by surgical operation or a biopsy together with the normal tissues surrounding the lesions. One part of each tissue was subjected to histologic examination and the other part to lipid analyses by means of TLC and GLC. The total lipid content was dcreased in 71% of gastric cancerous cases and in 75% of both cases of gastric polyp and
gastric ulcer
as compared with the respective control tissues. The phospholipid fatty acid content of the lesions was decreased in 83% of cancerous cases but in 50% of polypous cases as compared with the respective control tissues. As for the phospholipid fatty acid composition, the percentage value of C20:4 was increased and the percentage value of C18:2 was decreased in gastric cancerous tissues when compared with those of gastric polypous tissues. Such a change of the phospholipid fatty acid composition may produce the change of the fluidity of the cell membrane and may provoke a malignant growth of human gastric cancer cells.

This study was undertaken to determine the effect of ulcer induced by iodoacetamide on the development of gastric carcinoma by N-methyl-N'-nitro-N-nitrosoguanidine in male Wistar rats. Fifty-six of the 62 ulcers induced by IAM were located in the fundic gland area along the limiting ridge. The incidence of fundic carcinoma was 16% in the groups treated with IAM and MNNG, while no fundic carcinoma was found in the group treated with MNNG alone. This difference was statistically significant. All the carcinomas in the fundic gland area were confined within the ulcer itself or its scar tissue, produced by IAM. These findings indicate that if an ulcer is present, carcinoma develops even in the fundic mucosa which is, if intact, resistant to the carcinogenic stimulation of MNNG. It was concluded that
gastric ulcer
predisposes the development of gastric carcinoma.

The primary aim of the Japan--Hawaii Cancer Study was to identify factors that could explain the changes in cancer risk experienced by Japanese who migrated to Hawaii. Many investigations were conducted in this long-term prospective study since its inception in 1971. Among the findings that relate to gastrointestinal carcinoma were the following: 1) Bowel transit time does not appear to be related to the occurrence of large bowel cancer or to any of the benign conditions with which it is associated; 2) adenomatous and hyperplastic polyps, as well as diverticula, are much more prevalent among autopsy specimens from Japanese who had lived in Hawaii than of those in Japan; 3) adenomatous polyps and diverticula are positively associated with atherosclerosis in the necropsy population in Hawaii; 4) although the incidence of the diffuse histopathologic type of gastric cancer does not differ appreciably among the Japanese in Hawaii and Japan, the migrants have a significantly lower incidence of the intestinal type of stomach cancer; and 5) case-control studies indicated that the two conditions frequently associated with gastric carcinoma, i.e.,
gastric ulcer
and intestinal metaplasia of the stomach, are associated with high salt intakes and adherence to the traditional Japanese diet.

PMID:Gastrointestinal carcinoma in the Japanese of Hawaii: a status report. 61 38
The present study of the prognosis in peptic ulcer comprises (1) an analysis of deaths caused by the disease itself, and (2) an independent actuarial analysis of life expectancy. The computations are based on 235 deaths that occurred among 1905 patients with peptic ulcer who constituted a random sample of the occurrence of ulcer disease in an area of Denmark comprising half a million inhabitants. The two methods of analysis both showed that peptic ulcer does influence the survival, but the excess of deaths was small and, in the case of solitary peptic ulcer, was present only in the first year or two after the diagnosis. The prognosis tended to be more serious in
gastric ulcer
than in duodenal ulcer, but the difference was not significant. In duodenal ulcer the survival tended to be better when only a deformed bulb was present. The life expectancy was not significantly different for men and women. Patients with combined gastric and duodenal ulcers had a fatality twice as great as those with solitary ulcers. No excess of deaths was found in patients under the age of 50 years at diagnosis. The duration of prediagnostic symptoms was not predictive for the prognosis.

Idiopathic chronic gastritis is an autoimmune disease and the cause of this gastritis probably act through a final common immunologic pathway. Specific gastric antibodies are diagnostic of this gastritis and the tissue of the gastric mucosa is characterized by infiltrations of mononuclear cells damaging the glandular parenchyma with concomitant loss of gastric secretory function. With suitable predisposing factors operating, the patient with simple chronic gastritis can have pernicious anemia develop. Chronic gastritis is frequently associated with chronic
gastric ulcer
and carcinoma of the stomach but not with duodenal ulcer. Chronic gastritis clinically presents with vague dyspeptic symptoms. Females are more commonly affected than males and the incidence rises with advancing age. Treatment of chronic gastritis is supportive in nature and also similar to the treatment of other autoimmune diseases. The author has suggested a new treatment of duodenal ulcer whereby the patients are immunized against mucosal antigen with a view to induction of chronic gastritis. The resulting biochemical damage of the parietal cells will produce hypochlorhydria.

Uninvolved gastric mucosa from duodenal ulcer,
gastric ulcer
, and gastric cancer patients was incubated with [1-14C]glucose and [6-14C]glucose in order to assess the relative contributions of the pentose phosphate pathway and Krebs cycle to glucose metabolism. [14C]Glucose counts retained by the tissue, glycolysis, and pyruvate formation were also measured. Tumor tissue from the cancer patients was included in the study. Less than 1.2% of the glucose entering the tissues was metabolized via the pentose phosphate pathway; suggesting that this pathway plays a minor role in energy production from glucose. The major determinant of energy production was the Krebs cycle. Its contribution to glucose metabolism was greatest in the body mucosa of duodenal ulcer patients, less in the uninvolved body mucosa of
gastric ulcer
patients, and lower still in the corresponding body mucosa of gastric cancer patients. The low levels of Krebs cycle activity seen in the latter tissue resembled those of uninvolved antral mucosa. The smallest Krebs cycle contribution was seen in tumor tissue. [14C]Glucose counts retained by the tissue and glycolysis both tended to vary inversely with Krebs cycle activity among the tissues studied. Thus, both were small in the body mucosa of noncancer patients and somewhat larger in the body mucosa of cancer patients, in uninvolved antral mucosa and in tumor tissue.

In 15 patients with uncomplicated gastric ulcers, basal and peak gastric acid outputs and fasting serum gastrin levels were studied before and after healing. The mean basal acid output [4.0 +/- 1.3 (SEM) mEq H+/hr], the mean peak acid output (29.5 +/- 5.1 mEq H+/hr), and the mean fasting serum gastrin level (80.3 +/- 16.7 pg/ml) in these patients did not change significantly with healing. An instrument has been developed which creates an experimental model of an acute bleeding
gastric ulcer
. The diameter and depth of these gastric ulcers are reproducible. The instrument can be used endoscopically or at laparotomy. Using this ulcer model nonsurgical modalities for the treatment of upper gastrointestinal bleeding can be compared in a controlled manner. This standard experimental model may also facilitate comparison of results among different research groups.
