Gastric ulcers in children are often seen as a result of an imbalance between nocive factors and the resistance of the mucosa. Glanzmann's thromboastenia is a defect in platelet aggregation and its nature may be associated to local factors which precipitate or perpetuate bleeding. Prostaglandins are hormones of local action which inhibit the secretion of acid and therefore protect the mucosa. We report a case of an 18 year old male youngster with Glanzmann's thromboastenia and a gastric ulcer previously unsuccessfully treated with cimetidine, aluminum hydroxide solution, ranitidine, sucralfate and later controlled with PGE2. In sum, the usefulness of the cytoprotecting effects of the prostaglandins are successfully evaluated in a young man with platelet aggregation abnormalities.

Clonidine 2 mg/kg ig inhibited the rat gastric ulcers induced by pyloric ligation, stress and indomethacin by 71%, 77% and 82%, respectively. Clonidine 2 mg/kg ig tended to accelerate the healing of gastric ulcer induced by acetic acid, and the healing rate was 61%. Clonidine decreased the secretion of gastric acid and pepsin, and increased the release of gastric barrier mucus. These actions may contribute to its protective effect against ulceration.

Zinc sulfadiazine (ZnSD) 50, 100, 200 mg/kg ig inhibited the formation of gastric ulcer induced by indomethacin, stress and pyloric ligation in rats respectively and showed dose-dependently. ZnSD 200 mg/kg ig accelerated the healing of gastric ulcer induced by acetic acid. ZnSD 25 mg/kg ig was effective in preventing ethanol-induced damage of rat gastric mucosa. The amount of gastric mucus glycoprotein in gastric tissues was increased by ZnSD. In general, ZnSD did not influence the volume of gastric juice and pepsin output, but ZnSD 200 mg/kg ig decreased gastric acidity. In vitro, ZnSD also influenced the neutralization of acid. It is suggested that antiulcer action of ZnSD may be related to its preservation of the gastric mucosal barrier and neutralization of acid.

We evaluated the role of oxygen free radicals in the induction of acute stress gastric ulcer in rats. After 12 hr of immobility, ulcers of up to 4 mm were observed in the gastric mucosa. Pretreatment with allopurinol, a xanthine oxidase inhibitor, produced a significant reduction in the number and size of lesions (p < 0.0001). No protection was afforded by aluminum hydroxide or ranitidine alone, but enhanced protection was observed when given in association to allopurinol. A secondary role for H ions is suggested by these findings. Our results support the hypothesis of a role of oxygen free radicals in the pathogenesis of stress gastric ulcers. Allopurinol might be used in conditions predisposing to stress in patients.

The effect on the recurrence of gastric ulcers after suppression of Helicobacter pylori by combined treatment with cimetidine and the antimicrobial drug cefixime was investigated. Twenty one of 43 patients with endoscopically proved gastric ulcer and H pylori infection were randomly assigned to receive cimetidine 800 mg daily for 12 weeks; the remaining 22 patients received cimetidine 800 mg daily for 12 weeks plus cefixime 100 mg daily for the last two weeks. After treatment, 88% of 17 patients on cimetidine only remained H pylori positive, whereas combined administration of cimetidine and cefixime had suppressed H pylori in 78% of 18 patients (p less than 0.05). Seventeen patients in the former group whose ulcers healed but who remained H pylori positive and 18 patients in the latter group whose ulcers healed and who were no longer infected with H pylori continued to be followed after treatment. These patients underwent endoscopy to detect ulcer recurrence if symptomatic, or at 12 and 24 weeks if asymptomatic. At 12 weeks, recurrence was observed in seven of 15 (47%) patients in whom H pylori persisted, but in only one of 14 (7%) patients in whom H pylori had been suppressed (p less than 0.05). At 24 weeks, however, recurrence rates were similar between the two groups. These findings indicate that H pylori infection may be closely related to early ulcer recurrence.

X-ray examinations of the stomach and duodenum from a two year period of 135 Greenlanders and an equivalent number of Danes were reviewed in order to diagnose all radiological lesions. The results are presented. In 37% of the Greenlanders and 51% of the Danes, x-rays showed normal findings. Peptic ulcerations were diagnosed for the first time in 2.4% of Greenlanders over the age of 15 years. The incidence of gastric ulcer was 1.5% and of duodenal ulcer 0.9%. The male/female ratio was 1.3 in Greenlanders. The esofagus and cardia region of the Greenlanders revealed many serious lesions, emphasizing the importance of further investigations in this area. The incidence of peptic ulcer disease and gastric ulcer is higher than the incidence in Denmark but approximately the same as in the Faroe Islands. The incidence of duodenal ulcer is found to be lower among Greenlanders than in the other two populations.

We previously reported that the reported that the ratio of gastric ulcer to duodenal ulcer in Hokkadio was higher than that in other area of Japan. This time, we analysed peptic ulcer cases of five hospitals in Hokkaido on the point of geographical and pathological distribution. When we divided these hospitals into two groups (in Sapporo city and other provincial areas) and calculated the sex and age adjusted ratio of gastric ulcer to duodenal ulcer in each group, we found the difference between the two groups (in Sapporo city: 2.26, other provincial area: 5.86) and thought the feature of high ratio the feature of the provincial hospitals. This difference in Hokkaido may be due to the environmental factors for peptic ulcer or the biased sampling of the study. The further study is needed to reveal the geographical and pathological distribution of this disease.

The purpose of the paper is to assess the role of present-day fluorography combined with endoscopic investigations in the detection of peptic ulcer. A study of the results of gastrofluorography with videocontrol has shown high potentialities of the method during primary detection of ulcer among persons at high risk for the disease and subsequent control of therapeutic efficacy. The high coverage, low cost and low radiation load proposed by the program make it possible to considerably improve the whole system of prophylactic medical examinations of the patients. The modifications made by the authors in the examination make it possible to raise the accuracy of diagnosis of stomach ulcer recurrences by 28% and to reduce the annual rates of recurrence 1.5-fold.

Campylobacter pylori has been associated with gastro-duodenal inflammatory disease. Ninety-five adults with dyspepsia were examined for the presence of C. pylori in the gastric antrum and near gastric or duodenal ulcers (when present) by means of culture, Gram and acridine orange stains, and urease activity of biopsies. C. pylori was identified from 51 out of 67 patients with chronic gastritis, from 9 out of 9 patients with duodenal ulcer, and from 8 out of 10 patients with gastric ulcer. Acridine orange stain revealed the highest number of positive cases, followed by culture, Gram stain and urease test. The latter showed a 100% specificity when carried out with a selective urea broth containing colistin, trimethoprim, vancomycin and amphotericin B. It has to be considered a further diagnostic tool which enables clinicians and microbiologists to diagnose the etiology of a dyspeptic syndrome even at the patient's bedside.

This study was based upon deceased patients in a geriatric university hospital with a high autopsy rate (81%). Of 6200 autopsied patients, 333 (5.4%) had had an active peptic ulcer; agonal and other acute erosions were not included. 257 cases were selected for the study (average age 83.8 yr). The diagnostic accuracy, and the symptoms of peptic ulcer in stationary, elderly, chronically ill patients were studied retrospectively. Only 16% of cases with duodenal ulcer and 29% with gastric ulcer had been correctly diagnosed antemortem. The clinical features of ulcer disease in the elderly may often differ from the standard presentation in younger people. Prior to death, appetite and weight loss, nausea/vomiting, anaemia and positive occult blood test had been more common among patients with ulcer, than abdominal pain and heartburn. The predictive values of single symptoms and of combined findings were low (range 2-21%), thus supporting observations from clinical practice that diagnosis is difficult in geriatric medicine. Prospective studies of ulcer disease in living elderly are needed.
