Among 7015 upper gastrointestinal endoscopies done during five consecutive years (1984-1988) 642 patients were diagnosed as having benign
gastric ulcer
. Of this group, 213 patients have been followed-up during an adequate period of time. A recurrence was diagnosed endoscopically in 43 (20.18%). The authors analyze the influence on recurrences of general factors (age, sex, alcohol, tobacco and drugs), family history and local factors (site and size of the ulcer, histological lesions, concomitant duodenal ulcer or single or multiple ulcers). The conclusions are that in females and in males over 60 years of age, NSAIDS are the factors which greatly influence recurrences, while in males under 60 years of age, excessive smoking and alcohol. Chronic gastritis and intestinal metaplasia are common in recurrent ulcers. Multiple ulcers as well as those associated with duodenal ulcers have a greater tendency to recur. Recurring ulcers are more common at the incisura angularis. Recurrences are usually located at the site of a previous ulcer.

The distribution of (+)-(1R,4aS,10aR)-1,2,3,4,4a,9,10,10a-octahydro-1,4a- dimethyl-7-(1-methylethyl)-6-sulfo-1-phenanthrenecarboxylic acid 6-sodium salt pentahydrate (TA-2711) in stomach was studied after oral administration of 14C-TA-2711 (100 mg/kg) to rats. At 6 h after dosing, most of the radioactivity was found in the lower intestine, caecum and large intestine. However, the radioactivity in the stomach was higher than that in the upper and middle small intestine. The concentration of the radioactivity in the glandular stomach was about 1.5 mg eq. TA-2711/g at 30 min after administration. Thereafter, it decreased gradually to about 60 micrograms eq. TA-2711/g at 6 h after dosing. In the radioluminograms of 6 and 24 h after dosing, most of the radioactivity was observed on the surface of gastric mucosa. After administration to rats with
gastric ulcer
induced by acetic acid, higher radioactivity was observed in the ulcerated tissues of the stomach than in the nonulcerated control tissues. In the stomach damaged by ethanol, the concentrations of radioactivity in lesion parts were also higher than those in non-lesion parts. In the microautoradiograms of gastric mucosa damaged by ethanol, the developed silver grains were densely distributed in necrotic tissues.

A rare case of valvular pneumoperitoneum is reported. A 49-year-old Japanese man underwent an emergency laparotomy under a tentative diagnosis of perforating panperitonitis on the 19th post-open heart surgery day. A large volume of free intraperitoneal gas was revealed, but no identification of gastrointestinal perforation. On the 17th postlaparotomy day, pneumoperitoneum recurred. On the 41st postlaparotomy day, a tiny gastric perforation was detected with an oral contrast medium. The patient died of liver failure due to serum hepatitis. The autopsy revealed a minute
gastric ulcer
scar 2 mm in diameter in the mucosa of the fornix, which was thought to have caused the valvular pneumoperitoneum. A literature review was made of 8 reports on 11 cases. In two cases of valvular pneumoperitoneum, the perforating site could not be found even during laparotomy. It is suggested that either gastrointestinal series or endoscopy must be carefully repeated in order to identify the perforating site.

PMID:Recurrent valvular pneumoperitoneum caused by a minute gastric ulcer--a case report. 181 96
A review of children admitted to Christchurch Hospital with acute laryngotracheitis over a 10 year and nine month period revealed that 894 admissions occurred for this condition. Forty-four (4.9%) were admitted to the intensive care unit of the hospital, of whom 12 (1.3% of all laryngotracheitis admissions) required endotracheal intubation. The mean time of intubation was 170 hours (range 24-432). There were no deaths, although seven children who were intubated had serious complications, of which perforation of a
gastric ulcer
was the most important. In our group of patients intubation is a rare event, and the need for aggressive treatment with dexamethasone at the time of admission to the general ward is questioned.

Previous epidemiological studies have described the secular trends in peptic ulcer mortality in England and Wales as being characteristic of a cohort phenomenon. The most recent data on ulcer mortality, however, show increasing mortality rates from duodenal ulcer in women over 65 and from
gastric ulcer
in women over 75 years. While the rise in mortality rates in the oldest age groups is partly explained by their greater life expectancy, the increase in mortality from duodenal ulcer in older women shows evidence of an environmental (period) effect being superimposed on female cohorts born between 1895 and 1925. This effect could partly be due to the increasing consumption of non-steroidal anti-inflammatory drugs which coincides with the rise in mortality rates.

This retrospective study was undertaken to determine if the advent of H2-blockers has altered the surgical treatment and the outcome of patients with peptic ulcer disease. The records of patients undergoing surgery for peptic ulcer disease at Montecchio Maggiore Hospital, Vicenza (Italy), from 1968 to 1977 (group A) and from 1979 to 1989 (group B) were reviewed. Data recorded included, ulcer location, surgical procedures (emergency or elective), indications for emergency surgery (perforation, bleeding). Statistical analysis was performed by means of the chi-square test. From 1968 to 1989, 15.810 surgical operations were performed: the indication was peptic ulcer disease in 398 cases (2.51%). The percentage of surgical operations performed for peptic ulcer was 3.54% in group A and 1.55% in group B. A marked decline in surgery for peptic ulcer was noted in group B, concerning both elective and emergency surgery; surgical interventions strongly decreased for both duodenal and
gastric ulcer
. This study demonstrates that H2-blockers have definitely altered the surgical treatment for peptic ulcer disease.

The effects of zinc sulphate on
gastric ulcer
healing rate and mucosal mucus content of acetic acid-induced ulceration in rats have been assessed. Daily treatment with zinc sulphate progressively accelerated ulcer healing in a dose-dependent manner with a significant increase observed on day 15 after ulcer induction in rats treated with 44 and 88 mg kg-1 zinc sulphate. A significant increase in gastric mucosal adherent mucus was also observed in those animals treated with 88 mg kg-1 zinc sulphate. The results suggest that a minimum treatment period of 15 days is needed for the zinc sulphate to be effective, and that zinc ions may promote
gastric ulcer
healing by enhancing mucus formation to prevent acid back-diffusion into the gastric mucosa.

Helicobacter (Campylobacter) pylori has been cultured from the antral biopsies of 85-90% of patients of gastritis,
gastric ulcer
and duodenal ulcer at different centres. Studies conducted all over the world have firmly implicated this organism in the aetiology of active superficial gastritis and recurrences of duodenal ulcer. Two hundred patients with upper abdominal pain, distension, vomiting and/or haemetemesis were subjected to OGD scopy. In 163 of these patients there was endoscopic evidence of gastritis; in 24 there was DU; in 3, GU and in 10 it was normal. Diagnosis of H pylori infection was made by the rapid biopsy urease test which is nearly 100% specific and 98% sensitive. 170 out of 200 patients were positive for H pylori. Among these were 138 patients of gastritis (84.6%); 22 cases of DU (91.6%); 2 cases of GU (66.6%) and 8 in whom endoscopy was normal. Histological examination of the antral biopsy specimens showed mild to severe infiltration of mucosa with lymphocytes and plasma cells. None of the 170 H pylori positive cases showed polymorphonuclear infiltration which has been stressed repeatedly by most Western authors to be characteristic of "active" superficial gastritis associated with H pylori infection. Even in those with a history of dyspepsia of barely 4 weeks duration or less there was no PMN infiltration in the mucosa. Thus the local response to infection by H pylori of the gastric mucosa is different in Indian patients.

Seven hundred and twenty cases of endoscopically proven peptic ulcer were seen at PMRC Research Centre, Jinnah Postgraduate Medical Centre, during the last 13 years (June 1976-June 1989). Duodenal to
gastric ulcer
ratio was 5:1. Duodenal ulcer was predominantly seen between 3rd and 5th decade of life, with a male to female ratio of 5. 7:1. Seventy five percent cases were skilled persons. Places of origin showed 50% to be migrants; mostly from India while the rest belonged to various provinces of Pakistan. Sixty percent patients were smokers/tobacco chewers and 3% were simultaneously taking alcohol. History of NSAID intake was present in 5% cases only. Majority (85%) of the patients presented with pain, 42% had upper G.I. bleeding, 11% perforation and 1% pyloric stenosis.
