In 1971 to 1977, 34 patients were operated for gastric ulcer at Zaria in the Savannah region of Northern Nigeria. The most frequent operation (35%) was for perforation. The frequency of perforation is similar to Ibadan's but smaller than Dakar's. Compared with Ibadan and Burundi, there are fewer women, more Type I ulcers and more frequent haemorrhages. There were three giant lesser curvature ulcers. Only a few combined duodenal and gastric ulcers were seen suggesting that gastric stasis is not the immediate factor in the production of gastric ulcer in this region.

Stress and allopurinol (ALL) were investigated in rats with regard to their influence on serum uric acid and glucose concentration, gastric secretion, microcirculation (MBF) and stress ulcer index. There is a non-competitive type of interaction between severe stress and ALL-mediated xanthine oxydase inhibition (= per cent fall in serum uric acid) as compared with control conditions (= mild stress). Vmax is different (84.6 +/- 1.9 and 92.3+/-2.26; P less than 0.05), but not Km (0.39 +/- 0.09 and 0.68 +/- 0.08 mg/kg/h ALL). Serum uric acid is higher in rats with draining gastric fistula than those with closed fistula suggesting that already mild stress is associated with an increase in uricemia in this species. ALL does not significantly alter gastric acid and uric acid secretion but improves markedly gastric ulcer index during mild and severe stress. Since MBF is significantly elevated by ALL during the latter circumstances, a dissociation between MBF and acid secretion is one feature of ALL actions and might become a primary aim in treatment of this disorder.

Carcinoma of the gastric stump in patients who have undergone resection for duodenal and gastric ulcer represents a rare event which poses considerable diagnostic and therapeutic problems. 7 personal cases are examined, particular attention being paid to the utility of endoscopy, together with sampling for cytological and histological examinations, by comparison with radiology. Endoscopy in the present cases have confirmed the high positivity obtainable with transendoscopic cytological sampling using the brushing technique. The technique should therefore become routine in the study of patients undergoing gastric resection.

Biopsy specimens of the duodenal mucosa were assayed to determine their secretin-like activity in 9 controls, 9 patients with gastric ulcer, 19 patients with duodenal ulcer, 4 patients with gastric and duodenal ulcer, and 13 patients with chronic pancreatitis. The bioassay of secretin was done on the pancreatic secretion in anesthetized rats. The sensitivity was in the orcer of 0.0625 CHR unit/rat (4 ng/rat). In the range between 0.0625 and 0.5 CHR units a satisfactory dose dependency was recognized. The following results were obtained. 1) The level of duodenal mucosal secretin-like activity in patients with gastric ulcer was the same as that in the controls, but was elevated in 32% of the patients with duodenal ulcer, 50% of those with gastric and duodenal ulcer, and 8% of those with chronic pancreatitis. 2) The high level of secretin-like activity noted in patients with duodenal ulcer was suspected to be related to the hypersecretion of gastric acid which is characteristic of this disease, but there was no correlation between gastric acid secretion and secretin-like activity in the duodenal mucosa.

One hundred forty-four gastric ulcer patients in an inner city hospital were studied retrospectively over a five year period. Emphasis was placed on patient follow-up after discharge. The length of hospitalization and the mortality were higher for the surgical group; however, the recurrence rate was higher in the medically treated group. Almost half the patients were followed for less than six months. Reasons are given for this poor follow-up. The high recurrence rates necessitate close follow-up for a prolonged period.

Irritative effects of three steroidal anti-inflammatory drugs on the gastrointestinal tract of rats and dogs were determined. With either single or repeated subcutaneous administration these drugs dose dependently irritated the gastric mucosa of both species. The intestinal mucosa was less affected. Concomitant oral administration of aspirin or subcutaneous administration of indomethacin revealed an aggravation of aspirin-induced gastric ulcers by betamethasone valerate and inhibition of indomethacin-induced intestinal ulcers by beta-methasone dipropionate. These two steroidal drugs had no noxious effect on healing of chronic gastric ulcers induced in rats and dogs. Betamethasone valerate, however, delayed the healing of gastric ulcer in rats. Indomethacin, a non-steroidal anti-inflammatory drug, also induced serious damage to the gastric and intestinal mucosa both of rats and dogs. Indomethacin ingestion delayed the healing of chronic gastric ulcer in rats but not in dogs. Since both steroidal and non-steroidal drugs induce damage to the gastrointestinal tract, a careful monitoring of the patients' complaints should be carried out when these compounds are used as a systemic treatment. Steroidal drugs used in this study, however, appear to be highly safe from the point of dose inasmuch as they are used as a topical treatment.

Clinically, it is true that gastric ulcer is more common along the lesser curvature and less common along the greater curvature. To elucidate this difference in ulcerognesis, two experimental ulcers were prepared in dogs at the sites, one at the angular incisure and the other at the corresponding greater curvature after the method by Hatafuku & Thal. The fates of these ulcers studied endoscopically, macroscopically, histologically and with the use of microangiography. The results obtained are summarized as follows. 1) When two ulcers only were prepared, better healing was consistent for the greater curve ulcer. 2) Experimental ulcers with simultaneous ligation of gastric vessels resulted in poor healing of both ulcers, though the greater curve ulcer showed better healing than the lesser curve ulcer. 3) Endoscopically, dogs with vessel ligation showed antral congestion and edema. These findings, however, disappeared in 3 to 4 weeks. 4) Histamine injection accelerated the rate of early perforation and both experimental ulcers showed no tendency of healing. In conclusion, delayed healing was almost consistent for the lesser curve ulcer under various conditions. The cause for this may be explained by the increased motility with repeated ischemic condition at the lesser curvature.

Forty-four patients with chronic gastric ulcer were compared with a control group matched for age, sex, and place of residence. The personality pattern was studied by the Eysenck Personality Inventory and personality assessed in terms of neuroticism and extroversion. The relevance of social class was also studied. The gastric ulcer population did not differ from the control group except among the male population, in which the gastric ulcer population was significantly more neurotic and less extroverted than their controls.

125 cases of hernia, cholelithiasis, peptic and gastric ulcer, carcinoma of the stomach of stages III and IV were examined before operation and in the postoperative period. The peculiarity of the studied indices of energy metabolism was established in each group of patients. The authors believe, that the difinition of ATP level and of the erythrocytes energy charge can be used as one of criteria of patients' condition and of medicamental correction of metabolism.
