Comparative clinical trials were conducted by the double-blind technique in a series of 302 cases of gastric ulcer at fifteen medical institutions in order to investigate the clinical course of peptic ulceration and the effect of pharmacotherapy on it, using N-acetyl-L-glutamine aluminum complex (KW) as test drug and basic aluminum sucrose sulfate (UL) as reference drug as well as an inactive placebo of lactose (PL) with the results as follows: In 12 weeks of treatment with inactive placebo, a higher cure rates were noted in inpatients than in outpatients, and in patients with ulceration in the corpus ventriculi than in those with ulceration at the angulus. The overall evaluation of therapeutic responses indicated that, both KW and UL were significantly superior in efficacy to placebo in 12 weeks of therapy. These two medicaments displayed different modes of efficacy, possibly reflecting their different mechanisms of pharmacologic action. The efficacy of the medicaments was particularly superior in outpatients and patients with peptic ulceration at the gastric angle both of which showed lower rates of response to placebo.

The tests with lactose loading followed by the assay of blood sugar were conducted in 500 normal subjects, aged from 18 to 89 years, and 262 patients with gastro-intestinal diseases, aged from 25 to 55 years. When lactose malabsorption was detected, aspiration biopsy of the small intestine mucosa was performed followed by the study of the structure and the level of a number of disaccharidases (lactase, maltase, saccharase). Lactose malabsorption was detected in 72 (14.4%) out of 500 normal subjects (10.6%--aged 18-59, and 20%--aged 60-89 years), among them there were 12.5% of Russians, 13% of Byelorussians and 5.8% of Ukrainians (aged 25-55 years). The secondary lactose malabsorption was recorded in 44% of patients with ulcerative colitis, in 33% of patients with chronic enterocolitis, in 11.5% of patients with gastric ulcer, in 8% of those with duodenal ulcer, in 23.5% of patients with chronic gastritis attended by lowered secretory function, and in 8% of those with enhanced secretory function.

Currently available anti-ulcer drugs suffer from serious side effects which limited their uses and prompted the need to search for a safe and efficient new anti-ulcer agent. Boswellia gum resin (BR) emerged as a safe, efficient, natural, and economic potential cytoprotective agent. Thus, it is of medical importance to develop gastroretentive (GR) formulations of BR to enhance its bioavailability and anti-ulcer efficacy. Early attempts involved the use of organic solvents and non-applicability to large-scale production. In this study, different tablet formulations were prepared by simple direct compression combining floating and bioadhesion mechanisms employing hydroxypropyl methylcellulose (HPMC), sodium carboxymethyl cellulose (SCMC), pectin (PC), and/or carbopol (CP) as bioadhesive polymers and sodium bicarbonate (SB) as a gas former. The prepared tablets were subjected for assessment of swelling, floating, bioadhesion, and drug release in 0.1 N HCl. The optimized GR formulation was examined for its protective effect on the gastric ulcer induced by indomethacin in albino rabbits compared with lactose tablets. The obtained results disclosed that swelling, floating, bioadhesion, and drug release of the GR tablets of BR depend mainly on the nature of the matrix and the ratio of polymer combinations. Moreover, a combination of SCMC-CP in a ratio of 2:1 (SCP21) exhibited desirable floating, bioadhesion, swelling, and extended drug release. Also, a 6-h pretreatment with SCP21 tablets decreased the severity of inflammation and number of bleeding spots among ulcer-induced rabbits in comparison to those treated with lactose tablets.
