Samples of gastric juice were aspirated every 15 minutes from 54 normal subjects and 31 patients with chronic gastric ulcers during a maximal histamine infusion test. The known tendency of patients with gastric ulcers to secrete a less acid gastric juice than that of normal subjects was confirmed. However, the hypo-
acidity
was related to the extent by which the total ionic concentration was less than the isotonic value of 328 mEq/l. On the assumption that such hypo-
acidity
was produced by the neutralisation of hydrogen ions by bicarbonate ions refluxing into the stomach from the duodenum, the data were corrected and resulted in a normal estimate of the hydrogen ion concentration in the
gastric ulcer
group. Independent corrections, according to the sodium content of refluxed duodenal juice, yielded similar results for the volume of gastric juice aspirated. It is concluded that while back-diffusion can explain the hypo-
acidity
of gastric juice in patients with gastric ulcers, duodenal reflux can explain both the hypo-
acidity
and the hypotonicity, and is therefore more likely to be the correct explanation.

Antacids can reduce gastroduodenal
acidity
for long periods if taken in substantial quantities after food. Their healing effect on
gastric ulcer
is minimal, if present at all, and easily overwhelmed by the benefit obtained from admission to hospital. Intensive antacid therapy appears effective in healing duodenal ulcer and preventing haemorrhage from stress ulcer, and is comparable in these respects with cimetidine but with a higher incidence of side-effects. Clinical impression strongly suggests that antacids relieve pain in peptic ulcer but objective confirmation is lacking.

1. The operative technique and the postoperative results of the original pylorus-preserving gastrectomy (PPG) and those of the Maki procedure as applied to cases of total gastrectomy are reported briefly. 2. The essence of these operations is to retain the pyloric cuff of 1.0-1.5 cm in length in order to preserve sphincteric function without the use of the drainage procedure. Ingested food was seen to empty gradually and rhythmically through the pyloric ring into the duodenum thereby avoiding dumping syndrome and reflux esophagitis. 3. As gastric

is reduced sufficiently by PPG, this method is to be recommended for treatment of intractable

cases and other benign lesions in which the foci are located in the distal half of the stomach. Though the method seems to be also useful in selected cases of duodenal ulcer, stenosis at the level of the pylorus and duodenum may preclude the application of the method. 4. The Maki procedure with jejunal interposition following nearly-total proximal gastrectomy is recommended in patients whose pylorus and its surroundings have remained free of tumor.

The results are reported from the treatment of 30 patients with gastric and 13 with diodenal ulcers with biogastrone in the course of 20 days (300 mg were given the first week, where on -- 150 mg daily). A more favourable effect upon the clinical symptoms and ulcer were observed in the patients with
gastric ulcer
(the symptoms disappeared in 87 per cent whereas the ulcer -- in 32 per cent) as compared with the duodenal ulcer (the symptoms disappeared in 46 per cent, whereas -- the ulcer in none). Biogastrone treatment influences the pain in a higher per cent as compared with Caved-S and oxyferroscorbon (p greater then 0.05), whereas regards the rest of the clinical symptoms -- no statistically significant difference was established. As compared with the treatment with oxyferroscorbon, Caved-A and cimetidin, biogastrone has a slower effect on the subjective complaints and the ulcer niche. After biogastrone treatment, the indices of gastric secretion and
acidity
decrease and the sialic acid in gastric juice is increased only in the
gastric ulcer
, but does not change considerably in the duodenal ulcer. The rest of the laboratory indices do not change with biogastrone treatment. In two of the patients, in the course of biogastrone treatment, light edema appeared on the face and the arterial blood pressure was elevated in other two, disappearing after the treatment without any additional treatment. A long-term follow-up gastrocamera photographic study of 481 patients with
gastric ulcer
was conducted at intervals of 3 weeks to 6 months. It showed that gastric ulcers could remain healed for 2 months to 6 years of observation, yet, recurrence of ulceration occurred in 79% including 18% with multiple occurences. Gastric polyp developed in 8 patients whose initial gastrophotography showed no such lesion in the original study. Furthermore, 7 cancers of the stomach developed at sites away from the initial benign ulcer foci. Six of these cases proved to be mucosal cancer and one was an advanced cancer. In 5 of 7 cases of malignancy, the cancer occurred distal to the initial ulcer foci and anaplastic adenocarcinoma was seen in 3 of these. The others were well differentiated adenocarcinomas. No correlation was found among histopathological type of cancer, depth of malignant invasion, location of the new growth or gastric juice
acidity
in these 7 cases of malignancy. It is concluded that a thorough and routine gastroscopic examination is indicated for all sorts of gastric disorders whether they are initial or follow-up cases.

and the acid output were analysed separately. The results were compared with earlier studies of patients with

and duodenal ulcer. Starting from the secretory pattern these four groups of ulcer were characterized. Gastric ulcers thus show hypoacidity. The pyloric ulcers do not differ from a normal material. The combined gastric and duodenal ulcers and the duodenal ulcers show hypersecretion, most pronounced in duodenal ulcers.

and carcinoma in the fundic gland area were investigated with the surgically resected cases. 1. The frequency of the ulcer in the fundic gland area was 8.2% of 2194 cases of
gastric ulcer
and that of carcinoma in this area was 8.4% of 780 cases of gastric carcinoma. These two diseases showed high frequency in the age group of forty to fifty years withpreponderance in female. 2. The ulcers in the fundic gland area usually showed high
acidity
and high frequency of massive bleeding like as melena and hematemesis comparing with other lesions or the ulcers in other sites. 3. The ratio of the low-differentiated carcinomas to the well-differentiated ones was about 3.7 to 1 in the fundic gland area and 1 to 1.3 in the pyrolic gland area. 4. The frequency of the early gastric carcinomas was 27% and 41% in the fundic and pyrolic gland areas, respectively. 5. In the fundic gland area, as observed in the pyrolic gland area, the low-differentiated carcinomas usually occurred on the non-metaplastic mucosa and the majority of well-differentiated ones were frequently found in association with the intestinal metaplasia. 6. Although the solitary carcinoma in the anterior wall or greater curvature was found not infrequently, the solitary ulcer was not found in these sites in this series.

The role of gastric acid in the development of gastroduodenal ulcers in prostaglandin-deficient conditions is unclear. In the current study, the effect of the proton pump inhibitor omeprazole on the formation of gastric ulcers was examined in a previously validated rabbit model of antibody-induced prostaglandin deficiency. Intragastric administration of 20 mg/kg omeprazole every 12 hours caused a profound suppression of gastric
acidity
(i.e., pH above 5 continuously). This same dose of omeprazole significantly reduced
gastric ulcer
formation induced by passive immunization with 6-keto-prostaglandin F1 alpha antibodies. It is concluded from these observations that gastric acid plays a critical role in the formation of gastric ulcers in rabbits with antibody-induced prostaglandin deficiency.

The article analyses the results of pylorus-preserving resections of the stomach in 116 patients among whom 80 had
gastric ulcer
, 29 ad duodenal ulcer, and 7 had gastroduodenal ulcers. Changes of
acidity
and motor-evacuation function of the gastric stump were studied according to the method of pylorus-preserving resection. The late-term results of treatment in follow-up periods of 2 to 5 years are shown, they were found to be good in 96.6% of patients treated by operation and poor in 1.7%. It is noted that the function of the gastric stump is better when the vagus innervation of the pylorus is preserved. Mediogastric resection with selective proximal vagotomy in patients suffering from duodenal ulcer with a superacidic stomach led to stable normalization of acid production, which was a reliable measure for the prevention of a recurrent ulcer.
