Twenty-three patients, mean age of 65 years, planned for emergency surgery because of upper gastrointestinal bleeding, were electrocoagulated with an endoscopic Storz monopolar flushing electrode. Nineteen patients were managed successfully. Two out of twelve patients with
gastric ulcer
and both patients with
gastrojejunal ulcer
bleeding needed an operation. The overall success rate was 82% and the mortality was 9%. According to this study endoscopic monopolar flushing liquid electrocoagulation is a good alternative to surgery in upper gastrointestinal bleedings except gastrojejunal ulceration.

A descriptive investigation of the frequency of discharges and the frequency of complications for patients with ulcer disease on the basis of the National Patient Registry in Denmark covering the years 1978, 1979, and 1980 for the diagnoses

, peptic ulcer, site unspecified, and gastritis-duodenitis is presented. The number of discharges was constant--12,000-13,000 annually, or 250 per 100,000 inhabitants--with a 4% increase for
gastric ulcer
and an 11% decrease for duodenal ulcer during the period. The frequency of discharges was of the same size as for comparable groups of diseases in Denmark in 1970-1977 but greater than in the United Kingdom. The number of patients discharged was 10,000 annually. The consumption of bed days fell from 126,000 to 106,000, implying a reduction in the average length of stay from 9.6 to 8.3 days. The total number of surgical operations and the number of elective operations was constant, around 2000 and 1400 annually, respectively. Of the patients discharged 20% had had a bleeding and 8% a perforation from their ulcer. During the period of examination cimetidine was introduced in the treatment, and this may have influenced the decrease in the consumption of bed days and the frequency of discharges for duodenal ulcer.

Between 1972 and 1980 Hospital Activity Analysis from 5 Northern Ireland Hospitals demonstrated a gradual increase in the number of patients discharged for all diagnoses whilst there was a decline in discharges for peptic ulcer related disease, (duodenal ulcer,
gastric ulcer
,
gastrojejunal ulcer
, peptic ulcer site unspecified and hiatal hernia). The mean number of peptic ulcer related disease discharges per year for 1977-80, when cimetidine became generally available, was 10% lower than that of 1972-76, mainly due to a decline in male patient numbers. The mean annual number of male patients with a duodenal ulcer fell significantly by 18% (U = 1, p less than or equal to 0.025) during the 1977-80 period, whereas female discharges decreased by only 4%. Between 1972-76 and 1977-80 the mean annual number of duodenal ulcer perforations decreased significantly by 21% in males but only by 4% in females. Surgery for peptic ulcer related disease was 47% less in 1977-80 period, with significant decreases in duodenal ulcer,
gastric ulcer
and hiatal hernia procedures. From 1977 to 1980 there was considerable growth in Northern Ireland general practice cimetidine prescribing with 300,000 prescriptions being dispensed over the period. Apart from male duodenal ulcer cases, hospitalisation for peptic ulcer related diseases did not decrease substantially after the introduction of cimetidine but duodenal ulcer perforation and conditions warranting surgery did decline.
