Two cases of retroperitoneal schwannoma, one malignant, and the other benign, are herein reported. The first case was a 71-year-old female with a complaint of dull right flank pain. CT scan showed a large mass, which occupied almost the whole right half of the abdominal cavity extraperitoneally, accompanied by a low density portion in its central part. Angiography showed several feeding arteries, such as intercostal arteries, lumber arteries, and so on. Many tortuous and thin irregular vessels were seen. Tumor excision was performed under the diagnosis of malignant retroperitoneal tumor. No surrounding invasion or metastasis were seen. The tumor had a thick capsule, and cut surface showed lobulated appearance. No adjuvant therapy was done. Histological diagnosis was low grade malignant schwannoma. She is alive and well 17 months postoperatively. The second case was a 64 year-old male. His tumor was found incidentally, when he underwent a close examination of his gastric ulcer. According to CT scan, it was located behind the left kidney, and had a diameter of 3.0 X 2.5 cm. The tumor was removed including the fascia of lateral edge of quadratus lumbolum muscle, where it was firmly attached to. Surgical specimen showed a doughnut like appearance, because of its marked central necrosis. It had a white and thick capsule. Histological diagnosis was benign schwannoma of two different types, Antoni A and B. He remains well 13 months after operation. The 113 benign cases and 55 malignant cases reported in Japan, including our cases, were reviewed and discussed.

Gastric specimens from 259 patients (69 Swedish and 190 Japanese) were examined for the presence of intramucosal cysts. The number of cysts for each centimeter of mucosa analyzed (cystic index) was calculated. The material included 153 patients with peptic gastric ulcer, 41 with elevated dysplasias, and 65 with early gastric cancer. The results demonstrated that in the Japanese material, the normal-looking gastric mucosa adjacent to a variety of gastric lesions such as gastric peptic ulcer, focal elevated dysplasias, or incipient adenocarcinomas, had a significantly higher cyst index than the gastric mucosa adjacent to similar lesions in Swedish patients. At this point, it should be stressed that the Japanese have a high incidence of gastric cancer (82/100,000 males) while the Swedish have a relatively low incidence (28/100,000 males). The results presented herein appear to suggest that cystic formation in the gastric mucosa may herald an early neoplastic growth at the luminal aspect in some of the cysts.

The purpose of this experiment was to determine whether chronic gastric ulcers in the rat are predisposed to tumor formation when exposed to a usually noncarcinogenic dose of the carcinogen, N-Methyl-N'-nitro-N-nitrosoguanidine (MNNG). Two groups of rats were prepared; one subjected to a standard ulcer-producing operation, the other as control. Both groups were given oral MNNG (100 mg/liter as drinking water) for 12 weeks, the carcinogen was then stopped and replaced with tap water, and the experiment terminated at 52 weeks. Results showed that a low dose of carcinogen (200 mg) did not induce tumor formation in any of the normal rats. In the presence of a chronic gastric ulcer, only intestinal metaplasia and hyperplastic glandular nodules were observed, but there were no gastric tumors. It is concluded that the presence of a chronic gastric ulcer did not increase the likelihood of gastric tumor formation in rats treated with a noncarcinogenic dose of the carcinogen MNNG.

Biosynthesis of pyrimidine nucleotides was intensified in proliferating tissues. Age-dependent alterations in activities of thymidine kinase and thymidine phosphorylase were found in healthy persons of 17-70 years old. Distinct increase in activity of thymidine kinase was observed in patients with gastric ulcer but the activity was decreased with ageing. Alterations in the ratio between anabolic and catabolic reactions involving thymidine were studied also in patients with gastric tumor depending on age.

Fifteen cases of gastric pseudolymphoma (GPL) were studied and classified into three subtypes: The nodular type, which is elevated, representing a submucosal tumor. Histologically, the tumor is composed of lymphoid hyperplasia with no associated ulcer. The lesion is well demarcated with no sinusoidal structure. Those findings indicate a nonreactive state. The ulcerative type, which usually is accompanied by deep ulceration with extensive fibrosis. The erosive type, which is infiltrated by extensive lymphoid hyperplasia in the propria mucosa and submucosa. The latter two types represent the reactive state in response to chronic gastric ulcer or inflammation. One of four ulcerative GPLs, however, was associated with focal malignant lymphoma which accounted for one of the fifteen GPL studied. Although there is a low incidence of malignant lymphoma among GPL patients, pseudolymphoma should be considered as a precursor lesion with malignant potential.

A case of a bombesin-producing carcinoid tumor of the lung is reported. Morphologically, the tumor was a typical argentaffinic carcinoid which contained bombesin as demonstrated by diffuse granular cytoplasmic staining using the immunoperoxidase method. Electron microscopy revealed dense-core granules measuring 100-150 nm, consistent with bombesin-containing endocrine cells. In man, bombesin-producing endocrine cells are found predominantly in the bronchopulmonary tree and scattered throughout the intestinal tract. The majority of previously reported bombesin-producing tumors have arisen, as in this case, in the lung; thus the demonstration of bombesin-like immunoreactivity in a widely metastatic tumor should direct the search for a primary lesion to the lung. Bombesin is known to have gastrin-releasing hormone activity. The presence of a gastric ulcer in the present patient raised the possibility of a bombesin-mediated Zollinger-Ellison-like syndrome, an association not previously described.

We evaluated whether assay of tissue polypeptide antigen (TPA) in sera is valuable for the determination of cancer stages compared to other tumor markers such as CEA, AFP, beta2-microglobulin, ferritin, and elastase-1. The study population consisted of cancer patients (33 gastric cancers, 7 colo-rectal cancers and 15 hepatomas), 169 patients with benign gastro-enteric diseases and 72 healthy volunteers. The percentage of positive cases for TPA (higher than 200 u/l) was 61% in gastric cancer, 71% in colo-rectal cancer and 87% in hepatoma. In certain non-cancerous conditions, such as gastric ulcer (active stage), acute hepatitis and chronic hepatitis, the TPA levels were increased over the level of healthy volunteers. There was no significant correlation between TPA and the other tumor markers. Our study suggests that TPA may be useful in the identification and evaluation of cancer patients.

A case of metastatic islet cell carcinoma of the pancreas associated with the production of multiple polypeptide hormones (insulin, glucagon, and gastrin) is described. Three years prior to the histologic diagnosis the patient presented with a gastric ulcer and an androgen responsive pancytopenia with hypoplastic bone marrow. Discontinuation of androgen therapy resulted in relapse of pancytopenia. After the diagnosis of islet cell carcinoma of the pancreas was established, the patient was treated with 5-fluorouracil (5-FU) and streptozotocin and subsequently elevated serum polypeptide hormones returned toward normal levels. Concurrent with the normalization of peptide hormones another complete hematologic remission was achieved without use of androgens. Injection of the patient's serum into female rats produced a significant fall in leukocyte (P less than 0.02) and platelet counts (P less than 0.005), but no significant decrease in hematocrit. The clinical course and laboratory findings in this case suggest the presence of a previously undescribed serum factor released by an islet cell tumor capable of suppressing hematopoiesis.

A case of duodenal somatostatinoma is reported. The patient, a 54-year-old male, had complained of an epigastric pain due to gastric ulcer and a duodenal polyp was unexpectedly found at a gastrectomy. The polyp showed basically tubular adenocarcinoma, with negative argyrophil and argentaffin reactions. By an indirect immunofluorescent examination almost all of the tumor cells were revealed as somatostatin-immunoreactive. Big somatostatin was also positive. Radioimmunoassay of the tumor indicated 6400 pg of somatostatin-like immunoreactivity per milligram of wet tissue. This seems to be the second case of duodenal somatostatinoma, following the case reported by us previously.

Patients with stomach cancer, primary chronic gastritis (with intestine-type metaplasia of mucosa), old-age gastritis, post-resection gastritis, B12-deficiency-induced gastritis, stomach ulcer and chronic non-specific diseases of the lungs revealed a high sensitization to stomach cancer antigens matched by a low one to antigens of unaltered mucosa of the stomach. Patients with malignancies at sites other than the stomach, morphological patterns of gastritis as well as cases of primary chronic gastritis unaccompanied by intestinal metaplasia showed a reverse correlation: a high activity of T-lymphocytes with respect to unaltered gastric mucosa antigens and a low one--to stomach cancer antigens. An immunologic test using stomach and tumor antigens for screening of those at high risk for stomach cancer is suggested.
