This paper dealt with all patients who had visited their doctor in the gastroenterohepatologic (GEH) out-patient service in 1988. A total of 1716 and 2566 clinically subspecialistic gastroenterohepatologic and endoscopic surveys were made, respectively. Upper endoscopy was performed in 783 and lower endoscopy in 1783 patients. This paper was aimed at establishing the morbidity structure on one-year material from the patients visiting subspecialistic GEH service, whereby sex, age and frequency according to the kind of disease were taken into account. The most frequent diseases were as follows: ulcer disease 49.11%, biliary tract diseases 19.28%, liver diseases 14.62% and pancreas diseases 8.50%, while other diseases accounted for the less percent. The greatest morbidity ranged in patients from their fourth to sixth decade of life. Ulcer, liver and pancreas diseases were more frequent in males, while biliary tract diseases were more frequently found in females. Upper endoscopy revealed duodenic ulcer in 26.18%, stomach ulcer in 17.88%, gastritis and duodenitis in 18.52%, stomach polyps in 2.04% and stomach carcinoma in 1.92%. The analysis of 742 rectosigmoidoscopic patients showed hemorhoidal disease in 43.53%, chronic nonspecific collitis in 9.43%, rectosigmoidoscopic polyps in 7.15%, carcinoma of the rectum in 3.37% and ulcerous collitis in 5.66%. The ratio of polyps and carcinoma of the rectum was 2:1. In the out-patient GEH service apart from the basic health activity, health-educational, research and educative activities were performed. In the further development of this service, its scope of work, diagnostic and therapeutic procedures would be extended in dependence on economic power of society regarding the supply of equipment and manpower.

We evaluated whether assay of tissue polypeptide antigen (TPA) in sera is valuable for the determination of cancer stages compared to other tumor markers such as CEA, AFP, beta2-microglobulin, ferritin, and elastase-1. The study population consisted of cancer patients (33 gastric cancers, 7 colo-rectal cancers and 15 hepatomas), 169 patients with benign gastro-enteric diseases and 72 healthy volunteers. The percentage of positive cases for TPA (higher than 200 u/l) was 61% in gastric cancer, 71% in colo-rectal cancer and 87% in hepatoma. In certain non-cancerous conditions, such as gastric ulcer (active stage), acute hepatitis and chronic hepatitis, the TPA levels were increased over the level of healthy volunteers. There was no significant correlation between TPA and the other tumor markers. Our study suggests that TPA may be useful in the identification and evaluation of cancer patients.

A prospective study was performed to evaluate the epidemiological characteristics, clinical outcome and to determine the cause of bleeding in patients admitted to the Cayetano Heredia National Hospital with the diagnosis of gastrointestinal bleeding. Between August 1994 and May 1995, 100 patients were admitted, 86 patients with upper gastrointestinal bleeding and 14 with lower gastrointestinal bleeding. The mean age for the former was 45,25 years and the male/female ratio was 3.5/1. The main complaints were tarry stools and haematemesis in 56,9% and 26,7% only with melena. A history of non steroidal antiinflamatory drugs intake within 48 hours before the bleeding episode was obtained in 19,7% and alcohol ingestion was observed in 19,7%. In 36% of the patients a history of a previous episode of bleeding was obtained. The major causes of bleeding were duodenal ulcer in 38,3%, acute lesions of the gastric mucosa in 20,9%, gastric ulcer in 11,6% and esophageal varices in 5,8%, Mallory Weiss syndrome in 4,6%, gastric carcinoma in 2,3%, prolapse gastropaty 2,3%, esophagitis in 2,3%, esophageal ulcer 1,1 %, duodenitis 1,1%, arteriovenus malformations in 1,1%. The etiology of the hemorrhage could not be established in 8% of cases. The diagnostic rate of endoscopy was 91,7%.In 66% of the patients the endoscopy was carried out within the 24 hours of admission 20,9% of the patients had other potential bleeding lesions 9,1% of the patients had a new episode of bleeding during hospitalization and surgery was needed in 11,6%. Injectotheraphy was done in 7 (8,13%) patients (4 for variceal 2 for duodenal and 1 for gastric bleeding ) The overall mortality was 3,4% Fourteen patients with lower gastrointestinal bleeding were evaluated. The mean age was 53,14 years and the male/female ratio was 1.6/1. The main complaint was hematochezia. The major causes of bleeding were hemorrhoidal disease in 64,2% and rectal cancer in 14,28%.

We studied accelerated death benefit (ADB) claims at the Dai-ichi Mutual Life Insurance Company (Dai-ichi Life). The ADB provision is designed to pay all or a portion of the death benefit if the insured is expected to die within 6 months. Dai-ichi Life paid 243 ADB claims and did not pay 17 ADB claims between December 1994 and March 1998. Of the 260 ADB claims, 253 (97.3%) were caused by malignant neoplasm, 2 by intracranial hemorrhage, 2 by angina pectoris, 1 by dilated cardiomyopathy, 1 by hepatic cirrhosis, and 1 by bleeding gastric ulcer. The age range of the 243 paid claims at the time when the attendant physician predicted a life expectancy below 6 months was 21.6-72.6 years (48.7 +/- 8.7 years [Mean +/- SD]). By the end of March 2000, 236 cases were followed up among the above 243 paid ADB claims. Of the 236 followed-up cases, 149 (63.1%) died within 6 months and 203 (86.0%) died within 1 year. The range of survival periods of these 236 cases was 6-1516 days (210 +/- 237 days). Of the 217 dead cases due to malignant neoplasm, 45 (20.7%) died of gastric cancer, 44 (20.3%) of lung cancer, 24 (11.1%) of liver cancer, 16 (7.4%) of colon cancer, 13 (6.0%) of rectum cancer, and 12 (5.5%) of pancreatic cancer.
