In 1983, 685 out of 1,284 patients having suffered from a gastroscopically identified gastric ulcer in the period from 1972 to 1980 answered a questionnaire. Males prevailed in a proportion of 1,7:1. They showed an earlier onset of the disease and a higher operational rate. As much as 44.2% of them reported complications (mostly haemorrhage), 44.8% a gastric operation. A review of the operation reports showed an incidence of 1% of gastric carcinoma 1-3 years after gastroscopy. In order to demonstrate a better prognosis of gastric ulcer non-selected prospective long-term studies should be performed.

A case of a gastric ulcer with epithelial atypia following hepatic arterial infusion chemotherapy (mitomycin C and 5-fluoro-2'-deoxyuridine) for a metastatic carcinoid tumor in the liver was observed. The gastric ulcer was endoscopically detected about 2 months after the initiation of the hepatic arterial infusion chemotherapy. The gastric epithelial atypia was severe enough to cause erroneous cytologic interpretation of the smear as a carcinoma. The cytologic and histologic appearance of chemotherapy-induced epithelial atypia is described with special emphasis on possible diagnostic pitfalls in gastric cytology.

We report a case of a metastatic urinary bladder tumor from gastric remnant carcinoma. On August 23, 1984, a 70-year-old-woman visited us with the complaint of dysuria. She had undergone gastrectomy for gastric ulcer 25 years earlier. Cystoscopy revealed a non-stalk tumor in the dome of the bladder and the examination of the upper gastro-intestinal tract revealed gastric remnant carcinoma. We treated her with adriamycin, cis-diamminedichloroplatinum and mitomycin C but unfortunately she died of cachexia two weeks later. An autopsy revealed that the urinary bladder tumor was a signet ring cell carcinoma, metastasized from gastric remnant carcinoma.

A systematic analysis of the cellular components of intestinal metaplasia (i.e., goblet cells with or without brush border cells and Paneth cells) was performed in endoscopic gastric biopsies from 984 patients without gastric ulcer or carcinoma; 359 Swedish and 625 Japanese patients. The Japanese patients, matched for age and sex, had twice as much intestinal metaplasia as the Swedes. The frequency of goblet cells and columnar cells with brush (absorptive) border increased with increasing age in both ethnic groups. Complete intestinal metaplasia (i.e. the presence of at least two or all three cellular components) was twice as frequent in the Japanese. This may reflect differences in the environment (including food habits) of the gastric mucosa in the two populations. Structural mucosal changes (i.e. pseudo-villus formations) were found more than twice as often in the Japanese. Since pseudo-villus changes occurred as often as goblet cells and brush border cells in both groups, it is suggested that the above-mentioned mucosal structural change should be considered for inclusion among the histological criteria of intestinal metaplasia of the stomach. Incomplete intestinal metaplasia (i.e. the occurrence of goblet cells without the other cellular components) occurred in similar proportions in the two populations. The present findings support the theory that incomplete intestinal metaplasia may be a para- rather than a preneoplastic phenomenon.

In the periods between 1960 to 1973 and 1974 to 1984, 10 patients with stump carcinoma were observed for a total of 1399 and 618 gastric resections, respectively. Although the member of stump carcinomas is relatively low, it can be stated that the patients undergoing resection for gastric ulcer are more at risk in facing the development of stump cancer. It is striking that, even despite an improvement in diagnostic procedures, resectability has not changed. The prognosis of survival may afford some hope only in the case of an early diagnosis. Consequently, it is advisable to make an annual endoscopic control in the patients more at risk for having been resected for gastric ulcer.

This study was undertaken to determine whether the expression of ras oncogene product p21 can be used as a tumor cell marker of gastric and colonic carcinoma in brush smears. To detect p21 an immunocytochemical assay with RAP-5 monoclonal antibody was used. Benign epithelial gastric cells obtained from normal gastric mucosa or benign gastric lesions reacted negatively in 12 out of 13 cases. Similarly, benign epithelial colonic cells from normal colon or benign colonic lesions were negative for p21 in nine out of ten cases. Weakly positive reaction, confined to a few cell clusters only, was observed in one smear of a benign gastric ulcer and one smear of chronic ulcerative colitis. All 20 smears from colonic carcinoma and all 20 smears of gastric carcinoma contained cells that stained positively for p21, and the degree of tumor differentiation had no impact on the staining pattern. The results recorded in this study show that the immunocytochemical assay for the ras oncogene product may prove to represent a new tool for the cytodiagnosis of gastric and colonic carcinomas.

Surgically induced chronic gastric ulcers were created in rats. Cimetidine was administered for 1 year and the histological features of the gastric mucosa were studied to determine if cimetidine induced greater than normal proliferative or metaplastic changes or atypia. There were 60 rats in the study group and 51 and 55 in two control groups. The incidence of persisting chronic gastric ulcer in the cimetidine-treated study group at 1 year was 45% compared with 58% in non-treated control group (non-significant difference). There was no tendency for proliferative or metaplastic changes to be associated with cimetidine treatment--these changes reflected the presence of the chronic ulcers. Cimetidine did not induce atypia and no cases of gastric carcinoma occurred.

Biopsy specimens of human gastric mucosa of patients with gastric complaints and subjected to endoscopic examination were cultured microaerobically, and Campylobacter pyloridis was detected in 46 out of 80 cases (57.5%). The organism was found in 13 out of 22 patients with gastritis, 11 out of 16 with gastric ulcer scar, 7 out of 16 with gastric ulcer, 3 out of 9 with gastric polyp, 4 out of 5 with gastric carcinoma, 2 out of 2 with esophagus carcinoma, and 6 out of 9 with other gastric diseases. The isolates were identified as C. pyloridis, demonstrating its characteristic features such as positive for oxidase and catalase, negative for reduction of nitrite and nitrate, positive for urease, no growth at 25 C, growth at 37 C, not tolerant to 1% glycine, and resistant to nalidixic acid. Positive alkaline phosphatase activity was considered as an additional feature characteristic for the strains of C. pyloridis. The major cellular fatty acids were tetradecanoic acid and 19-carbon-cyclopropane acid. This pattern is unique among Campylobacter species. The survival of the organism for a longer period than 60 min at pH 2.5 indicates its significant resistance to acidic environment.

Analysis was undertaken of 693 endoscopies performed in Suva, from July 1984 to June 1986. Although Indians presented more often in the ratio of 5:2, Fijians had a higher percentage of abnormalities. Gastric ulcer and carcinoma were more common in Fijians, whereas duodenal ulcer dominated in Indians. Gastric ulcer size and location showed no racial differences. Erosive disease of the oesophagus, stomach or duodenal bulb was uncommon in both races. Fijians were much more likely to present with bleeding, most commonly from gastric or duodenal ulcer (equal numbers).

The expression of the carbohydrate antigen CA 19-9 in human gastric mucosa was studied by using the monoclonal 1116 NS 19-9 antibody, immunohistochemical ABC staining technique and formalin-fixed, paraffin-embedded biopsy specimens. Among 26 gastric carcinoma (GCA) patients CA 19-9 antigen was found in 16 patients (62%). The antigen was also found in 10 (48%) out of 21 non-cancer patients who had benign gastric ulcer and in 9 (12%) out of 74 non-ulcer, non-cancer patients. In non-cancerous mucosa, the CA 19-9 antigen was localized in the surface epithelium (foveolar type of staining) and/or in goblet cells and sometimes in absorptive epithelium of metaplastic intestinal glands (goblet cell type). The foveolar type but not the goblet cell type was related to the nonsecretor status of the subject, to Le positivity and to anti-H negativity of the gastric surface epithelium. It is concluded that, although being commonly present in GCA tissue, the expression of the antigen in benign gastric tissue is too common to advocate the practical value of CA 19-9 in predicting malignant gastric lesions in immunohistochemistry.
