The distributions of acid alpha1-glycoprotein, alpha1-fetoprotein, beta-galactosidase and gastrin in gastric carcinoma and
gastric ulcer
as well as in the neighbourhood of these lesions were studied by means of immunohistochemical methods on imprint preparation. We could not find significant differences between gastric carcinoma and the nonneoplastic lesions, except for the acid alpha1-glycoprotein. The results of this first study indicate that the immunochemical and immunohistological assay of acid alpha1-glycoprotein might be of practical value in diagnosing malignant changes of gastric mucosa.

Four patients who apparently had benign
gastric ulcer
(G.U.) were treated with cimetidine. The ulcers healed and their symptoms disappeared. However, when cimetidine was stopped the symptoms recurred. Intramucosal
cancer
was found only at histopathological examination of the resected stomachs in two of the four patients, and in all the cases
malignancy
had not been detected by the initial serial biopsies and brush cytology. Relief of symptoms of malignant gastric ulcers by cimetidine may delay diagnosis and appropriae treatment.

The incidence of gastric carcinoma was studied in a series of 210 patients presenting with apparently benign
gastric ulcer
. In eight cases (3.9 per cent), carcinoma was diagnosed within 18 months and was almost certainly present from the outset; in 2.4 per cent the diagnosis was delayed for over three months, and the five-year death-rate due to gastric carcinoma was 3.2 per cent. In a mean follow-up period of 5.7 years after the first diagnosis of an ulcer (8.2 years after first symptoms), three fresh cases of gastric carcinoma were found, and in two of these the
cancer
was at a different site from the ulcer. The five-year incidence of fresh gastric carcinoma was 0.6 per cent. Unsuspected superficial spreading carcinoma was detected by histology in 5.4 per cent of gastrectomy specimens. The significance of these findings is discussed in relation to the management of
gastric ulcer
and the early diagnosis of
cancer

The giant
stomach ulcer
can be defined as a crater measuring more than 30 mm in diameter. This variety of
stomach ulcer
represents 10-15% of the whole range of gastric ulcers, but they are not quite different from the nosologic point of view. It appears effectively that no etiopathogenic clinical or evolving particular factors can distinguish this kind of ulcer from the niches of normal size. To be taken into consideration is the risk of transforming into
cancer
which is multiplied by 3 or 5 times when compared with normal stomach ulcers, whence the importance of the endoscopy with many biopsies. In spite of the endoscopy the percentage of error is 5%, this is the reason for which, when in doubt, the more or less extensive gastric resection of the total gastrectomy is required with even more necessity then in normal cases of ulcers.

Eighty patients with chronic
gastric ulcer
, shown to have complete healing by gastric photography, were followed for 18 months to nine years. Ulceration recurred in 24, always at the site of the original ulcer, the majority (14) of relapses occurring within 12 months. Those with recurrent ulcers were more likely to show delayed healing of their initial ulcer, than the ulcers not shown to recur. At two months, this was statistically significant (P = 0.004) and even more so at three months (P = 0.004). Recurrences were more frequent in males (P = 0.09). There were no differences in ulcer site or size compared with those ulcers not known to have occurred. The appearance of complications such as perforation, haemorrhage, or
malignancy
was rare after complete healing.

Two cases of gastro-colic fistula occurring in analgesic abusers are described. In both patients, the fistulous communication was diagnosed at endoscopy and subsequently verified by upper gastrointestinal tract barium studies. Gastro-colic fistula is a rare complication of benign peptic ulcer disease. Whilst salcylates and cortico-steroids have been implicated as aetiological agents, abuse of compound analgesics has not previously been reported. Since the first description of gastro-colic fistula in 1755, thirty documented cases associated with benign
gastric ulcer
have been reported. Most often, gastro-colic fistula occurs secondary to gastric or colonic
malignancy
. Barium enema examination is the most accurate diagnostic study. Endoscopy has been confined to the visual inspection of the ulcer and establishing the benign nature of these lesions.

Seven elderly cases with reversible electrocardiographic changes simulating acute myocardial infarction in the absence of gross myocardial infarction on postmortem examination were observed following the blood transfusion. The underlying diseases were
cancer
of gastrointestinal tract or gall bladder in 4,
gastric ulcer
in 2, and 1 of pseudomembranous enterocolitis. The electrocardiogram revealed the abnormal Q waves with monophasic ST elevation and following coronary T inversion. These findings lasted only for 2 to 7 days and returned to the previous normal tracings. The hematocrit was elevated from 28.9 to 47.7 after the blood transfusion of 800 to 1,800 ml. The disseminated intravascular coagulation was shown in 5 cases. GOT levels were within normal ranges except 1 case. Pathological findings in cases with recent electrocardiographic changes were characterized by the mural thromboses, extending into the myocardium through the Thebesian vein. The focal small necroses of the adjacent myocardium or around the thrombosis of small vessels were also observed. In the later phase the fine interstitial fibrosis took place after the resorption of the thrombi and necrotic foci. From these clinical and pathological findings we proposed a new concept of reversible myocardial infarction induced from the hypercoagulability, disseminated intravascular coagulation, and elevated hematocrit.

With rabbit antibodies to nuclear 0.01 M Tris-HCl, pH 8, extract or "nucleolar preparations" of human HeLa S3 cells and fluorescein-labeled goat anti-rabbit antibodies, bright nucleolar immunofluorescence was observed in 61 or 63 human adenocarcinomas, squamous cell carcinomas, sarcomas, hematological neoplasms, and other malignant tumors. With these antibodies, nucleolar immunofluorescence was not found in 23 normal tissue specimens, 10 benign adenomas and hyperplastic tissues, and 8 specimens of inflammatory diseases. In the nontumorous tissues examined, positive nucelolar fluorescence was found in a few sections of a
gastric ulcer
and chronic ulcerative colitis which have been known propensities for malignant change; these areas may have been undergoing focal malignant changes.

Among 267 surgical specimens of histologically proved
cancer
of the stomach during the last 20 years (1957 to 1976), 25 were classified as early gastric cancer. Of these early gastric cancers, 16 were found during the last 5 year period. The increasing number of early gastric cancers found in the recent years may be attributed to the more aggressive diagnostic approaches in patients who have symptoms suggestive of peptic ulcer disease. Another factor which may contribute to the increased incidence of early detection of gastric cancer is a more aggressive surgical treatment in patients with a
gastric ulcer
. Some patients with early gastric cancer will remain undetected if the surgical treatment was vagotomy and pyloroplasty. Lymph node metastases were present in six patients in this series. Of these 25 patients who had gastric resection for early
cancer
, follow-up was available in 20. Two patients were noted to have local recurrence at 3 and 8 years, respectively, after the initial operation. One of these two patients died after reoperation and the other still is living. One other patient died of cerebral hemorrhage 12 years following the initial operation of gastric resection. The remaining 17 patients all are living, with no evidence of recurrence. From these results it is concluded that (1) routine gastroscopy should be done in patients with radiological gastric abnormalities and in patients with symptoms suggestive of peptic ulcer disease, even though the upper gastrointestinal series is normal; (2) multiple biopsies should be taken from gastric lesions seen on gastroscopy; and (3) gastrectomy is the treatment of choice, since some of the early gastric cancers in this series would have remained undetected if vagotomy and pyloroplasty had been performed.
