The clinical and neuropsychiatric features of seven workers from the grain storage industry are described. All were exposed to fumigants, especially carbon disulfide and malathion. Clinical manifestations included cogwheel rigidity, resting and action tremor, peripheral neuropathy, pyramidal tract signs, an EEG pattern of
sleep apnea
, and characteristic measurable neurobehavioral dysfunction. Similar signs and symptoms due to carbon disulfide exposure in the viscose rayon industry strongly supported the conclusion that these grain storage workers were suffering from carbon disulfide poisoning.
Malathion
exposure probably had a contributing role. We conclude that exposure to these fumigants poses a serious occupational risk to the nervous system.

Refreshing sleep requires both sufficient total sleep time as well as sleep that is in synchrony with the individual's circadian rhythm. Problems with sleep organization in elderly patients typically include difficulty falling asleep, less time spent in the deeper stages of sleep, early-morning awakening and less total sleep time. Poor sleep habits such as irregular sleep-wake times and daytime napping may contribute to insomnia. Caffeine, alcohol and some medications can also interfere with sleep. Primary sleep disorders are more common in the elderly than in younger persons. Restless legs syndrome and periodic limb movement disorder can disrupt sleep and may respond to low doses of antiparkinsonian agents as well as other drugs.
Sleep apnea
can lead to excessive daytime sleepiness. Evaluation of sleep problems in the elderly includes careful screening for poor sleep habits and other factors that may be contributing to the sleep problem.
Formal
sleep studies may be needed when a primary sleep disorder is suspected or marked daytime dysfunction is noted. Therapy with a benzodiazepine receptor agonist may be indicated after careful evaluation.

Sleep Apnea
Quality of Life Questionnaire (SAQLI) and The Functional Outcomes of Sleep Questionnaire (FOSQ). We examined the reliability, validity, stability and responsiveness of both questionnaires. Cronbach's alpha coefficient amounted to 0.94 for both questionnaires, which confirmed their reliability. Scores of both questionnaires highly correlated with measures of daily sleepiness, general health and SF-36 questionnaire scores. Both SAQLI and FOSQ gave stable results, but SAQLI was found to be more sensitive than FOSQ. The Polish version of FOSQ was proved to be a useful tool in cross-sectional assessment of HRQOL in patients with SAS. Stability and responsiveness of the Polish version of SAQLI and normal distribution of its scores make this questionnaire a preferable instrument in repeated assessments of HRQOL in the same group of patients.
