Several studies indicate that active oxygen species play an important role in the development of pulmonary disease (asbestosis and silicosis) after exposure to mineral dust. The present study was conducted to determine if inhaled fibrogenic minerals induced changes in gene expression and activities of antioxidant enzymes (AOE) in rat lung. Two different fibrogenic minerals were compared, crocidolite, an amphibole asbestos fiber, and cristobalite, a crystalline silicon dioxide particle. Steady-state mRNA levels, immunoreactive protein, and activities of selected AOE were measured in lungs 1-10 days after initiation of exposure and at 14 days after cessation of a 10-day exposure period. Exposure to asbestos resulted in significant increases in steady-state mRNA levels of manganese-containing superoxide dismutase (MnSOD) at 3 and 9 days and of glutathione peroxidase at 6 and 9 days. An increase in steady-state mRNA levels of copper, zinc-containing superoxide dismutase (CuZnSOD), was observed at 6 days. Exposure to asbestos also resulted in overall increased enzyme activities of catalase, glutathione peroxidase and total superoxide dismutase in lung. In contrast, silica caused a dramatic increase in steady-state levels of MnSOD mRNA at all time periods and an increase in glutathione peroxidase mRNA levels at 9 days. Activities of AOE remained unchanged in silica-exposed lungs. In both models, increases in gene expression of MnSOD correlated with increased amounts of MnSOD immunoreactive protein in lung and the pattern and extent of inflammation. These data indicate that the profiles of AOE are dissimilar during the development of experimental asbestosis or silicosis and suggest different mechanisms of lung defense in response to these minerals.

The association between silicosis and lung cancer mortality was estimated in 9,912 (369 silicotics and 9,543 nonsilicotics) white male metal miners. These miners were examined by the U.S. Public Health Service during 1959-1961 and were followed through 1975. Miners were excluded from this study if they were employed in a mine during 1959-1961 that used diesel equipment underground. The ores that were mined consisted of copper, lead-zinc, iron, mercury, lead silver, gold and gold-silver, tungsten, and molybenum. The standardized mortality ratio (SMR, U.S. white male rates) for lung cancer was 1.73 (95% CI: .94-2.90) in silicotics and 1.18 (95% CI: .98-1.42) in nonsilicotics. Additionally, SMRs were higher in silicotics than in nonsilicotics, even in most subgroups stratified by cigarette smoking habit, type of ore mined, years of service in an underground job, radon exposure group, or year of hire. When lung cancer mortality between silicotics and nonsilicotics was compared, the age-adjusted rate ratio (95% CI) was 1.56 (.91-2.68), and the age- and smoking-adjusted rate ratio was 1.96 (.98-3.67). Corresponding figures for miners who were employed in mines with low levels of radon exposure were 1.90 (.98-3.67) and 2.59 (1.44-4.68), respectively. These findings indicate that lung cancer mortality risk was increased in silicotics, and this probably did not result from chance or bias. However, confounding from radon exposure could not be ruled out. The findings indicate that further follow-up of this cohort is needed.

Although most were unknown a few years ago, present evidence indicates that at least 25 trace elements have some pertinence to health. Unlike vitamins, they cannot be synthesized. Some trace elements are now considered important only because of their harmful effects but traces of them may be essential. Zinc is especially important during puberty, pregnancy and menopause and is related to protein metabolism. Both fluoride and cadmium accumulate in the body year after year. Cadmium is positively correlated with several chronic diseases, especially hypertension. It is obtained from smoking and drinking soft water. Silicon, generally associated with silicosis, may be necessary for healthy bone and connective tissue. Chromium, believed to be the glucose tolerance factor, is obtained from brewer's yeast, spices, and whole wheat products. Copper deficiency may be implicated in a wide range of cardiovascular and blood related disorders. Either marginal deficiencies or slight excesses of most trace elements are harmful. Nurses should instruct patients to avoid highly refined foods, fad diets, or synthetic and fabricated foods. A well balanced and varied diet is the best safeguard against trace element excesses or deficiencies.

Two cases of extrathoracic silicosis in buffaloes raised near a quartz quarry and suffering from clinically severe silicosis are described. The extrapulmonary changes were characterized by silicoconiotic nodules in the tonsils, mesenteric lymph nodes and spleen. A combination of energy dispersive X-ray microanalysis and scanning electron microscopy revealed that the mineral component of these lesions consisted mainly of silicon, aluminium, iron, calcium, magnesium, zinc, sulphur and potassium. It is concluded that domestic animals raised in polluted environmental conditions represent an important biological source from which helpful data may be obtained for assessing risks to human health and gaining new insight into pathogenetic mechanisms.

Though metals represent the largest group of elements they rather rarely cause respiratory diseases. This article will therefore review the most important ones caused by inhaled dusts of metals and some of their inorganic compounds, but leaving aside silicosis and silicatosis as well as iatrogenically induced metal pneumopathies. Among toxic inflammatory diseases metal fume fever, an influenza-like condition caused by zinc oxide, ranks as the commonest. Activities such as oxi-acetylene cutting and welding of zinc covered metal pieces account for about 90% of all cases compensated in Switzerland. Due to the non-recurrent character of this type of work, the typical waning of symptoms while exposure is going on has become seldom. Toxic pneumonia caused by inhaled metal fumes occurs rather seldom. However, serious cases have been reported where soldiers were exposed to zinc chloride from smoke bombs. The existence and extent of chronic airflow limitation due to occupational exposure to metallic dusts have not been widely examined but are to be assumed when there is poor occupational hygiene. Concerning asthma, there are at least four metals and several of their compounds which have been proven to cause variable airway narrowing, namely chromium, nickel, platinum and cobalt (the latter as hardmetal). Platinum complex salts (chloro-compounds) are very potent sensitizers leading to a notable prevalence of asthma among exposed workforces. Nevertheless, there have been no such cases in Switzerland for more than ten years. Hard-metal not only causes asthma but also an alveolitis-like interstitial lung disease progressing to fibrosis.(ABSTRACT TRUNCATED AT 250 WORDS)

Zambia is endowed with mineral wealth that includes copper, cobalt, gold, nickel, lead, silver, uranium, zinc, and numerous precious and semi-precious stones. Mining activities are predominantly found on the Copperbelt and North-Western Provinces, although these minerals are dotted all over the country. Copper mining in Zambia dates back to the 1900s and this period witnessed massive investment in mine development with concomitant increase in support facilities including building of new towns, roads and other commercial infrastructure. The mining sector has therefore evoked considerable national attention for its potential to contribute towards economic growth, job creation and poverty alleviation. However, mining and mineral processing by its very nature comes with environmental costs and the effects can continue long after the mining has stopped. The aim of this article was to review the relevant publications on the impacts of air pollution arising from mining operations with respect to human health, plants, animals and infrastructure and synthesize the views of researchers and suggest any additional research required to inform policy and remedial actions. This review has revealed that there is a paucity of studies on mining-related air pollution in Zambia. The main identified air pollutants were SO2 and particulate matter (PM), both fine and ultrafine (PM10, PM5.0, PM2.5 and PM0.1). The main sources of these pollutants were flue gases from smelter operations and dusts within the mines and those blown from both operational and abandoned waste rock, overburden and tailings dump sites. The identified occupational diseases for miners in Zambia were silicosis and tuberculosis, which have been compounded by the prevalence of HIV/AIDS. In the hotspot townships of air-borne exposures from smelter emissions in Mufulira, ambient air SO2 levels exceeded the 'safe' limits of international and National standards. Moreover, the top soils have turned acidic and have become laden with heavy metals (Pb, Zn, Cu, Co and Fe). These metals were also found in the dust deposited on leaves of crops. There were also visual signs of impaired vegetation cover and corroded housing infrastructure in the affected areas. In the vicinity of the abandoned Pb-Zn mine in Kabwe, the soils have been contaminated by heavy metals and pathological lead poisoning of children and wild mammals have occurred. The review article has further examined study gaps and suggested areas that need further research in order to address the challenges arising from the legacy of copper mining in Zambia. These include comprehensive PM characterization from mining environments, extent of occupation exposure to air pollutants, efficiency and efficacy of airborne control technologies, health risks and epidemiological studies in mining towns, and the influence of exposure to PM on pulmonary tuberculosis and HIV/aids among miners.
