In order to study the lung defence mechanisms and the mucociliary transport system scanning electron microscopy was performed on biopsy specimens obtained at bronchoscopy under local
anaesthesia
. There were great variations in the appearance of the cilitated epithelium from the large bronchi in 40 patients with bronchial carcinoma, sarcoidosis,
silicosis
, tuberculosis and other lung diseases. Areas with a low frequency of cilia were observed mainly in granulomatous disorders, and in
silicosis
, atypical cilia occurred abundantly. Squamous metaplasia with a complete loss of cilia was found in chronic bronchitis, bronchial carcinoma, after radiotherapy and in two of seven cases of sarcoidosis. Scanning electron microscopy seems to be an excellent method for studying the surface of the bronchial epithelium in many lung diseases, and may prove to be helpful in occupational medicine.

The authors report 93 observations during which OCB were performed under local
anesthesia
. They insist on the validity of the method which can be used on fragile patients or on those with respiratory insufficiency for whom an exploring thoracotomy would be contra-indicated. Incidents were minor and momentary (pneumothorax, pleural reaction). It is mostly in diffuse lesions of miliary and fibrotic nature that this technique was used to identify various lesions :
silicosis
, sarcoidosis, tuberculosis, histiocytosis, etc... In a third of the cases, lesions were not specific and could not be definitely identified in parieto-alveolar fibrosis or inflammatory alveolitis.

A bilateral angiographic study of the upper limbs was carried out under general
anaesthesia
in a group of 81 subjects--mostly (91.3%) miners--using vibrating tools. The patients' age ranged from 30 to 57 years; the time they worked as vibrating tool operators varied from 5 to 36 years; the duration of symptoms ranged from 3 months to 10 years. The mean values were 46.1, 20.9 and 3.7 years, respectively. Seventy-four subjects (91.3%) complained of Raynaud's-like phenomenon, which in 36 (44.4%) of them was elicited too, although partially, by provocative tests (cold exposure). Angiography was carried out under general
anaesthesia
maintained with halothane. Bilateral angiograms were available in 61 instances; in the remainder, only either right or left angiograms could be obtained. In all the subjects examined arteriographic lesions were found, mainly represented by organic stenoses and occlusions. The prevalance of such lesions was minimal at the level of the forearm arteries, and maximal at the level of the digital arteries. The 2nd finger, bilaterally, was the most frequently involved; the index finger on either side and the middle one on the right side were the most involved as to severity and extent of lesions. The 1st finger, however, was quite frequently involved too. No definite relationship between prevalance, severity and extent of arterial lesions, on one hand, and some history, clinical and functional parameters (such as age, vibrating tool usage time, smoking habit, hand arterial pattern, pulmonary
silicosis
, instrumental findings and Raynaud's-like phenomenon), on the other hand, could be established. As to Raynaud's-like phenomenon, it is hypothesized that such a finding could be, at least in part of cases, subsequent to the previous development of organic lesions in the digital arteries.

On the basis of our own experience the importance and usefulness of the scalene-node-biopsy for the diagnosis of intrathoracic diseases will be reported. If there is no positive histological result in the obtained lymph nodes during surgery, one can continue with the mediastinoscopy (Carlens) without aggravating Daniels biopsy. The scalene-node biopsy is a short surgical procedure, that can easily be done in local
anaesthesia
. A positive lymph node makes mediastinoscopy or a following thoracotomy unnecessary. Especially in sarcoidosis, systemic diseases (malignant lymphomas) and
silicosis
biopsy yields good results.

Three adult female North American river otters (Lutra canadensis) demonstrated severe anesthetic complications shortly after being immobilized for dental procedures. Two of the animals died shortly after
anesthesia
, and the third otter died 2 mo after immobilization. All three animals were diagnosed with pulmonary
silicosis
on the basis of histopathology, polarized light microscopy, and mass spectrometry. One animal also had primary pulmonary bronchoalveolar carcinoma. Significant concurrent disease was not found in other organs. Analysis of the lung tissue by laser microbe mass spectrometry confirmed the presence of an aluminum silicate in the lung tissue associated with insulation material used in nest box construction.

Pulmonary alveolar proteinosis (PAP) is a rare pathology characterized by accumulation of phospholipoproteinaceous material within the alveoli. The evolution of PAP is variable and treatment modalities are limited. Pharmacological therapeutic targets are being actively developed, but whole-lung lavage (WLL), first described in the 1960s, remains the cornerstone of therapy. The preferential treatment for PAP in our center is sequential WLL, where each lung is separately and sequentially perfused with warmed saline. However, some patients do not tolerate single lung ventilation (SLV), as there is a greater risk of severe hypoxemia with this method. Extracorporeal membrane oxygenation (ECMO), referring to an extracorporeal circuit that directly oxygenates and removes carbon dioxide from the blood, may be considered in highly selected patients with severe respiratory failure who otherwise would not be able to undergo WLL. In this context, veno-venous ECMO is most often utilized. We describe a case of a 44-year-old male diagnosed with

five years earlier who presented with severe hypoxemic respiratory failure not amenable to WLL under general
anesthesia
with SLV, which was successfully managed with ECMO-assisted WLL.
