A case of 70-year old male manifesting a simultaneous adenocarcinoma, malignant lymphoma and silicotuberculosis in the same lung has been reported. The patient, who had worked as a miner for 24 years and had been treated for silicosis 11 years previously, was found to have a new, abnormal shadow during a routine chest X-ray. Later, sputum cytology revealed the adenocarcinoma. He was treated with chemotherapy, but died 24 months later, his death attributed to lung cancer, complicated with respiratory insufficiency and weakness. An autopsy confirmed the presence of an adenocarcinoma and silicotuberculosis, and a histological examination revealed the coexistence of malignant lymphoma in the same lung.

After reports appeared from other countries indicating an excess risk of lung cancer among silicotics, a cohort of workers compensated for silicosis during the period 1959-1963 in the Veneto region of Italy was constructed and followed for mortality through 1984. The results of the study showed a large mortality excess for infectious diseases (180 observed versus 9.5 expected), due to silicotuberculosis, and for diseases of the respiratory system (270 observed versus 33.5 expected) due to silicosis. An elevated standardized mortality ratio of 239 (70 observed versus 29.3 expected) from lung cancer was also detected. An increasing pattern was observed with time since first exposure, while the relationship with employment category and duration of exposure was less clear-cut. The lung cancer excess was also strongly associated with cigarette smoking, there being a dose-response relationship with daily cigarette consumption. The study confirms the results from other epidemiologic studies on silicotics which show this pathological condition to be associated with increased lung cancer mortality.

Several recent studies (animal and human) have suggested an association between lung cancer and silica exposure. To test the hypothesis, we have studied death benefit records of 1,905 members of the Granite Cutters Union. A proportionate mortality analysis (PMR) was conducted, using U.S. deaths as a comparison population. Statistically (PMR) was conducted, using U.S. deaths as a comparison population. Statistically significant excesses were observed for death from nonmalignant respiratory significant excesses were observed for death from nonmalignant respiratory disease (largely silicosis) (183 obs, 43.7 exp) and for tuberculosis (largely silicotuberculosis) (262 obs, 19.3 exp). Other significant excesses were observed for bone cancer (6 obs, 1.9 exp) and arthritis (5 obs, 1.5 exp). A significant decrease was observed for leukemia (5 obs, 13.0 exp). For lung cancer a slight but nonsignificant excess was observed (97 obs, 81.1 exp, PMR = 1.19, 95% CI 0.97-1.46). A proportionate cancer mortality analysis (PCMR) showed similar results for lung cancer (PCMR = 1.09, 95% CI 0.89-1.33). Lung cancer mortality also failed to show any trend with either calendar time or duration of exposure. Although no significant excess of lung cancer was observed for the entire silica-exposed cohort, there was an indication that those who were silicotic had an excess risk of lung cancer, based on a review of contributing causes on the death certificate.

The use of short-course chemotherapy in silicotuberculosis in the goldmining industry in South Africa is described in a comparative trial of 94 cases of silicotuberculosis and two matched-pair control groups with pulmonary tuberculosis only. The theoretical and practical considerations of short-course chemotherapy are discussed. It is concluded that this is an effective means of treatment for pulmonary tuberculosis in patients with silicosis as well as in patients with pulmonary tuberculosis only, irrespective of previous exposure to silica dust. There are no valid reasons why the basic tenets of short-course chemotherapy for the treatment of pulmonary tuberculosis should not apply in silicotuberculosis.

In 1974 and 1975, a selective tuberculosis screening program was conducted in a small mining community in Ottawa County, Oklahoma. Former miners, 50 years of age and over, who had abnormal chest radiographs consistent with silicosis or inactive tuberculosis and positive tuberculin skin tests, were offered isoniazid preventive therapy. Since the project ended, morbidity from silicotuberculosis has declined but silicotuberculosis has continued to occur. In this report, we examine the reasons for the failure to eradicate this disease from the community.

In spite of a decreasing number of new cases of silico-tuberculosis even today there still remains a serious complication of silicosis. The job of radiology is to recognise the disease, evaluate the course of the disease during therapy and classify the disease for compensation purposes. Due to the pathogenetic and pathomorpohologic similarities of silicosis and tuberculosis, it is often difficult and sometimes even impossible to recognise the presence of tuberculosis in cases of silicoses or to identify and isolate the TB component in silicotuberculosis. Careful consideration of all radiological and clinical parameters improves the accuracy of diagnosis. Since the radiographic examination provides the only method of evaluating the morphologic state of the disease, radiology will keep its central position in the diagnosis of silicotuberculosis.

After retrospective evaluation of numerous expertises of patients exposed to dust with and without silicosis the high value of the arterial partial oxygen pressure and of the ergooxytensiometry in the estimation of the cardiorespiratory functional breadth is explained. On an average in patients with silicosis III and silicotuberculosis, respectively, more frequently a respiratory insufficiency is found than in patients exposed to dust with lower stages of silicosis or without clue to the presence of a silicosis. In the individual case there is no firm relation between the x-ray-morphological findings and the result of the ergooxytensiometry. The roentgenological stage of silicosis does not allow a conclusion to the loss of function, on the contrary, the frequently existing bronchitis in patients exposed to dust plays an important part in the development of the disturbances of the pulmonary function.

Increased mortality from cancer of the lung has been found in several studies of miners exposed to high levels of radioactivity in underground air. In view of their exposure to raised levels of radiation, we have studied the mortality of a group of men recorded as Cornish tin miners in 1939. Using occupational description, a crude classification of exposure was derived for these miners. The meaningfulness of this classification was supported by differences in mortality from silicosis and silicotuberculosis. A twofold excess of cancer of the lung was found for underground miners, while for other categories mortality from this cause was less than expected. This supports the findings of previous studies on exposure to radon and its daughters. An excess of cancer of the stomach was also observed among underground miners.

The prospective study covered some principles of silicosis formation in workers of polymetallic mines. Mild and mainly diffuse sclerotic type of silicosis was revealed in 13% of the examinees with suspected silicosis, no additional X-ray data and no history, who were under exposure to the dust during 8-10 years. Variably calcified thoracic lymph nodes prove prominent risk of nodular silicosis (16.7%) and mostly progressive silicotuberculosis (50.0%). Increased vascular markings primarily in lower lungs and history of frequent respiratory infections and repeated pneumonia shows diffuse sclerotic silicosis complicated by chronic bronchitis. The data obtained necessitate further improvement of occupational safety and differentiated treatment and prophylaxis to prevent silicosis with its complications.

Prevalence of the basis types of pneumoconioses (silicosis, silicotuberculosis, asbestosis) in Bulgaria was studied during a 7-year period (1985-1991). Information was taken from reports of the Department for silicosis and other pneumoconioses control in the country. As a basis of the investigation serves the total number of workers endangered by dust exposure in different risk branches. The more rarely encountered pneumoconioses are referred to in absolute numbers. Trends in the development and prognosis of the most frequently encountered pneumoconiosis in Bulgaria could be followed. Recommendations are given for future preventive activity.
