124 newborns admitted to the Department within the period of 3 years with suspected sepsis were analysed. In 55.6% of children, boys prevailing (62.3%), generalized infection was confirmed with positive bacterial cultures. Late-onset disease predominance was found (72.8%). The mortality rate in this group was 5.8% (11.3% boys and 1.9% girls). Analysis of the causative organisms revealed more frequently the Gram positive bacteria (81% of all positive cultures) with a predominant role of Staphylococcus epidermidis (43.5%). The pathogenic role of Staphylococcus epidermidis (regarded by some other authors as a contamination) was stressed. Its increasing clinical significance has been correlated with a deterioration of sanitary and hygienic conditions at the obstetric and neonatal wards, wider application of invasive procedures and antibiotics.

Four cases of extraintestinal salmonellosis caused by Salmonella enteritidis were described. Underlying diseases in the three patients were haematological neoplasms (2 splenectomized and 1 with massive leukemic infiltrations of the spleen) and in the fourth haemophilia B: only that patient had a prior symptomatic intestinal infection. Blood cultures for S.ent. were positive in all patients and additionally in that suffering from Hodgkin's disease urine, lymph node and stool cultures also showed S.ent. In the haemophiliac patient culture of suppurated hematoma was positive. All patients recovered from S. sepsis but three then died of their neoplastic diseases. The haemophiliac patient is in a good condition. Various disturbances in the immunological tests were observed.

We present a boy who had been under observation for 4 years, suffering since the age of nine from necrotizing vasculitis, which appeared during Proteus sp. sepsis. Complex treatment with antibiotics, large doses of steroids, antithrombotic and peripheral circulation improving drugs, as well as local drugs applied to the skin and oral cavity mucosa, resulted in a complete cure. This illness left some cosmetic skin defects on the face and extremities, a shortening of the tongue and malocclusion.

Diagnostic difficulties in a case of a large tumor localized in the deep tissue of buttock in a 29-year old woman is described. The patient was very severely ill presenting with neurological symptoms and immobilization of the left coxal joint, what strongly suggested a neoplasm and this was supported by the CT scan. USG examination allowed diagnosis of a deep hematogenic staphylococcal abscess of the buttock, probably due to extensive oral sepsis and staphylococcal septicaemia. Systemic antibiotic therapy as well as evacuation of pus caused a complete recovery of the patient.

There is presented an atypical case Wegener's granulomatosis of maxillo-nasal region without bony destruction. The case is unusual because of sepsis and purulent coxitis. The patient responded well to treatment with vincristine and cyclophosphamide. ANCA assays have very good sensitivity and specificity for Wegener's granulomatosis.

The authors described a course of pregnancy and delivery of pregnant women who was admitted to the hospital in the third trimester with symptoms of sepsis in course of the pyelonephritis. Signs of a kidney insufficiency and pulmonary oedema were observed. During hospitalization ultrasound examination showed polycystic kidneys.

We present a case of a 17-year old patient with extreme hepatosplenomegaly, hyperthrombocytosis, hyperleucocytosis and the presence of myelo- and megakaryoblasts in the peripheral blood film. Numerous complications that occurred in the course of the disease made cytostatic treatment difficult. Since Ph chromosome and hybrid gene bcr/abl were absent, the diagnosis of unclassified chronic myeloproliferative syndrome in the phase of blast crisis was established. Immunophenotyping confirmed a mixed myelo- megakaryoblastic character of the crisis. In the differential diagnosis other myeloproliferative syndromes were taken into account including i(17q) syndrome. The patient died after a 13-month observation due to neoplasm progression and sepsis.

Aim of our study was making easier choice of right antibiotic to treatment newborns with sepsis after receiving results of bacteriological test. We analysed antibiograms that were results of first bacteriological test of blood samples taken from 59 neonates, who were admitted in first week of life to the Neonatal Pathology Clinic in Zabrze. On the ground of our analysis we propose applying netilmicin and cephalotin in every case of neonate with symptoms suggesting the sepsis. These two antibiotics have the best effects in treatment the sepsis.

PMID:[An attempt to choose the optimal antibiotic for early treatment of neonates with bacterial sepsis]. 864 84

The aim of our work is to estimate usefulness of morphological blood tests, as simple to make and easy to estimation, in screening diagnostics of neonatal infections. We chose the following indications for estimation, as possible to obtain in every hospital: total leucocyte count, total neutrophil count, total immature neutrophil count, total thrombocyte count, ratio of immature neutrophil count to total neutrophil count (I : T), ratio of immature neutrophil count to mature ones. We examined 143 newborns admitted to the Neonatal Pathology Clinic in Zabrze in first week of life, divided into three groups: with sepsis, with suspicion of sepsis, and without infection. We assume that deviation of (at least) three indicators or two (necessary with I : T) is incorrect result. We obtain sensitivity: 54.35% for sepsis, 50% for suspicion of sepsis, and 52,70% for both groups (together with infection), and specificity 98.55%. We consider that our haematological scoring system is helpful in early diagnostics of neonatal bacterial infections.

The authors present rare case of splenic necrosis during meningococcal sepsis in an eight-month-old infant. The diagnosis was based on ultrasonographic examination and confirmed by CT. These investigations were conducted because of splenomegaly and gastrointestinal tract disturbances. Splenectomy gave good results.
