The authors have treated 399 fractures of the femoral shaft by blind nailing with reaming following Kuntscher's technique. They fully describe the necessary technical details for a safe operation. They emphasize the importance of an adequate instrumentation and report peroperative complications and the way for avoiding them (poor reduction of the fractures, difficulty for bridging the fracture site, creating of additional fragments, blockage of reamers or nails in the medullary canal). Indications and results are described. I non-union, 15 malunions and 4

The authors advocate the technique of blind medullary nailing with reaming of the medullary canal followed by a several weeks-traction in the treatment of compound fractures of the femoral shaft. This method is specially indicated in bursts of the bone, when no stability can be obtained after osteosynthesis and was used in 44 cases. In the same time blind nailing without traction was applied to 272 cases. The results were:

PMID:[The nailing of fractures of the diaphysis of the femur by Kuntsher's method. A report of 44 cases]. 13 56
The authors have treated 41 cases by Papineau's technique. There were 19 cases of osteitis, and 35 septic non-unions (9 in the femur, 16 in the tibia and 10 in other sites). Twenty-four cases healed, the average follow-up being one year. The causes of failure were analysed. Some are related to the technique itself -
sepsis
and fractures at the sites of graft removal, inadequate quantities of cancellous bon, long delay in bone union and skin problems particularly in the leg. The indications for this technique are compared with those of other techniques. It is concluded that it is a useful technique; only two patients required amputation.

The authors have studied the results of attempts to restore joint movement in stiff knees after septic non-union of fractures of the femoral shaft or after septic arthritis of the knee by arthrolysis and quadriceps release. Fifteen procedures were done on fourteen patients with severe limitation of movements. The results were gratifying; eight very good and four good. No case was made worse. The technique is described in detail and operative and post-operative complications described. A long-term follow-up study showed that the results tended to improve after several years. The prognosis can be based on the degree of flexion obtained during operation, though free flexion should be distinguished from forced flexion. The authors have observed three instances of
sepsis
due to Mycobacterium fortuitum complicating total hip replacement for osteoarthritis. The case histories are fully described. The requirements are given for recognition of the organism which grows on special culture media and which may be mistaken for Mycobacterium tuberculosis. This feature explains why, in some cases, the organism may not be discovered. Antibiotics were ineffective but the general condition of the patient was not greatly affected.

The author presents a series of 112 patients, suffering an infected fracture or nonunion of the tibia, and operated on by intertibio-fibular grafting (ITFG), between 1960 to 1990. 110 of them were followed up during average 5 years. Some points of the technic are detailed. The whole treatment associates immobilization, antibiotherapy, cleaning procedures and eventually complementary grafting. All patients were consolidated, 73 after ITFG alone, 13 by association, in the same stage, of an open cancellous filling, following Papineau technique. 24 needed a complementary grafting, among them 7 because a technical flaw. 5 iterative fractures were observed, only one of them has not united. 96 patients were cured of any
sepsis
, most often by associated cleaning procedures, achieved either before or after ITFG. It existed 14 persistent draining sinus, but only 4 may be considered as real failures. The involvement of the ankle joint was unfortunately quite frequent, but it did not seem to be the consequence of the synostosis, but rather of the duration of immobilization. After review of the literature, the other methods of treatment were discussed, particularly these of Papineau and of Ilizarov. The conclusion was that each method has indications, but the ITFG must keep the largest place, being indicated in about two cases out of three.

PMID:[Role of tibial-fibular grafting in the treatment of infected pseudarthrosis of the tibia]. 130 38
This retrospective study was based on 141 diaphyseal fractures of the humerus, treated by Hackethal fasciculated pinning, among 371 fractures followed up during 10 years in our department. There were six preoperative radial paralyses. The mean consolidation delay was 65 days. Six fractures did not unite and there was no
sepsis
. The only immediate neurological postoperative complication was a regressive cubital paralysis. 72 fractures could be followed-up with a mean of 4 years, to establish a functional result chart. 94.4 per cent of the results were good and very good, 2 shoulder stiffness and only one elbow stiffness. The displaced fractures of the humeral diaphysis on D2 to D5 zones, as well as pathological fractures are good indications for Hackethal fasciculated pinning.

The authors have made an evaluation of the results obtained in orthopaedic surgery and traumatology after a four year practice at the Korhogo hospital (Cote d'Ivoire). They analyse the peculiar conditions bond to the pathology, climate, environment, technical level. The incidence of post operative
sepsis
was 2.8 per cent, comparable to european statistics. Bed sores were exceptional, the general rate of nonunions was 2.9 per cent. The functional results were hardly analysable. However they seemed to be rather satisfactory despite the lack of post operative rehabilitation. It is concluded that an orthopaedic and traumatologic practice can be valuably conducted in Africa even far from the main centers, provided a rigorous organisation.

Fifty-six renal transplanted patients having an average age of 33 years have been operated on for necrosis of the femoral head from 1973 to 1988. They totalise 85 cephalic necrosis due to the therapeutic peaks of corticotherapy. The femoral condyles were affected by the necrosis in 21 patients, and the humeral head in 16 patients. 82 surgical operations of first intention and 18 resumptions leading to 100 the total number of operations. The 26 drilling gave place to 11 by arthroplasty. The good results of the drillings only concern the infraradiological stages and the MRI allows to detect the non symptomatic stages 0. The 33 intermediate prosthesis did not lead to septic complications but 9 cotiloidal alterations of which 4 important with a resumption by total prosthesis. 36 total prosthesis were realized out of which 13 for resumption. Two early and two late suppurations after
septicemia
complicated the evolution. If at stage 0 or 1 the drilling seems to remain a reliable indication, the total prosthesis is the only possible solution for invalidating stages IV, the intermediate prosthesis seems to be able to keep a place in the intermediate stages.
