During 10 years 3000 patients were operated upon for chronic calculous cholecystitis. Twenty of these patients died during 30 days after operation, in 16 of them death resulted from incompetence of the vital organs. Endotoxicosis and polyorganic insufficiency after technically correct operations for chronic calculous cholecystitis are thought by the authors to be a variant of anaerobic infection with possible development of sepsis.

The characteristics of the natural history of acute sepsis and the nature of the intoxication syndrome are analysed an on ample material (120 patients), and microbiological and immunological diagnostic findings are shown. Endotoxicosis in sepsis is attended by marked disorders of the immune status in the form of varying degrees of immunodeficiency. The authors generalize their experience with the management of endotoxicosis in sepsis. The article first shows the results of the combined use of various methods of endotoxicosis correction (homosorption, hemodiafiltration, plasmapheresis, endolymphatic therapy, ultraviolet irradiation of autoblood, hyperbaric oxygenation, application sorption) in the treatment of 120 patients with sepsis of various etiology. A biophysical method for assessment of hemoperfusion and intensive care efficacy in patients with endogenous intoxication syndrome is suggested. Endotoxicosis developed because of sepsis in 31 patients, because of acute hepatic insufficiency due to cholestasis, hepatitis, or mechanical jaundice in 12. Blood magnetic susceptibility was studied by Gui's method. The findings indicate that changes in blood magnetic susceptibility correlate with changes in medium-molecule level and leukocytic index of intoxication. The suggested method helps detect endotoxicosis whatever the pattern of its development and permits assessment of therapy efficacy.
