Fundamental and clinical studies on cefotetan (CTT), a new cephamycin antibiotic, were carried out under a joint study programme in pediatric field, and the following results were obtained. Pharmacokinetic study In 20 pediatric patients with normal renal function, weighing 15 to 48 kg, CTT was injected intravenously at 20 mg/kg in 3 to 5 minutes. The mean blood concentration of CTT was 215.6 micrograms/ml at 15 minutes after the end of injection, 90.7 micrograms/ml at 1 hour, 57.2 micrograms/ml at 2 hours, 33.9 micrograms/ml at 4 hours and 10.2 micrograms/ml at 8 hours. The half-life of the drug in the beta-phase, computed from the mean blood concentrations up to 8 hours postdosing, was 2.61 hours. The peak of the mean urinary excretion of cefotetan appeared in 0 to 2 hours after the injection and 36.5% of the dose was recovered in the urine. The mean excretion at 0 to 8 hours was 68.1%. Clinical study Clinical effects of CTT was evaluated in 285 patients with 287 diseases, since 1 patient had both pneumonia and erysipelas, and another both pneumonia and acute otitis media. Daily dosage of CTT ranged from 15 to 123 mg/kg, and 266 patients (93.3%) received the drug either 2 or 3 times daily. The clinical response was seen in 83.3% of the 6 cases with
sepsis
, 89.3% of the 122 cases with pneumonia with or without pyothorax, 96.2% of the 52 cases with either
acute bronchitis
or tonsillitis, 92.5% of the 67 cases with urinary tract infection and 92.5% of the 40 cases with other infections. The causative organisms were detected in 160 patients and the rate of complete disappearance was 80.6%. Out of 310 patients, side effects were seen in 9 cases, diarrhea in 8 (2.6%) and rash in 1 (0.3%). Abnormal clinical laboratory findings were seen in 24 cases, elevation of serum transaminases in 19 (7.8%), elevation of TTT and LDH in 1 (0.4%) and eosinophilia in 4 (1.6%). None of these cases showed serious side effects or abnormal clinical laboratory findings. From the above results, it is concluded that CTT is one of the useful drug for treatment of infections in pediatric field.

Twenty-four pediatric patients with infections were treated with ceftazidime (CAZ) by one-shot intravenous injection in the doses of 39 approximately 149 mg/kg/day in 4 divided doses as a rule. These patients' ages ranged from 2 months to 13 years 4 months. The duration of the administration ranged from 4 to 19 days, and total doses ranged from 1.38 to 57 g. Infections consisted of respiratory tract infections in 19 cases (acute tonsillitis in 3,
acute bronchitis
in 7, and pneumonia in 9), urinary tract infection in 1 case, acute peritonitis in 1 case, and suspected
sepsis
in 3 cases. Clinical efficacy was excellent in 18, good in 1, fair in 1, and poor in 4 cases, and the efficacy rate (excellent + good) was 79.2%. Bacteriological response was evaluated on 14 strains of bacteria isolated from lesions, assumed as the causative organisms (7 strains of S. aureus, 3 of P. aeruginosa, 1 of H. influenzae, 1 of K. pneumoniae, 1 of E. coli, and 1 of S. marcescens). Out of these strains, 10 were eradicated, and 1 (P. aeruginosa) decreased, but 2 strains (both S. aureus) persisted. (One strain of S. aureus was not examined.) No adverse effect suspected to be related to the drug was observed either in subjective symptom or in objective findings.

Ceftazidime ( CAZ ), a new injectable cephem antibiotic, was used for treatment of infections in children, and the following results were obtained. After an intravenous injection of CAZ at a dose of 20 mg/kg, the mean blood levels in 2 patients were 41.5 micrograms/ml at 30 minutes, 18.1 micrograms/ml at 2 hours and 2.55 micrograms/ml at 6 hours, with the half-life (T 1/2) of 1.37 hours. In a 22-day-old baby with meningitis given CAZ intravenously at a dose of 43.5 mg/kg, the blood levels were 100 micrograms/ml at 30 minutes, 68 micrograms/ml at 2 hours and 25 micrograms/ml at 6 hours, with the half-life (T 1/2) of 2.96 hours. After intravenous administration of CAZ in doses ranging from 35.7 to 50 mg/kg, CSF concentrations ranged from N.D. to 6.3 micrograms/ml in 3 patients with purulent meningitis, although 19 micrograms/ml at 1 hour and 13 micrograms/ml at 2 hours in 1 patient after intravenous administration of 46.7 mg/kg. , pyothorax and purulent meningitis, each 3 patients with pyelonephritis and enteritis) were treated with CAZ intravenously, at the daily doses of 178.2 mg/kg and 200 mg/kg in 4 divided doses in patients with meningitis and 44.1 to 103.4 mg/kg in 3 divided doses in patients with other infections (two of them were given by intravenous drip infusion for 30 minutes). The clinical responses were excellent or good in all the patients except for 1 case of Salmonella enteritis (poor) and 1 case of Campylobacter enteritis (poor). The efficacy rate was 88.2%. It was noteworthy that the clinical response was excellent in 1 case of
septicemia
with P. aeruginosa with leukemic stage of malignant lymphoma and in 2 cases of purulent meningitis. As side effects, fever, eruption, leukocytopenia, elevation in GOT and positive CRP considered to be allergic, were observed on day 16 of administration in 1 case of pyothorax. These symptoms disappeared by discontinuance of administration. The chemistry, microbiology, pharmacokinetics, therapeutic use, adverse effects, and dosage of amoxicillin-potassium clavulanate, a beta-lactamase-resistant antibiotic combination, are reviewed. Clavulanic acid is a "suicide" inhibitor of bacterial beta-lactamase enzymes and has been effective in preventing destruction of penicillins by these enzymes. Clavulanic acid alone has weak antibacterial activity against most organisms. After oral administration, clavulanic acid is rapidly absorbed; amoxicillin appears to increase its absorption. Absorption of amoxicillin-clavulanic acid is not affected by food. Amoxicillin-clavulanic acid is effective in treating both acute uncomplicated and complicated urinary-tract infections and exacerbations of chronic bronchitis caused by amoxicillin-resistant organisms in adults. It appears to be comparable in efficacy to cefaclor for treating uncomplicated urinary-tract infections in adults and children,
acute bronchitis
and bronchopneumonia, and acute sinusitis, otitis media, and skin and soft-tissue infections in children. Other infections for which the combination has been effective include cellulitis and intra-abdominal and pelvic

Amoxicillin-clavulanic acid has also successfully cured urethritis in men caused by penicillinase-producing Neisseria gonorrhoeae and is superior to amoxicillin alone for beta-lactamase-positive Haemophilus ducreyi infections (chancroid). Diarrhea or loose stools is the most common side effect seen with amoxicillin-clavulanic acid; nausea, vomiting, and skin rash may also occur. Clinical studies on cefoperazone (CPZ), a new cephalosporin, were carried out at our department. Seventeen children wih the following bacterial infections were treated with CPZ; pneumonia (7),
bronchitis
(6), tonsillitis (1),
sepsis
(2) and purulent meningitis (1). The dosage was 56 approximately 182 mg/kg/day, divided into 4 doses, and given intravenous injection. The duration of administration was from 4 to 15 days. Clinical results were excellent in 3 cases, good in 9 cases, moderate in 3 cases, poor in 1 case and uncertain in 1 case. The overall efficacy rate was 75.0%. No side effects were observed.

A new semisynthetic 1-oxa-beta-lactam derivative, 6059-S, was evaluated for its safety and efficacy in children. Twenty-five patients were treated with 10 to 274 mg/kg per day of 6059-S by intravenous administrations. The diagnosis of the patients were acute pharyngitis (2),
acute bronchitis
(2), pneumonia (4), pertussis (4), acute enterocolitis (2), recurrent urinary tract infection (2), suspected
septicemia
(3), and acute purulent meningitis (1); and the remaining 5 patients were considered to have nonbacterial infections. The pathogens recovered were Streptococcus pneumoniae (1), Haemophilus influenzae (4), Haemophilus parainfluenzae (1), Enterobacter cloacae (1), Enterobacter aerogenes (1), Proteus morganii (1), Psuedomonas aeruginosa (2) and Salmonella typhimurium (1). All the patients of bacterial infections were cured after the 6059-S therapy. However, Pseudomonas aeruginosa and Salmonella typhimurium were not eradicated after the 6059-S therapy, and the rate of bacterial disappearance was 75%. Diarrhea (3), precordial pain (2, only in cases with high-dose therapy), transient elevation of GOT and GPT (2), and transient eosinophilia (2) were found to be associated with the 6059-S therapy. However, no severe adverse reactions were encountered. Half life of the serum 6059-S level was 1.34 +/- 0.16 hours. CSF concentrations in a case with Haemophilus influenzae meningitis ranged 4.0 to 9.7 mcg/ml after an intravenous injection of 34.3 to 75 mg/kg of 6059-S. From the present study, 6059-S appears to be a safe and effective antibiotic when used in children with susceptible bacterial infections. It remains to be further determined whether 6059-S is superior to ABPC in the treatment of Haemophilus influenzae meningitis.

Pharmacokinetic and clinical studies on cefotetan (CTT), a new cephamycin antibiotic, were carried out and the following results were obtained. Pharmacokinetic study Two patients, 7 years and 10 months of age (22 kg of body weight) and 9 years of age (28 kg of body weight), were administered 20 mg/kg of CTT by 30 minutes intravenous drip infusion. Serum levels of CTT were 148 micrograms/ml and 92 micrograms/ml immediately after the end of drip, 118 micrograms/ml and 63 micrograms/ml at 1 hour after the drip infusion. 76 micrograms/ml and 39 micrograms/ml at 2 hours after, 34 micrograms/ml and 18.2 micrograms/ml at 4 hours after and 18 micrograms/ml and 8.2 micrograms/ml at 6 hours after. Serum half-lives calculated were 1.92 hours and 1.78 hours respectively. Clinical study CTT was administered to a total of 14 patients, 3 with pneumonia, 2 with acute pyelonephritis, 2 with acute enteritis, each one with acute tonsillitis,
acute bronchitis
, acute bronchiolitis,
sepsis
, acute lymphadenitis, stomatitis and measles. Because that stomatitis and measles, however, were not indications of CTT, 2 cases with those diseases were excluded. CTT was administered at daily dose of 40 to 73 mg/kg in 2 to 4 portions for 3 to 5.5 days by intravenous drip infusion. Marked response was seen in 2 cases, moderate response in 9 and no response in 1, thus effectiveness rate was 91.7%. Neither side effects nor abnormal clinical laboratory findings were observed.

Cefotetan (CTT), a new cephamycin antibiotic having a long serum half-life (2.93 +/- 0.78 hours), was evaluated for its safety and efficacy in children. Twenty-four patients were treated with a daily dose of 30 to 100 mg/kg of CTT by intravenous administrations mostly in 2 divided doses. The diagnoses of the effective patients were
acute bronchitis
(5), pneumonia (4), acute urinary tract infections (4), acute enterocolitis (2), presumed
septicemia
(1), and phlegmon (1); and the effectiveness was 77.3%. The pathogens recovered from these patients were S. pneumoniae (1), H. influenzae (3), S. marcescens (1), E. coli (2), and K. oxytoca (1). CTT was not effective in staphylococcal pneumonia and empyema (each 1 case), in Pseudomonas pneumonia (2), and in a case of brain abscess and mastoiditis of unknown etiology. Diarrhea (2), and transient elevations of the serum GOT, GPT, and LDH (1) were associated with the CTT therapy, but no severe adverse reaction was encountered. The CSF level of CTT seemed to be lower among several new cephalosporins. From the present study, CTT appears to be a safe and effective antibiotic when used in children with susceptible bacterial infections. A twice-a-day schedule was recommended from its long serum half-life.

Thirty-six febrile patients were administered cefpiramide (CPM) of 20 approximately 75 mg/kg/day for 3 approximately 11 days, and the clinical and side effects were evaluated. Among children with bacterial infections, including pneumonia, urinary tract infection,
sepsis
, pharyngitis and
bronchitis
, the results were excellent in 9, good in 13, and fair in 3 patients. Out of 36 patients, adverse reactions were observed in 9 cases, i.e. vascular pain at one shot intravenous injection in 4, diarrhea in 2, eosinophilia in 2, and diarrhea and eosinophilia in 1 case. One shot intravenous administration of CPM of 10 mg/kg to 4 patients yielded mean serum level of 100 micrograms/ml at 15 minutes and mean serum half-life of 2.5 hours, and administration of 20 mg/kg to 3 patients yielded mean serum level of 200 micrograms/ml at 15 minutes and mean serum half-life of 3.5 hours. The half-life in 1 patient with slight liver lesion was 5.36 hours. The rates of urinary recovery within 8 approximately 12 hours were 7.2 to 28.0% in 5 patients, 45.1% in a patient with nephrotic syndrome, and 50.9% in a patient with slight liver lesion.

Ten inpatients at the Second Department of Internal Medicine, Mie University Hospital, developed infections in the course of treatment for hematopoietic disorders and were administered cefoxitin (CFX). Patients suffered from the following infections: pharyngitis, 2;
bronchitis
, 2; pneumonia, 2;
sepsis
, 2; bacteremia, 1; suspected cases of bacteremia, 2; and fever of unknown origin, 1. The number of infections totaled 12 as 1 patient with pharyngitis also developed
sepsis
and 1 patient with pneumonia developed bacteremia. Duration for the administration of CFX ranged between 5 and 18 days with a total dosage of between 30 and 108 g. Of the 10 patients treated with CFX, 9 were treated concomitantly with micronomicin (MCR), doxycycline (DOXY), or sulbenicillin (SBPC), some were treated concomitantly with only 1 of the drugs and some were treated concomitantly with 2 of the drugs. The following clinical results were obtained: Following treatment, 4 patients were considered "excellent", 5, "good", and 3, "poor". Clinical efficacy rate was 75%. Four strains of Gram-positive cocci (1 strain of S. aureus, 2 strains of S. epidermidis and 1 strain of Streptococcus sp.) and 3 strains of Gram-negative rods (2 strains of P. aeruginosa and 1 strain of E. cloacae) were found in the clinical specimens of the 10 patients. These results differed somewhat from reported data that Gram-negative rods such as E. coli, Klebsiella sp., Pseudomonas sp., Serratia sp., are dominant. No serious side effects requiring cessation of treatment were observed. Elevations in the levels of S-GOT, S-GPT, serum alkaline phosphatase, blood urea nitrogen, etc. were observed.(ABSTRACT TRUNCATED AT 250 WORDS)
