Trends in sex specific mortality from six conditions (hip fracture,
septicemia
, pneumonia, cancer,
heart disease
, and stroke) were examined for the period 1968 to 1980 to determine if recent increases in life expectancy at advanced ages were associated with significant shifts in the pattern of cause specific mortality at those ages. Changes in life table parameters were assessed both at birth and age 85 to determine if the relative significance of certain conditions had increased or decreased at advanced ages. In particular, I examined whether three conditions (hip fracture,
septicemia
, pneumonia), often viewed as being associated with mortality among debilitated persons, had increased in the proportion of deaths they affected at advanced ages and if there had been changes in the mean age at death for persons with these conditions. I also examined trends in the mortality rates of three conditions (
heart disease
, stroke, cancer) that often are viewed as primary disease processes with high mortality risks. Overall there seemed to be little evidence that mortality for conditions associated with a debilitation had increased markedly at later ages.

Although only about 2-3 percent of annual deaths are attributed to one of the four underlying causes linked to chronic alcohol use, research results suggest that the impact of alcohol is much greater. The recent issue of mortality multiple cause of death tapes from National Center for Health Statistics permit exploration of associations of chronic alcohol abuse with conditions coded as underlying cause of death and provide leads for case-finding. Data analysis is reported from certificates of resident deaths in the United States in 1978. There are 12 groups of underlying causes for which the percentage of pairings with chronic alcohol abuse ranges from 4-16 percent of the number of deaths from the underlying cause. Age, sex, and metropolitan status of residence are associated with a listing of chronic alcohol abuse among decedents of liver cancer, varicose veins, symptomatic
heart disease
,
septicemia
, and respiratory system disease. Planners concerned with secondary prevention can use these clues provided by logistic regression modelling as an aid in case-finding.

A neonate with total anomalous pulmonary venous connection draining via the right cardinal vein into the superior vena cava had a chest-X-ray unsuspicious for congenital
heart disease
, and initially was treated for neonatal
sepsis
. But as the clinical state impaired and cyanosis increased, sectorechocardiography revealed the right diagnosis. The child died soon after corrective surgery.

Non-otogenic cerebral abscess is becoming relatively more frequent compared with otogenic. It arises in a setting of
sepsis
, trauma, or congenital
heart disease
. The onset presents a meningo-encephalitic picture, often with two stages, followed by epilepsy and/or a hemiparesis. The best investigations are an EEG and, probably, an isotope scan. The angiogram may be misleading as 40% may not have a midline shift. 'Acute localized non-suppurative encephalitis' seems to be more frequent than in the past. Antibiotics and aspiration appear to be the best treatment.

Maternal deaths were reviewed at the American University of Beirut Medical Center during an 11 year period, 1971-82. There were 35,058 livebirths and 45 deaths, making a maternal mortality rate of 128/100,000 livebirths. Hemorrhage,
sepsis
, and toxemia were the main direct causes of obstetric death. The most important indirect causes were cerebrovascular accidents and
heart disease
. In this, review, an analytic discussion of the direct and indirect causes of maternal death in Lebanon are presented and preventive measures are discussed.

Renal venous thrombosis, a clotting process that originates in the venous radicles and progresses into the main renal vein and vena cava, predominantly affects newborn infants. It may be manifest in one or both kidneys and follow maternal diabetes, diarrhea and dehydration, congenital
heart disease
, acute blood loss,
sepsis
, asphyxia, and shock. The most common signs include gross hematuria, enlarged palpable kidneys, and thrombocytopenia. Evaluation should include ultrasonography of the kidneys (demonstrating renal enlargement with disruption of the normal echo pattern), computed tomography, and renal isotope scanning. The initial treatment is supportive. Surgical intervention is not indicated in the acute phase except in the rare instance of bilateral disease. Anticoagulant therapy is still controversial. Late sequelae include impairment of renal function, shrunken hypoplastic kidney, arterial hypertension, and tubular defects.

The study has revealed that active forms of infectious endocarditis constitute 30-50% of all septic diseases in hospitals. It is the normal cusps of the valvular apparatus of the left cardiac portion that are predominantly damaged, deformed or destroyed. Endocardial involvement of the right portion of the heart and the pulmonary artery occurs very rarely. Infectious patients develop
sepsis
more frequently than patients with non-infectious diseases but it takes the form of acute infectious endocarditis less commonly. Currently the clinical course of acute infectious endocarditis is characterized by predominantly ulcerous thrombotic damage of the normal valves with the formation of
heart disease
, the development of congestive insufficiency of the circulation and thromboembolic complications and the lethal outcome in the first weeks of hospitalization in 95.6% of patients with acute endocarditis in infectious and 56.6% in non-infectious hospitals.

Endocarditis and unilateral endophthalmitis due to Actinobacillus actinomycetemcomitans
heart disease
. The ocular infection was notable for its localized presentation and slow evolution. Treatment with systemic, subconjunctival, and topical gentamicin sulfate and ampicillin sodium achieved 20/20 acuity with a residual chorioretinal scar in the nasal periphery. Intravitreal injection of the organism into a rabbit confirmed its minimal pathogenicity within the eye. This organism must now be considered in patients with differential diagnosis of endogenous endophthalmitis complicating endocarditis and
septicemia
.

A case of bacterial endocarditis in a newborn without any congenital
heart disease
is reported. The clinical diagnosis was suspected on evolutive heart failure by mitral dysfunction with
sepsis
. T.M. mode echocardiography could detect only the valvular defects. Correlations between anatomic and echocardiographic findings allowed to discuss the limits of the method. Neonatal bacterial endocarditis is a rare event. Its diagnosis is difficult and its prognosis very poor. This affection must be prevented.

The clinical features of endocarditis of the aortic valve in 24 dogs were reviewed. This condition was found most commonly in large-breed, middle-aged male dogs. Evidence of antecedent infection or immunosuppression was usually not historically verified or found at necropsy. However, an association with congenital
heart disease
, especially discrete subaortic stenosis, was demonstrated. The most frequent clinical findings were systolic and diastolic murmurs and bounding arterial pulses, with or without signs of congestive heart failure. The most commonly isolated organisms were Corynebacterium sp, Erysipelothrix rhusiopathiae, and Streptococcus sp. In addition to antibiotic therapy, treatment for congestive heart failure often was required. Despite aggressive therapy, most affected dogs died as a result of congestive heart failure, arrhythmias, infarction,
sepsis
, or renal failure.
