To assess the possible role of amine neurotransmitters in human epilepsy, we measured metabolites of serotonin (5-hydroxyindoleacetic acid [5-HIAA]), dopamine (homovanillic acid [HVA]), and norepinephrine (3-methoxy-4-hydroxyphenylethylene glycol [MHPG]) in the lumbar cerebrospinal fluid (CSF) of patients with partial complex seizures and in neurologic controls. Untreated epileptic patients had lower concentrations of 5-HIAA and HVA in the lumbar CSF than the controls, but the differences were not statistically significant. Among epileptic patients receiving effective antiepileptic drug treatment, the HVA concentration was within the control range. Mean MHPG concentrations were similar in patients and controls. From the epileptic patients whose CSF was obtained at pneumoencephalography we obtained a second sample of CSF that was originally in the basal cisterns. No significant differences between treated and untreated patients were found for any of the three metabolites. The concentrations of HVA and 5-HIAA were higher in cisternal than in lumbar CSF, but there was no such gradient for MHPG.

5-Hydroxytryptophan (5-HTP) reduced the intensity of both audiogenic and pentylenetrazol seizures. p-Chlorophenylalanine reduced audiogenic seizure (AGS) susceptibility but failed to change the pentylenetetrazol seizure (PTS). Drugs blocking brain serotonin (5-HT) receptors suppressed AGS but caused no clear effects upon PTS. Pentylenetetraziol-induced shock increased brain 5-hydroxyindoleacetic acid (5hiaa) concentrations and decreased 5-HT levels. Single audiogenic shock decreased the acumulation of 5-HT and 5-HIAA in the brains of mice pretreated with 5-HTP. On the other hand PTS increased the accumulation of 5-HT and 5-HIAA in the brains of mice pretreated with 5-HTP. It is suggested that AGS decrease brain 5-HT turnover whilst PTS cause an opposite effect.

Clinical, electrical and biochemical studies in an eight years old boy with movement-induced seizures were reported. The attack was usually triggered by sudden initiation of movement, but rarely occurred without any apparent movement. Repeated jumping provoked attacks constantly, which was recorded cinematographically. No abnormality was found either in ictal or interictal EEGs. Haloperidol aggravated the condition, but 1-DOPA had no effect, while DPH (100 mg/day) controlled attacks perfectly with the serum DPH concentration of just 2.0 ug/dl. In overnight sleep analysis, sleep rhythms and characters of REM sleep were not differed significantly from the standard. After DPH therapy, stabilization of sleep in general was noticed; that is, total sleep time prolonged, number of sleep stages decreased and interrupting awakening disappeared. Probenecid loading test revealed that 5-HIAA was normal, HVA high, and large amount of octopamine was detected in CSF.

The investigations were carried out in 10 patients with epileptic seizures. The obtained results were compared with those in a control group of 20 healthy subjects. The 24-hour urinary excretion of 5-HIAA efore hydantoinal and on the 1st, 2dn and 3rd days of hydantoinal administration was determined by the method of Udenfried et al. as modified by McFarlane et al. It was found that healthy subjects excreted significantly more 5-HIAA before and after hydantoinal load. Administration of this drug increased significantly the excretion of this metabolite with urine, in healthy subjects but in epileptics it had no significant effect on 5-HIAA level in 24-hour urine.

Intracortical injections of iron ions have been shown to induce recurrent seizures and epileptic discharges in the EEG. (-)-Epigallocatechin (EGC) and (-)-epigallocatechin-3-O-gallate (EGCG), isolated from green tea leaves, have been reported to prevent or diminish the occurrence of epileptic discharges induced by iron ions, and to inhibit catechol-O-methyltransferase. Iron ions significantly increased DOPAC and HVA levels in the intrastriatal perfusate 140 and 180 minutes, respectively, after injection. EGC and EGCG inhibited the increases induced by iron ions. Furthermore, EGCG decreased the HVA level in the perfusate 200 minutes after injection whether or not iron ions were injected. Iron ions had no effect on the 5-HIAA level, and EGC and EGCG raised it. These results suggest that formation of an epileptic focus induced by iron ions might be accompanied by activation of dopaminergic neurons, and that EGC and EGCG inhibit that hyperactivity.

The effects of thiophosphoric acid alkaloid derivatives from Chelidonium majus L. (Ukrain, UKSR-222) on the central nervous system (CNS) of mice and rats was studied. Intraperitoneal (ip) administration of Ukrain in doses of 9.5 and 19 mg/kg for mice depressed spontaneous motor activity, decreased body temperature and potentiated the action of hexobarbital. Only in a dose of 19 mg/kg Ukrain produced analgesic action in the hot plate test. It had no protective effect against electroshock or pentetrazol-induced seizures. In rats, ip administration of Ukrain in dose of 14 and 28 mg/kg potentiated the action of amphetamine and apomorphine but had no effect on catalepsy induced by haloperidol. Ukrain used in dose 9.5, 14, 19 and 28 mg/kg antagonized the head twitches induced by 5-HTP and hyperthermia-induced by m-CPP. Biochemical studies indicated that Ukrain did not affect the NA and DA concentrations in the whole rats' brain and did not affect the 5-HT and 5-HIAA concentrations in the whole brain of rats. These findings demonstrate that the central action of Ukrain involves the stimulation of the dopaminergic system and the inhibition of the serotoninergic system.

Combination therapy of high-dose pyridoxal phosphate (PAL-P, 40-50 mg/kg/day) and low-dose ACTH beta 1-24-Z (tetracosactide acetate-Zn, Cortrosyn Z, 0.01 mg/kg/day) was instituted in 26 children suffering from West syndrome and related disorders--pretreated without success with high-dose PAL-P alone; 18 with West syndrome (14 with symptomatic and 4 with cryptogenic types), 2 with symptomatic Lennox-Gastaut syndrome, 5 with cerebral palsy with hypsarhythmia or diffuse slow spike-waves and one with myoclonic seizures (secondary generalized epilepsy). Clinical, electroencephalographic and neurochemical investigations were carried out. The results were summarized as follows. 1) Only one of 27 children with West syndrome and related disorders pretreated using high-dose PAL-P alone before ACTH showed a clinically excellent response. 2) Clinical seizures were completely suppressed in 19 of 21 children who initially had seizures (90%) after this combination therapy. 3) Twenty-one of the total 26 children (80%) had disappearance of hypsarhythmia or diffuse slow spike-waves in EEG after this therapy. 4) During PAL-P treatment alone transient increases in liver enzymes occurred in 37 percent. The brain shrinkage of CT and the significant rise in CSF NSE were seen in 95% and 78% after ACTH, respectively. 5) Twenty-three children have been followed for one to 29 months after tapering off of ACTH. No relapses were experienced in 11 of 18 who initially had seizures (61%) and 13 of 23 with hypsarhythmia or diffuse slow spike-waves (57%). 6) Postictal PRL elevations were suppressed during high-dose PAL-P. 7) No significant changes in the CSF levels of HVA and 5-HIAA were seen during this combination therapy. The CSF levels of HVA were significantly lower than the controls. 8) Daily ACTH therapy transiently suppressed the secretion of anterior pituitary hormones (GH, TSH, PRL, LH and FSH) and thyroid hormones (T3 free T3, T4 and free T4). It is recommended that the combination therapy of high-dose PAL-P and low-dose ACTH is a promising new method and should be tried in children with West syndrome and related disorders. The mechanism of action of this combination therapy remains obscure although some information has been obtained from our investigations.

The present study examined the effects of electroconvulsive shock (ECS) on interstitial concentrations of dopamine (DA), its metabolites DOPAC and HVA and the serotonin metabolite 5-HIAA in the striatum of freely moving rats using on-line microdialysis. DA increased sharply following a single ECS. Interstitial concentrations of DOPAC. HVA and 5-HIAA also increased significantly. The ECS-induced increase in DA varied as a function of days following implantation of the microdialysis probe, being 1300%, 305% and 300% of baseline 24, 48 and 72 h after surgery, respectively. In contrast, the response of the metabolites to ECS did not differ across days following surgery, being approximately 130%, 140% and 110% of baseline for DOPAC, HVA and 5-HIAA, respectively. Seizure activity induced by the convulsant agent flurothyl did not influence dialysate DA concentrations, suggesting that the ECS-induced DA release was related to the passage of current and not to the seizure activity. Interstitial concentrations of acetylcholine and choline in the striatum increased by approximately 20% and 140%, respectively, in response to a single ECS. The DA (but not the DOPAC or HVA) response to ECS was refractory to a second ECS delivered 2 h after the first. A second ECS delivered 24 h after the first produced the normal increase in DA. The ECS-induced increase in DA was attenuated following repeated ECS (eight treatments, one every second day). Tryptophan (Trp), 5-HTP, 5-HT, 5-HIAA, HVA, and MHPG in CSF and total Trp(T-Trp), free Trp(F-Trp) and serotonin in serum were determined in 80 children with epilepsy and also in a control group. It was found that Trp, 5-HT, 5-HIAA in CSF and F-Trp in blood decreased in children with epilepsy. But the decrease of F-Trp was not the main cause of decreased 5-HT metabolism, because the no positive correlations among the four substances were found. Each of them returned to normal levels after the treatment with phenytoin or valproate. 5-HT concentration was negatively correlated with the frequency of the epilepsy episodes. 5-HIAA and HVA levels were relatively higher in the epileptics with brain damage as with compared with those who had no brain damage. The MHPG level was higher in the patients simple partial seizures. Complex partial epileptics and those patients receiving antiepileptic drugs had a lower serum T-Trp level. T-Trp was negatively correlated with the serum valproate concentration. Both T-Trp and F-Trp levels decreased in the patients treated with phenytoin.
