Iopamidol, a water-soluble contrast medium, has been rarely associated with seizures. We describe 3 patients (from a series of 785) who had generalized tonic-clonic seizures after iopamidol myelography. Two of the patients underwent lumbar and one cervical myelography. There was a history of seizures in 2 patients, and the dose of iopamidol used in the patients who convulsed was high, ranging from 3,000 to 4,500 mg iodine. Despite the reported low incidence of complications with iopamidol, seizures may occur, especially in patients with previous seizures and also in those receiving higher doses of iopamidol.

Two nonionic contrast media, iopamidol and metrizamide (Amipaque), were used for cervical myelography (C1-C2 puncture) in 95 consecutive patients. Both contrast media gave excellent radiographic results. Headache and vagal symptoms were similar in both groups, whereas metrizamide produced more electroencephalographic changes and epileptic seizures. Meningeal irritation occurred in both groups and was severe in three cases. Cerebrospinal fluid showed protein and cellular changes of inflammatory type in both groups. Iopamidol is considered to be the more suitable contrast medium for cervical myelography despite its slight neurotoxicity.

A 32-year-old man presented to the emergency department (ED) with seizures 6 hours after a lumbar myelogram with iopamidol (Isovue-M 200, Squibb, Princeton, NJ). Seizures are a rare complication after myelography with the current nonionic contrast agents and have not been previously reported in the emergency medicine literature. Emergency physicians should be aware of this potential complication because outpatient myelograms are frequently performed, and these patients may present to the ED after seizures.

To summarize the points discussed in this article, Small animal myelography can be performed competently and safely without specialized equipment. The newer myelographic contrast media (iohexol, iopamidol) have less neurotoxic potential than those media previously used. Myelography is necessary for the diagnosis of many neurologic disorders in small animals. Metrizamide is no longer recommended for myelography in dogs and cats. Iohexol (Isovue) in concentration of 180 mg I/mL to 300 mg I/mL is recommended for small animal myelography. Some patients with serious spinal cord disease can exhibit normal myelograms. Recognizing abnormal myelographic patterns is the hallmark of myelographic interpretation. Anticonvulsive drugs (diazepam, pentobarbital, and phenobarbital) should be available for therapy of postmyelographic seizures. Epidurography is an effective diagnostic technique in the evaluation of canine lumbosacral compressive disease. Abnormalities identified with epidurography correlate well with surgically confirmed lesions.

Iopamidol, a water-soluble contrast agent, has been rarely associated with seizures. We describe a case of generalized tonic-clonic seizure after cervical myelography with iopamidol in a previously healthy young man. In patients presenting with seizures, a history of recent myelography should be considered as an etiology. Iopamidol myelography may be associated with a risk of seizures. Clinicians need to be aware of this complication and inform their patients about such risk.
