As a follow-up to pilot observations that six manic patients who failed to respond to unilateral electroconvulsive therapy (ECT) recovered rapidly when switched to bilateral treatment, a retrospective study was conducted. Twenty-five patients who responded after switchover from unilateral to bilateral ECT, 25 age- and sex-matched controls, and 25 concurrent controls who responded to right unilateral ECT alone were evaluated. Demographic variables and DSM-III diagnosis did not discriminate between the groups, nor were they different in terms of electroencephalographic (EEG) findings, neuropsychological test results, numbers of ECT, and duration of
seizure
discharges. Standard assessments of psychopathology performed by independent psychiatrists showed no differences in ratings of psychosis or depressive phenomena. However, scales assessing manic symptoms showed highly significant differences with many more features of unrestrained behavior,
elevated mood
, hurried speech, and other typical features of mania in the patients who were switched from unilateral to bilateral ECT. Although there were no differences in prescribed drugs, the use of prn medications for sleep was greater in the experimental-switched patients than in controls. Patients who responded to unilateral ECT alone exhibited virtually no manic features, whereas those who demonstrated these characteristics failed to respond to unilateral ECT but benefited when switched to bilateral treatment.

Three patients were evaluated for effects of formaldehyde on central nervous system function. Three patients had used formalin, formaldehyde with or without phenol, to fix whole animals for 14-30 y, and a fourth patient was covered several times by formaldehyde and phenol rainout from manufacturing spills. elevated mood
state scores (82 to 162) and elevated symptom frequency scores (111 to 138), compared with referent subjects. There was excessive fatigue, somnolence, headache, difficulty remembering, irritability, and instability of mood. Compared with referents, choice reaction time was prolonged in four of four (4/4) subjects, blink latency was delayed in 2/2, balance was abnormal in 3/4, and visual fields were constricted in 2/3. Cognitive functions, measured by Culture Fair, block design, and digit symbol tests, were impaired in all. Delayed verbal recall and visual reproduction were impaired in 3/4. Perceptual motor speed on slotted pegboard and trail making A and B tests was reduced in 4/4. Errors on fingertip number writing were abnormal in all. Long-term memory was decreased in only one. Extensive use of formaldehyde at work or repeated airborne exposure to formaldehyde and phenol appears to have impaired central nervous system function.

We report two adults who received gabapentin (GBP) and subsequently developed behavioural side effects. Indications for GBP treatment were newly diagnosed epilepsy in one and painful paraesthesiae in the other. Both had no past history of psychiatric or behavioural disorder. Abnormal behaviour consisted of
elevated mood
, euphoria, and increased energy in both patients, and pressure of speech and decreased need for sleep in one of them. These symptoms were transient and fully reversible. One patient had to discontinue GBP. Behavioural changes were not related to
seizure
activity. They should be recognized as a possible side effect of GBP treatment in adults.

We report on a 33-year-old woman who had a paraneoplastic syndrome with anti-N-methyl-d-aspartate receptor encephalitis, associated with ovarian teratomas. She presented with acute onset
seizures
, grandiosity,
elevated mood
, disorganized thoughts, and paranoia. Cerebral spinal fluid analysis revealed anti-N-methyl-d-aspartate receptor antibodies. A PET/CT was requested, which identified a possible fat and calcification containing right adnexal lesion on the noncontrast CT portion, which was suspicious for a teratoma. Subsequent ultrasound revealed bilateral masses consistent with teratomas. She underwent laparoscopic removal of these masses, which were both confirmed to be mature cystic teratomas on pathology. After surgery, her symptoms progressively improved.
