Antiepileptic drug selection is based on efficacy for specific

, the drug of choice is valproate. Secondary generalized epilepsies with tonic, atonic, and other

, any of the other major antiepileptic drugs can be effective. If valproate cannot be used, carbamazepine, phenobarbital, phenytoin, or primidone is effective, but ethosuximide or a benzodiazepine needs to be added to control associated absence or myoclonic

. The drugs of first choice for partial epilepsies with partial and secondarily tonic-clonic
seizures
are carbamazepine and phenytoin. Increasing evidence suggests that valproate may be a third alternative. Phenobarbital and primidone are second choice selections because of side effects. A combination of two of these five major antiepileptic drugs may be necessary for inadequately controlled patients.

syndromes such as neonatal and infantile epilepsies, febrile epilepsy, and alcoholic epilepsy require specific drug treatment. For all these

are CBZ and PHT. Increasing evidence suggests that VPA is a good alternative when CBZ and PHT fail. PB and PRM are second-choice selections because of adverse effects. A combination of two of the five standard AEDs may be necessary to treat intractable

, but no studies have been done to indicate an optimal combination.
Other epilepsy
syndromes such as neonatal and infantile epilepsies, febrile epilepsy, alcoholic epilepsy, and status epilepticus require specific AED treatment. Ultimately, AED selection must be individualized. No "drug of choice" can be named for all patients. The expected efficacy for the
seizure
type, the importance of the expected adverse effects, the pharmacokinetics, and the cost of the AEDs all must be weighed and discussed with the patient before a choice is made. A number of new AEDs with unique mechanisms of action, pharmacokinetic properties, and fewer adverse effects hold important promise of improved epilepsy treatment.

Symptomatic epilepsy secondary to hereditary metabolic or degenerative disorders, is usually associated to neurological deterioration. Though epilepsy by itself does not induce neurological deterioration, we should remind that some epileptics encephalopathies, such as the West or Lennox-Gastaut syndromes, do actually induce limited neurological deterioration. Furthermore, in some forms of complex partial epilepsy, motor problems and behavior disorders can be observed, specially in adolescents with temporary lobe epilepsy.
Other forms of epilepsy
, such as the atypical benign partial epilepsy or the Landau-Kleffner syndrome, can present a certain degree of cognitive deterioration in the evolution, although they can recover later lost functions, totally or partially. The evolution of some refractory epilepsy, as patients are submitted to a multiple treatments, can make us suspect a degenerative disease. In some cases, the diagnosis of the hereditary metabolic and heredodegeneratives diseases can be made by the characteristics of the

but in most cases the diagnosis will be established by the symptoms of the basic disease and the lab data.

Having a chronic condition like epilepsy imposes a serious burden for the sufferer. This is reflected in a higher level of psychological complaints. The way in which one copes with epilepsy is an important determinant in this level of complaints and in the self-perceived severity of the
seizures
. In this paper the relation between coping styles and self-perceived
seizure
severity as well as psychological complaints, is studied. A group of epilepsy patients was asked to fill out a number of questionnaires on coping styles, self-perceived
seizure
severity and psychological complaints. The patients with epilepsy were seen to have coping styles different from the general population: in general they tended to use less active strategies in coping with their problems. A less active coping style was found to be related to a higher self-perceived
seizure
severity.
Other epilepsy
-related variables, like
seizure
frequency, showed no relation with self-perceived
seizure
severity. This could mean that stimulating patients to use active coping styles results in a decrease in self-perceived
seizure
severity.
