We report a 3-year-old boy with unilateral
proptosis
, painful swelling of the right thigh and aphasia. He had radiographic evidence of
scurvy
in the limbs and bilateral frontal extradural hematomas with a mass lesion in the left orbit on MRI. He was treated with vitamin C and on follow-up 8 weeks later had recovered with no evidence of the orbital mass on clinical or radiological study.
Scurvy
manifesting as
proptosis
and extradural hematoma is rare.

Scurvy
is one of the oldest diseases known to mankind, caused by vitamin C deficiency. Nowadays it exists primarily in certain vulnerable populations such as patients with neurodevelopmental disabilities.
Proptosis
due to orbital haemorrhage is a rare complication of
scurvy
. Here we describe a case of diaplegic cerebral palsy presented with sudden onset of bilateral
proptosis
and swollen and tender knee joints. Subsequent computed tomography of the orbit revealed hematoma in the superior aspect of both orbits in the extraconal position. X-ray of the knee joints revealed bony changes consistent with
scurvy
. After 6 weeks of treatment with vitamin C orally, the
proptosis
and the bony changes returned to normal. Although
scurvy
is a rare disease in the general pediatric population, it still exists in certain vulnerable populations like neurologically disabled ones, and this uncommon cause for
proptosis
should be considered in such children.

Scurvy
usually presents with tender and painful limbs, swelling of joints, gum bleeding, poor wound healing, and muscle weakness. Here, we report a case of 5-year-old child with global developmental delay who presented with soft swelling of the head over scalp and protrusion of the left eye with extremely irritability. Neuroimaging was suggestive of diffuse extensive soft-tissue swelling involving the entire scalp with large necrotic collections with mild
proptosis
of the left orbit. It is not mentioned elsewhere, so we are giving name to this magnetic resonance imaging finding as "headband" sign or "turban" sign.

Spontaneous unilateral orbital haematoma in children is not common and very rarely caused by
scurvy
.
Scurvy
is a clinical syndrome with a spectrum of clinical manifestations due to severe prolonged vitamin C deficiency leading to impairment of collagen synthesis over skin, bone, teeth and blood vessels. This paper presents a unique case of a 7- year-old girl with learning difficulty who presented with spontaneous right
proptosis
due to
scurvy
. Imaging studies suggestive of intra- orbital extraconal haemorrhage. The child was treated with a higher than recommended dose of vitamin C initially in emergency situation. She responded well and discharged without complication. Spontaneous orbital haematoma due to
scurvy
is very rare with less than 10 cases published in literature. The present case should raise the awareness regarding this forgotten disease and importance of balance nutrition amongst children.
