Since results of conventional FFT-power-analysis are reference-dependent, only unambiguous neurophysiological data should be used for a functional physiological interpretation of EEG-data. FFT-approximation with successive center-of-gravity-dipole calculation gives unambiguous EEG-data with regard to recording reference. In the present investigation 22 medicated schizophrenic patients were compared with 22 healthy age- and sex-matched controls with regard to spontaneous resting EEG. More anterior and superficial equivalent-dipoles were found in the beta-bands for schizophrenic patients compared to healthy control subjects. There was a tendency of increased beta-activity in schizophrenic subjects. With more severe schizophrenic symptoms (higher BPRS-score), the more anterior was the equivalent-dipole localization in the beta 1-band, and the deeper in the theta-band. The different locations of the beta-band dipoles in schizophrenic patients suggest that different neuronal populations generate beta-activity in schizophrenia compared to healthy controls. FFT-approximation allows a substantial and meaningful data reduction in multichannel recordings and will hopefully help in understanding pathological brain functions in schizophrenia.

This study assessed whether therapist adherence to the family focused treatment model for patients with bipolar disorder and their relatives was associated with patient outcomes at one year after treatment entry. A total of 78 videotaped sessions of FFT consisting of 26 families with a member with bipolar disorder (3 sessions/family) were rated on fidelity using the Therapist Competence/Adherence Scale (TCAS; see Endnote 1, p. 130). Patients' outcomes (relapse status) were assessed using the Brief Psychiatric Rating Scale (BPRS) and selected items from the Schedule of Affective Disorders and the Schizophrenia-Change (SADS-C) scale (measured at 3-month intervals for 12 months). Contrary to expectations, therapist fidelity was not related to overall outcome as assessed by the BPRS and the SADS-C. Among patients who did relapse, higher levels of cooperation among therapists predicted a later date for relapse than did lower levels of cooperation. Surprisingly, and in opposition to the study's hypotheses, patients who were hospitalized because of relapses had therapists who were rated as more competent in their ability to conduct the problem-solving module of FFT. Study implications are discussed.

The pathophysiology of schizophrenia includes abnormalities in subcortical-cortical transfer of information that can be studied using REM sleep EEG spectral analysis, a measure that reflects spontaneous and endogenous thalamocortical activity. We recorded 10 patients with first-episode schizophrenia and 30 healthy controls for two consecutive nights in a sleep laboratory, using a 10-electrode EEG montage. Sixty seconds of REM sleep EEG without artifact were analyzed using FFT spectral analysis. Absolute and relative spectral amplitudes of five frequency bands (delta, theta, alpha, beta1 and beta2) were extracted and compared between the two groups. Frequency bands with significant differences were correlated with BPRS positive and negative symptoms scores. Patients with schizophrenia showed lower relative alpha and higher relative beta2 spectral amplitudes compared to healthy controls over the averaged total scalp. Analysis using cortical regions showed lower relative alpha over frontal, central and temporal regions and higher relative beta2 over the occipital region. Absolute spectral amplitude was not different between groups for any given EEG band. However, absolute alpha activity correlated negatively with BPRS positive symptoms scores and correlated positively with negative symptoms scores. Since similar results have been reported following EEG spectral analysis during the waking state, we conclude that abnormalities of subcortical-cortical transfer of information in schizophrenia could be generated by mechanisms common to REM sleep and waking.
