282 pedigrees in the
MRC
Cytogenetics Registry, Edinburgh, with familial autosomal anomalies were examined for the presence of associated mental illness. In one large pedigree there were 23 cases of mental and/or behavioural disorders meeting Research Diagnostic Criteria. 34 of the 77 family members available for cytogenetic analysis carried a balanced translocation t(1:11) (q43,q21). Psychiatric diagnoses had been recorded for 16 of the 34 members with the translocation compared with only 5 of the 43 without it. The lod scores (against chance linkage of the translocation with mental illness) were greatest when the mental disorders in the phenotype were restricted to
schizophrenia
, schizoaffective disorder, recurrent major depression, and adolescent conduct and emotional disorders. Although the mental illness in this family may not be typical of that in the general population, the findings suggest that the q21-22 region of chromosome 11 may be a promising area to examine for genes predisposing to major mental illness.

302 mentally retarded adults, sampled by epidemiological criteria, were examined with regard to handicaps, behaviour, skills and psychopathology by use of the
MRC
HBS-schedule and a list of psychiatric items. Based on research criteria, a computerized psychiatric diagnosis was made on a hierarchial scale. A psychiatric disorder was diagnosed in 85 (27.1%), which is a smaller prevalence rate than found in other studies. Next to behaviour disorder (10.9%), psychosis of uncertain type (5%) was the most common disorder. Dementia and early childhood autism were found equally often (3.6% each). Neurosis was seldom (2%), while
schizophrenia
(1.3%) and affective disorder (1.7%) occurred at about the same rates as found in similar investigations. No cases of alcohol or drug abuse were found.

In a previous large epidemiological survey of patients with strictly defined
schizophrenia
in the London borough of Camden, we extracted four behavioural syndromes (Social withdrawal, Thought disturbance, Anti-social behaviour and Depressed behaviour) by factor analysis of
MRC
Social Behaviour Schedule (SBS) data. These syndromes had significant differential relationships to symptoms assessed using the Manchester Scale (MS), symptom-derived syndromes, and social functioning variables. A second inner-London epidemiological survey of
schizophrenia
in South Westminster using identical methodology found the same four behavioural syndromes with identical core component items. The same four behavioural syndromes were extracted, whether applying strict Feighner diagnostic criteria (n=112) or broader DSM-III-R criteria (n=198). The four syndromes extracted from the Feighner positive sample showed relationships to symptoms and social functioning variables similar to those found in the original Camden study. However, the symptom-derived factors were not the same and did not conform to the three recognised symptom-based syndromes of
schizophrenia
. This successful replication suggests that assessment of the four behavioural syndromes of
schizophrenia
offers a different perspective on disability and a potentially relevant measure in clinical practice, clinical trials and studies of the neuropsychology and pathophysiology of
schizophrenia
.

Neurochemical transmission is a fundamental element of brain organisation that has been relatively unexplored in the living human brain. Continuing advances in radionuclide imaging, particularly positron emission tomography (PET) and single photon emission tomography (SPET), mean that elements of neurochemical transmission can now be directly measured in vivo. With these techniques convincing abnormalities of monoaminergic neurotransmitter systems have been revealed in illnesses such as Parkinson's disease and
schizophrenia
. Furthermore, mechanisms of drug action and treatment responses can be monitored in vivo. This brief review describes some of our recent attempts to image the neurochemical brain in health and disease at the
MRC
Cyclotron Unit, Hammersmith Hospital, London.

This article is based on an evaluation carried out by the DGPPN in 2011 surveying 50 German university and non-university institutions about the scientific research output focussing on psychiatric and psychotherapeutic research. The results of the survey show that on average there are 1 to 2 professors with 12 assistants scientifically active per institution. According to self-disclosure an estimated 1.8 million Euros of external funds are raised each year.Compared to international standards regarding the illnesses major depression,
schizophrenia
and alcohol abuse, Germany ranks second and third place behind the USA and the UK or Canada in terms of publication output. In terms of authors and institutions, German scientists and universities rank in the upper third to upper tenth for these illnesses. These data show that psychiatric research in Germany is not only internationally competitive but outstanding in its excellent achievements in these fields. Unfortunately, many funding programmes are limited to a 3-6 year period. In view of this fact and in order to achieve a long-term improvement in the translation of funding structures, as in the US (NIMH) or the UK (
MRC
) to benefit the mentally ill the implementation of a German centre for mental disorders is inevitable.
