The aim of this study was to determine if error negativity/error-related negativity (N(e)/ERN), error positivity (P(e)), correct response negativity (CRN) or correct response positivity (P(c)) amplitude are influenced by state changes in
schizophrenia
. Event-related potentials (ERPs) were recorded from nine schizophrenic patients while they performed a simple go/no-go task during the early stages of an acute episode and again following 6 weeks of treatment with antipsychotics. ERPs were also recorded from nine healthy participants while they performed the same task. Response-locked potentials were computed for errors of commission and for correct responses. Scores for reality distortion syndrome, psychomotor poverty syndrome and disorganization syndrome were determined for the schizophrenic participants before and after treatment using the
Signs and Symptoms
of Psychotic Illness (SSPI) Scale. N(e)/ERN amplitude was significantly reduced, compared with that in healthy participants, in the schizophrenic patients when acutely ill, and increased significantly following treatment. N(e)/ERN amplitude remained significantly larger in the healthy group than in the patients with
schizophrenia
after treatment. This study suggests that N(e)/ERN and CRN amplitude are modulated by clinical state in
schizophrenia
and provides further support to findings that decreased N(e)/ERN amplitude is a potentially useful trait marker for
schizophrenia
, while P(c) and P(e) amplitude are not abnormal.

Impaired insight in
schizophrenia
spectrum disorders has been linked to several psychopathologic features including positive symptoms, although not all dimensions of psychopathology have been studied and confounds from other symptoms have not been ruled out. In addition, the nature of the association between insight and specific positive symptoms, in particular delusions, remains unclear. The present investigation examined whether, in patients with
schizophrenia
spectrum disorders insight is associated with specific symptom dimensions including delusional severity. The factor structure was determined from scores of 151 patients rated on the
Signs and Symptoms
of Psychotic Illness scale. Associations of the
Signs and Symptoms
of Psychotic Illness insight item with the resulting components and delusions were assessed using regression-based methodology. Principal component analysis revealed 4 orthogonal symptom clusters. Correlational analyses demonstrated that only depression/anxiety and psychomotor excitation were significantly related to insight. Hierarchical regression indicated that delusions explained unique variance in insight over and above depression/anxiety and psychomotor excitation. These results suggest that depression/anxiety is associated with better insight and that psychomotor excitation and delusions are associated with poorer insight.

Stress predicts outcomes in adults with
schizophrenia
, but its effect in adolescents is unknown. We used exploratory data analysis to examine changes in symptoms and to correlate symptoms with stress at 0, 6, 30, and 54 weeks in 40 adolescents with
schizophrenia
. Symptoms were measured with the Early
Signs and Symptoms
Scale; stress was measured with the Hassles Scale. From baseline to 54 weeks, symptom severity decreased significantly. Symptom severity and stress correlated significantly at baseline, 30 weeks, and 54 weeks, after controlling for the number of hospitalizations and baseline symptom severity. These results implicate stress in
schizophrenia
symptoms in adolescents and justify examining stress management for these patients.
