A group of 25 hospitalised schizophrenics who had attempted
suicide
within the past year was compared with another group of 30 hospitalised schizophrenics who had not attempted
suicide
, using the PSE symptoms. The former had a significantly higher number of symptoms indicative of a depressive disorder. It is concluded that suicidal risk in
schizophrenia
is significantly raised when there are concomitant depressive symptoms.

The authors analyzed
suicide
cases recorded over 25 years (1957-1981). Male to female ratio among suicides was 70.90% to 29.10%, with subjects aged over 41 accounting for 68.30% of all cases. Strangulation was the method of realizing suicidal tendencies in 87.50% of cases. The number of suicides tended to increase in spring and summer months (with the peak observed in May) as compared to winter months, with the rise in May, June and July being statistically significant as against the corresponding rate in December, January and February. Among the suicides patients with
schizophrenia
comprised 34.20%, with chronic alcoholism 29.70%, depressions of various genesis 18.90%, vascular psychoses 5.00%, epilepsy 3.10%, psychopathy 3.90%, somatogenic and posttraumatic psychoses 2.70% and 2.50% respectively. Further efforts are needed to identify individuals with an elevated suicidal risk.

Seventy-seven patients of Puerto Rican origin who had attempted
suicide
and were admitted to the inpatient psychiatric unit of a general hospital were administered the Schedule for Affective Disorders and
Schizophrenia
(SADS), selected items of the Life Crisis Inventory, and the Beck Depression Inventory. Their scores on the SADS medical lethality and suicidal intent scales were in the minimal range, and their Beck inventory scores were in the mild range. Logistic regression analyses showed that alcohol and other substance abuse was a significant predictor of attempted
suicide
; place of birth (Puerto Rico versus the continental United States) was not.

A review of 46 subjects who suicided after having contact with a psychiatric hospital is presented. There were 33 men and 11 women, both with a mean age of 37 years. In comparison with a control group, those who suicided had a greater number of hospital admissions, a greater length of hospitalisation, were more often unemployed, had a history of more previous suicide attempts, more often received the diagnosis of
schizophrenia
or manic depressive illness (depressed phase), were more often overly depressed at their last contact, and were more often prescribed neuroleptic medications. Although these differences emerged,
suicide
is an infrequent event, and these factors lack specificity in prediction. The important association of psychiatric illness with subsequent
suicide
is noted.

It appears certain that the causes of self-destructive dermatoses are many and complex. The disorder spans diagnostic categories and varies from unconscious picking at the skin to severe self-destructive actions. Although not limited to any one diagnosis, skin disorders appear to be more prevalent in depression. This association may involve activation of the hypothalamic-pituitary-adrenal axis commonly found in depression. Two specific types of commonly occurring dermatoses-neurotic excoriations and dermatitis artefacta-are reviewed in this article. The major distinction of these disorders centers on whether the patient can admit to self-mutilation. Because of the difficulties in dermatitis artefacta with insight and body-image, it has been compared with anorexia nervosa. Often, dermatitis artefacta coexists with anorexia nervosa. In both disorders, neurotic excoriations and dermatitis artefacta, the personality style tends to be introverted with emotional immaturity. These patients have difficulty when they are under stress; the problem is compounded because of poor communication skills. Pharmacotherapy is of limited usefulness, and psychotherapy is often times hindered by strong resistance to exploring long-standing emotional issues. Once an alliance is established with the therapist, however, these issues may be examined. Prognosis is variable but does seem to directly correlate with the duration of the illness. Young individuals may experience alleviation of symptoms after one session of psychotherapy, whereas older patients may never have resolution. Dermatologic abuse involving psychosis has many presentations; one of the most common involves infestation. Organic causes must always be excluded as part of the differential diagnosis. In
schizophrenia
, this presentation has one of the highest incidences of
suicide

In a prospective investigation of 5,412 formerly hospitalized psychiatric patients, 68 committed
suicide
and 38 died from accidental causes. Both figures are significantly in excess of expectations based on a relevant control population. Those at significant risk included men and women of all ages, except those older than 69 years. Comparison of standardized mortality ratios suggests relatively greater risk for women and young persons. Risk for
suicide
was significant for patients of both sexes with acute
schizophrenia
,
schizophrenia
, affective disorders, and alcohol and other drug abuse, for men with neuroses, and for women with depressive neuroses. Risk for accidental death was greatest for those aged from 30 to 49 years and for those with personality disorders.

In a study of
suicide
in Singapore in 1980 there were 230 cases and the
suicide
rate for the general population was 9.5 per 100,000. The age-specific rates increased steeply after 50 years and there was a male preponderance especially in the age group 60 years and over. Comparing the three major ethnic groups in Singapore, the highest rate was seen in the Indians and Chinese, whereas the Malays had the lowest. Amongst the suicides there were 59 (25.7%) with mental illness, mainly
schizophrenia
, and they were of a younger age group, 20-39 years. The commonest method of
suicide
was jumping from high-rise flats.

Depressive symptoms and suicidal behavior in 64 adolescent psychiatric patients were assessed by a structured interview and the Schedule for Affective Disorders and
Schizophrenia
. The medical seriousness of suicidal behavior was associated with conscious intent to die and with the number of previous nonlethal suicide attempts. Suicidal behavior was associated with depressed mood, negative self-evaluation, anhedonia, insomnia, poor concentration, indecisiveness, lack of reactivity of mood, psychomotor disturbance, and alcohol and drug abuse. The results suggest that adolescents can be reliable reporters of their
suicide
potential and that clinicians need to be sensitive to symptoms of major depressive disorder in assessing potentially suicidal adolescents.

Eighty-two patients, who were treated for post partum illness between 1946 and 1971, were identified and followed up. Diagnostically, the sample comprised unipolar depression (52%), bipolar disorder (18%),
schizophrenia
(16%), abnormal personality with depression (8%), organic disorder (2%), and obsessional state with depression and paranoid disorder (1% each). The overall prognosis was good, except for
schizophrenia
, in which more than 50% of patients had chronic disability. Further childbirth intensified, and caused deterioration of, the underlying
schizophrenia
process. Following an initial illness in the puerperium, the probability of a recurrent affective illness was 43% for unipolar and 66% for bipolar disorder. The risk of developing another post partum illness varied from 1 in 3 to 1 in 5 pregnancies. Five percent of the sample ultimately committed
suicide
, and the probable incidence of infanticide was 4%.

A patient with gout and
schizophrenia
is described who during a schizophrenic paroxysm with paranoid-hypochondriac-hallucinatory syndrome attempted to commit
suicide
and took 200 tablets milurit (20 g). He developed the picture of acute intoxication with nausea, vomiting, profuse diarrhea, abdominal pain, flushing, temperature, collapse manifestations, hepatomegaly, direct hyperbilirubinemia, elevated transaminase, leukopenia, accelerated ESR. After reanimation and infusion therapy, the patient recovered within 4 days and 2 weeks later all blood indices reached the limits of the norm.
