Four instances are presented of self-amputation of the pinna of the external ear carried out by three right-handed, white males between January 1993 and May 1993. The common characteristics of these subjects--two men with personality disorder and one with schizophrenia--are discussed and compared with other examples of self-mutilation involving the face and ears, including that of van Gogh. A survey of Australian and New Zealand prisons was conducted to determine the frequency of this form of self-mutilation within the last five years, and yielded only one other case. Connections exist between the amputees supporting the notion that self-mutilation is "contagious"; the relevance of this to issues of management is considered.

The pattern of external ear abnormalities in patients with schizophrenia and bipolar disorder was studied. Sixty-seven male patients having schizophrenia (n=30) and bipolar disorder (n=37) were examined using a scale constructed for the study. Stepwise logistic regression analysis showed that 'prominent crux of helix' and 'ear lobe crease' could differentiate between schizophrenia and bipolar disorder. These external ear abnormalities need further characterization and consideration for possible inclusion in scales that assess minor physical anomalies.

Ear self-mutilation have been reported in schizophrenia, Mechanical damages to the auditory canal, like cleaning ear with hard and unsterile objects are predisposing factors of otomycosis. We present a case of repeated self-induced auricular trauma in schizophrenic patient. Aural swab were collected and examined by direct microscopy and culture, revealing Aspergillus flavus. A traumatized external ear canal skin can present favourable condition for fungal growth in the psychiatric population.
