A patient is described who presented with symptoms suggesting pericarditis and cardiac tamponade, and who was found at necropsy to have a rhabdomyosarcoma of the right ventricle. The presenting features of right ventricular tumours are discussed and some useful pointers to the correct diagnosis suggested.

We reported the first case of a very rare tumor in Thailand, a case of cardiac Rhabdomyosarcoma. The first clinical presentation was cardiac tamponade. Echocardiogram, CT scan of the heart, autopsy and microscopic findings proved its origin.

Cardiac tamponade resulting from malignant pericardial effusion is an uncommon initial presentation of various extracardiac malignancies. Only twenty-nine cases of extracellular malignancies with this initial presentation have been previously published. Lung carcinoma leads as the most common malignancy involved, followed by carcinoma of the stomach, pancreas, kidney and ovary, mediastinal rhabdomyosarcoma, malignant lymphoma and leukemia. This report describes a case of breast carcinoma in a 47-year-old woman who initially presented with cardiac tamponade. To the best of the authors' knowledge, no similar case has even been reported in the literature.

Primary, malignant, cardiac tumors are extremely rare in infants and children. Only a few cases of cardiac rhabdomyosarcoma have been reported in childhood. Echocardiography greatly increases the rate of correct diagnosis of cardiac tumors. There are some cases in the literature which were diagnosed by echocardiography and confirmed histopathologically. In this report, a 13-year-old girl with the clinical findings of cardiac tamponade and who was diagnosed as having a cardiac tumor by two-dimensional echocardiogram is presented An echodense, solid tumor with irregular borders extending especially toward the left ventricular cavity from the apex was demonstrated on echocardiography. The diagnosis was confirmed histopathologically on postmortem examination.

A seven-year-old male labrador retriever presented in right heart failure with weak femoral pulses, and pleural, abdominal and mild pericardial effusion. No diagnosis could be established initially. Two days later, the dog developed severe pericardial effusion causing cardiac tamponade. A tumour in the right ventricular wall was visualised on ultrasonographic examination. An exploratory thoracotomy was performed and biopsies of the mass submitted for histopathological examination. A diagnosis of rhabdomyosarcoma arising from the myocardium was established. Cardiac rhabdomyosarcoma has been reported in only two dogs. Neither report was associated with pericardial effusion.

The case report describes a 9-year-old boy who presented with an acute cerebrovascular accident and was found to have cardiac tamponade caused by cardiac rhabdomyosarcoma. Symptoms of rhabdomyosarcoma can be indolent and nonspecific, even with metastatic disease. Echocardiography and cardiac magnetic resonance imaging are explored as adjuncts to diagnosis. The radiologic features that helped with the diagnosis of this rare condition are described.
