One hundred one patients with advanced pediatric malignant solid tumors, refractory to conventional chemotherapy, were given Novantrone in a Phase II study. A dosage of 18 mg/m2 was administered as a short intravenous infusion every 3 weeks. One complete and 2 partial responses were observed among 26 patients treated for
rhabdomyosarcoma
; one of 22 patients with neuroblastoma developed a partial response. Nausea and vomiting were uncommon. Leukopenia and/or
granulocytopenia
developed in 90 of 98 evaluable entries. Two patients developed fatal congestive heart failure, which may have been related to the fact that these patients previously had received doxorubicin; 3 other patients developed evidence of changes in cardiac function, without congestive heart failure. Evidence of activity of this agent in patients who had previously received doxorubicin suggests that Novantrone should be evaluated in pediatric subjects with malignant solid tumors who have had no prior exposure to anthracyclines.

We have reviewed our experience using single-dose dactinomycin (Act D). Twenty-nine patients with Ewing's sarcoma or
rhabdomyosarcoma
received 114 courses of Act D (2 mg/m2) in combination with vincristine, cyclophosphamide, or DTIC with or without radiotherapy. Side effects included nausea and vomiting (100% of the courses), severe thrombocytopenia (16%),
granulocytopenia
(18%), and mucositis (25%). In addition, skin reactions were observed in 74% of the courses following concomitant radiotherapy. These results were compared with those following 152 courses of combination chemotherapy which contained doxorubicin in place of Act D and which were administered to these same patients. It is concluded that single-bolus Act D is no more toxic, is at least as effective, and is more conveniently administered than the traditional divided daily dose of Act D.

A case of rarely occurring
rhabdomyosarcoma
of the urinary bladder in a boy of 2 is presented. The period from the first clinical manifestations (pyuria, macrohematuria) to death was 4 months. Urological examinations were done at the period of manifest clinical signs of disuria. A tumour of the urinary bladder almost completely filling the lumen was found in the form of numerous papillary-polyplike formations, with uropassage disorders. On the materials of operation biopsy the histological conclusion of urinary bladder
rhabdomyosarcoma
was reached. The death occurred with urogenic infection and manifestations of drug
agranulocytosis
(cytostatic therapy). The tumour outgrew into the posterior wall, no metastases were found. Histologically the tumour was characterized by random arrangement of mono- and multinuclear polymorphic cells with oxiphilic cytoplasm. The granularity and fibrous pattern of the cytoplasm were discernible. Typically, an argirophilic framework was observed. By its histological structure the tumour belongs to the embryonal type.
