The present study aimed at accessing the natural history of untreated asymptomatic bacteriuria in diabetic patients observed for 14 years. Two groups of diabetic patients were examined. Group I comprised 53 patients with significant bacteriuria, without clinical symptoms of urinary tract infection. Group II consisted of 54 patients with sterile urine. All patients were clinically and bacteriologically examined every 3-6 months for 14 years. The results obtained in this study have proved that clinical symptoms of acute pyelonephritis occur with similar frequency in both groups (group I-6, group II-5 cases). No deterioration of kidney function was found. Arterial hypertension did not differ significantly in both groups at the beginning and end of the follow-up. The usefulness of antibacterial treatment of asymptomatic bacteriuria in diabetic patients is not adequately documented.

The authors presented the frequency of urinary tract infection (u.t.i) in pregnancy in own practice. Additional complications in pregnancy with asymptomatic bacteriuria and acute pyelonephritis were described. Postnatal period of neonates born from mothers with u.t.i. in pregnancy were analyzed. The authors defined the frequency of intrauterine hypotrophy and infectious complications in above newborns.

Urinary tract infections are accounted to serious complications, particularly in pregnancy complicated by diabetes. In this paper, cases of pregnancy have been analysed, affected by diabetes of type 1 and type 2, according to status of metabolic control and the type of urinary tract infection. In a group of 217 diabetic pregnant women, the incidence of urinary tract infections was 26.7%, 19.0% of them being recurrent. In the group with bad metabolic control, infections were statistically more frequent (17.4% vs. 37.3%, p = 0.001); bacteriuria without clinical demonstrations 10.4% vs. 19.6% (p > 0.05), pyelonephritis (7.0% vs. 17.7%, p = 0.001). The following types of pathogenic bacteria were found: E. coli--44.4%, Staphylococcous--28.9%, Enterococcocus--18.7%. A high frequency of Gram (+) bacteria was observed. A good metabolic control without chronic diabetic complications correlated with less frequent infections of the urinary tract.

Procalcitonin (PCT), a 116 amino acid prohormon of calcitonin, is a novel diagnostic marker of severe bacterial infection. The aim of the study was to evaluate the diagnostic usefulness of serum procalcitonin concentration in severely sick patients admitted to the department of internal diseases. Fifty one patients were included into the study and divided into two groups: group A--36 severely sick patients with different medical problems without signs of bacterial infection; group B--15 patients with severe bacterial infection (sepsis--7 patients, pneumonia--6, pyelonephritis--1, peritonitis--1). Twenty eight healthy controls were also included into the study. Serum PCT concentration measured by immunoluminometric assay was undetectable or low in control group (range 0.0-0.1 ng/ml) and group A (range 0.0-1.8 ng/ml). In group B (range 0.0-183 ng/ml) markedly elevated PCT levels were observed in all patients with sepsis, peritonitis and some patients with pneumonia. We conclude that high serum PCT level support the diagnosis of severe bacterial infection.

The case of lingual cancer in the 25-years old man treated in 2nd ENT Department Silesian Medical Academy in Zabrze was presented in this paper. The patient has been treated from 1989 due to chronic pyelonephritis finished with the renal insufficiency. In 1991 the renal graft was performed and in 1993 graftectomy due to its rejection. In 1991-1993 the long-term immunosuppression was taking including medicines like Cyclosporin, Imuran, Azatioprin and Encorton. The aim of this study was to consider the influence of immunosuppression therapy on lingual cancer appearance in our patient.

The author presents the case of complication caused by nonabsorbable silk surgical sutures which occurred two years and seven month after cesarean section and next hysterectomy performed at the same time. The sutures caused the formation of big vesical stone as well the appearance of acute recurrent pyelonephritis and stricture of the urethra. After lithotripsy, the patient completely recovered.

In the group of 289 pregnant diabetic women hospitalised and followed-up between 1991-2000 in the Maternal-Fetal Medicine Dept., Research Institute Polish Mothers Memorial Hospital, 44 patients were diagnosed with hypertension arterialis (15.2%), significantly more frequently in women with long lasting diabetes complicated by angiopathy and whose who trend to be obese. Metabolic control did not differ in the group with hypertension and without. In the group of pregnant women with hypertension following symptoms occurred significantly more frequently: proteinuria (29.5%), pyelonephritis (11.4%), anaemia (25%) and the risk of premature delivery (25%). Hypertension arterialis shortened significantly the duration of pregnancy (34.7 weeks of gestation vs. 37.3) and affected the obstetrical outcome such as:-low birth weight and longer time of newborn hospitalisation.

The aim of the study was to determine the main clinical and organising difficulties affecting quality and efficacy of medical care in children with acute pyelonephritis (AP). 41 children aged 3 wk--17 yr 4 mo hospitalized for AP at the Department of Nephrology, District Children's Hospital, Szczecin, Poland in 2000 were investigated. Epidemiologic and demographic data, history, clinical presentation, auxiliary investigations, treatment and ambulatory follow-up were analysed. Instead of typical clinical picture neither diagnostic management nor the therapy fulfilled accepted standards. The main problems found were: delay of referral to hospital by primary care doctor, misdiagnosis or inaccurate primary diagnosis followed by insufficient treatment, difficulties with performing simple ambulatory tests (urinalysis, urine culture, ultrasonography) before hospitalisation and lack of co-operation between parents and nephrological outpatient clinic after discharge. For reflux and obstructive nephropathy, often presenting as urinary tract infection, are still the main cause of chronic renal failure in children in Poland the special attention was paid to necessity of thorough education of family doctors in the subject of management of such cases as well as improvement of accessibility to specialistic care.

Urinary tract infection is a common disease in childhood affecting 7,8% girls and 1,6% boys. Despite frequent reinfections, the ultimate outcome is favorable. Progressive renal damage may occur in children with obstructive uropathy, high-grade vesicoureteral reflux and those with recurrences of acute pyelonephritis. The infection, especially in children under 5 years of age should be treated promptly to prevent possible renal scarring. In children with vesicoureteral reflux and recurrences of acute pyelonephritis long-term antibacterial treatment is indicated.

Pregnancies resulting from a spontaneous ovulation and fertilization are extremely rare in women with Turner syndrome. The majority of them end with abortion, still-born, or neonates with congenital defects. The article presents two women with Turner syndrome, mosaic karyotype 45,X/46,XX, who got pregnant without any medical intervention. The course of one pregnancy was complicated by imminent premature delivery, pyelonephritis and anaemia. Because of the lack of progress and threatening infection, the delivery was conducted by caesarean section in 40th week of pregnancy. Healthy male neonate was born. The other patient gave birth in 38th week of pregnancy to a healthy baby boy, by caesarean section, because of lack of progress in delivery. Children develop correctly. It is important to note, that despite distinct clinical features, in first patient Turner syndrome was identified 3 years after delivery.
