A 57 year-old-man with acute renal failure was diagnosed as having retroperitoneal fibrosis. Malignant lymphoma is suspected because of peripheral lymphadenopathy, but biopsies are unrevealant. Nephrostomy restores kidney function. Many other histologic samplings performed during three laparotomies are necessary to diagnose Hodgkin's disease. Nephrostomy is complicated with iterative acute pyelonephritis and septicaemias which make chemotherapy unadvisable. Ureterolysis is unsuccessful. Urinary tract infection disappears after uretero-ileoplasty. Subsequently chemotherapy induces a complete remission during 5 years with normal renal function. We review two previously reported cases of this unusual cause of malignant retroperitoneal fibrosis. Diagnostic procedures and therapeutic management are discussed.

Fifty-two intravenous pyelographic examinations performed in diabetic patients, over a period of ten years, were studied. Certain radiological patterns proved sufficiently specific to be of diagnostic use. The main function of radiology was in follow-up studies and in the detection of complications. A variety of patterns are described: pyelonephritis, renal emphysema, papillary necrosis, perinephric abscess, emphysematous cystitis, fungal infection, neurogenic bladder and calcification in the vas deferens, seminal vesicles, uterine artery and branches of the renal artery. The problem of intravenous injection of iodine contrast media in diabetic patients is discussed and risk factors are described. It is concluded that although iodine contrast media can be harmful, complications are usually mild.

Schistosoma haematobium eggs may occasionally be found in the renal parenchyma, usually with no pathological consequences, the main risk to the kidney being obstruction of the urinary tract by bilharzial lesions. Egg deposits in the submucosa and muscles of the excretory tract induce obstruction and sclerosis, mainly in the ureters. Bilateral stenosing bilharzial ureteritis results in mechanical obstruction of the renal function, sometimes, unfortunately, accentuated by sepsis and pyelonephritis. This condition is a strong indication for surgical repair of the stenotic lesions of the urinary tract, which must be performed before the onset of irreversible damage to the renal function.
