In 85 patients with ascertained chronic

the relations between the measured values of the combined
PAH
-inulin-clearance, the index of the adrenal cortex and other radiological findings were investigated. Here the determination of the cortex of the adrenal cortex resulted in a better correlation with the sizes clarification values than the calculation of the size of the kidney. By variance analysis (F-test) significant differences between the individual function groups became clear.

whether it is possible to diagnose relatively early disturbances of the renal function in values of the hippuran clearance lower than 487. It was established that in 28.9% of these patients the ING was normal, in 45% the concentration power maintained and in 70% the rest nitrogen in the serum was not increased. These results allow the recommendation of a broader application of the hippuran clearance in patients with
pyelonephritis
, since by means of this method relatively earlier restrictions of the renal function can be established than by means of other functional tests (ING and Volhard's test).

Felodipine, a dihydropyridine calcium antagonist, was used to treat eight patients with severe uncontrolled hypertension: five had essential hypertension, two had renovascular disease, and one chronic
pyelonephritis
. Mean blood pressure (BP) was 221 +/- 14/120 +/- 4 mm Hg despite treatment with three or more antihypertensive drugs. All patients experienced an immediate and pronounced lowering of BP after adding felodipine, which persisted during long-term treatment in combination with previous medication except for vasodilating drugs. In all cases, an increase in glomerular filtration rate (51Cr-EDTA clearance) after 6 and 12 months of felodipine treatment was seen (59 +/- 10 to 63 +/- 7 and 70 +/- 6 ml/min, respectively, p less than 0.05). Renal plasma flow (
PAH
clearance) exhibited only a slight increase (315 +/- 68 to 340 +/- 63 and 314 +/- 69 ml/min), giving a nonsignificant rise in filtration fraction (18 +/- 1 to 21 +/- 1 and 20 +/- 1%, respectively). It is concluded that felodipine decreases BP dramatically in patients with previously refractory hypertension and that the drug causes an improved renal function in these patients.

In an attempt to elucidate the reduced concentrating capacity in acute
pyelonephritis
, we studied the renal function in 14 children, 5 to 16 years of age. Clearance of inulin,
PAH
, and free water, urinary sodium excretion, and maximal urine osmolality after 18 hours of thirst have been determined at and within 10 months after infection. Glomerular filtration rate and renal plasma flow were found to be increased during infection and normalized after infection. Urinary sodium excretion, distal sodium delivery, and maximal urine osmolality were decreased during infection and increased afterwards. Kidney size was also increased during infection. It is postulated that the reduced concentrating capacity is due to a washout effect mediated by an increased medullary blood flow and to a low distal tubular sodium delivery.
