Isoelectric focusing in thin-layer polyacrylamide gel is a method with high resolving power, e.g. for urinary proteins. Therefore more information can be obtained with this method than with ordinary electrophoresis. The present version of the method has a high capacity and allows study of up to 25 samples in parallel on each gel. Only a little more than one hour is required for separation. A sensitive staining procedure is used which allows detection of as little as 0.1 mug of protein in each zone. When effects on the kidney are present more than 20 different protein zones can sometimes be separated. Evaluation is made by densitometry as well as by comparison and classification of patterns. Urine samples have been studied from workers with varying exposure to
cadmium
lead and various chlorinated hydrocarbons, and patients with glomerulonephritis,
pyelonephritis
, nephrosis and mixed tubular and glomerular diseases. Typical tubular patterns, i.e. as developed in association with
cadmium
exposure were characterized by elevated excretion of beta1-microglobulin. Other types of kidney malfunction showed different features.

A 3-year-old boy with mixed glomerular/tubular proteinuria, mental retardation, and hyperkinesis is described. The proteinuria was discovered at the age of 3 years on urinary mass screening. Most of the urinary protein consisted of albumin, accompanied by increases in low molecular weight proteins, including beta 2-microglobulin and alpha 1-microglobulin. Mixed glomerular/tubular proteinuria is known to be caused by the following conditions: chronic renal failure, chronic
pyelonephritis
,
cadmium
poisoning, tubulointerstitial nephritis of various etiologies, and after strenuous, short-term, exhaustive exercise. The present patient did not display any of these disorders or conditions.

Zinc and
cadmium
in both serum and erythrocytes from patients with prostatic carcinoma of different histologies, BPH, other urological tumors, and
pyelonephritis
were analyzed by atomic absorption spectrometry. The variance of the results obtained was very high for each group. No significant differences in the Zn or Cd concentration in either the blood plasma or the erythrocytes could be found between any of the different groups of patients. There were also no trends to be seen in the concentrations of these elements in human blood over a period of months. We conclude that the concentration of Zn and Cd in serum or erythrocytes are not an index for the diagnosis or therapy of prostatic carcinoma, BPH, urological malignancy, or inflammations.
