The results are discussed obtained from 198 patients with the two most frequently met kidney diseases--chronic glomerulonephritis and
pyelonephritis
, with a total of 396 renal clearance tests. In 65.7 per cent of the patients with chronic glomerulonephritis decreased values of blomerular filtration (GF) and the renal flow effect (RFE) were established, with a parallel decrease in both clearances in 56.5 per cent and isolated disturbances of GF in 31.9 per cent and of RFE in 11.6 per cent. In the patients with chronic
pyelonephritis
a parallel decrease of GF AND RFE was found in 66.7 per cent and isolated disturbances of GF in 13.6 per cent and RFE--in 19.7 per cent. The high percentage of parallelly decreased GF and RFE reveals the sensitivity of the method in the diagnosis of early renal functional alterations while the application of only one of the clearances might not detect the existing alterations. The results from the carried out comparative investigations between the isotope clearances and isotope nephrography with 131I--hippuran are also reportentage of the depuration radioisotope tests as compared with ING in the detection of early renal functional laterations. Radiation risk is rendered to a minimum with the use of the "cocktail" of 169U-
EDTA
and 125I-hippuran.

Felodipine, a dihydropyridine calcium antagonist, was used to treat eight patients with severe uncontrolled hypertension: five had essential hypertension, two had renovascular disease, and one chronic
pyelonephritis
. Mean blood pressure (BP) was 221 +/- 14/120 +/- 4 mm Hg despite treatment with three or more antihypertensive drugs. All patients experienced an immediate and pronounced lowering of BP after adding felodipine, which persisted during long-term treatment in combination with previous medication except for vasodilating drugs. In all cases, an increase in glomerular filtration rate (51Cr-

clearance) after 6 and 12 months of felodipine treatment was seen (59 +/- 10 to 63 +/- 7 and 70 +/- 6 ml/min, respectively, p less than 0.05). Renal plasma flow (PAH clearance) exhibited only a slight increase (315 +/- 68 to 340 +/- 63 and 314 +/- 69 ml/min), giving a nonsignificant rise in filtration fraction (18 +/- 1 to 21 +/- 1 and 20 +/- 1%, respectively). It is concluded that felodipine decreases BP dramatically in patients with previously refractory hypertension and that the drug causes an improved renal function in these patients.

The results of the study revealed that the best tests for evaluation of the renal function in chronic renal diseases are the electrolyte clearances of Na, K, Ca, Cl, and in arterial hypertension--the lithium clearance.

Infection-induced suppressor cells may be associated with a depression of cell-mediated immune (CMI) mechanisms in
pyelonephritis
. In the present study, cell viability and cell to cell contact were established as prerequisites for immunosuppression and the role of mononuclear phagocytic cells and polymorphonuclear leukocytes, as immunoregulatory cells affecting CMI, was also examined. Fractionation of spleen cell suspensions was carried out using carbonyl iron, nylon wool, glass beads, and sephadex. These procedures restored mitogenic responsiveness to splenic lymphocytes from pyelonephritic animals, and it was possible to isolate cells with accessory and suppressor activity from nylon wool columns. Elutable cells (that is, cells which adhere to the column but could be recovered by the addition of
EDTA
) were characteristically accessory cells and increased the mitogenic responsiveness of normal lymphocytes. Adherent splenocytes which suppress mitogenic responses were isolated from pyelonephritic animals. Additionally, neutrophils, at concentrations readily demonstrable in lesions, depressed CMI responses in vitro. With this information available it should now be possible to carry out a detailed analysis of the cellular mechanism by which CMI in renal infection is depressed.

Previous reports have demonstrated lesions on computerized axial tomography (CT), and nuclear scintigraphy (DMSA) in acute
pyelonephritis
(PN). We undertook a prospective study of all patients presenting to our hospital with PN over 40 months. Patients who fulfilled diagnostic criteria, were treated with intravenous antibiotics. Excluding two who were pregnant, all patients had imaging by intravenous urography (IVU), CT and DMSA during their admission. Urine samples were collected prior to treatment. Patients without IVU evidence of cortical scarring but with parenchymal defects on CT and/or DMSA underwent a repeat DMSA three or more months after the acute episode. Of the 164 patients, 142 were female. E. coli was found in 116 patients. Forty-six patients had an abnormality on IVU. Of the 106 patients without IVU evidence of cortical scarring, 59 had a defect on CT and/or DMSA. Late DMSA scans in 35 of these 59 patients showed a persistent abnormality in 77%. E. coli characteristics such as P-fimbriae and Type 1 fimbriae were not predictive of acute imaging abnormalities. Inhibition of E. coli growth by the addition of
EDTA
was highly predictive of acute CT and DMSA abnormalities with a sensitivity of 83.3% and a specificity of 82.8%. Acute pyelonephritis is often associated with acute CT and/or DMSA abnormalities which may evolve into renal cortical scars. Acute scan abnormalities can be predicted by the presence of E. coli which were susceptible to
EDTA
in culture. Late scarring could not be predicted by clinical features, response to treatment or antibiotic used.

Direct and indirect radionuclide cystography are two alternative modalities for X-ray MCUG. Their relative place in the strategy of management of vesicoureteral reflux is discussed.

We have sequenced a cDNA of calcium oxalate urinary stone protein extracted with

. cDNA sequences showed complete homology between urinary stone protein and human osteopotine (OPN, bone sialoprotein 1). In this study, we investigated the expression of OPN mRNA in rat kidney serving as experimental models for several conditions that are considered to be risk factors in human renal stone formation. In the renal stone formation model, the expression of OPN mRNA in the distal convoluted tubule of the kidney was enhanced compared with the control which was found sporadically positive by in situ hybridization. By Northern blot analysis, the expression of OPN mRNA was increased in
pyelonephritis
and hydronephrosis models compared with the control, but no changes were observed in dietary-acid or base-loading models. The expression of OPN mRNA was markedly inhibited in the renal stone formation model by concomitant administration of estradiol and/or progesterone.
