Computerized tomography was performed on 19 patients diagnosed as having uncomplicated acute pyelonephritis. The relationship was investigated among the laboratory findings, presence of flank pain, clinical course and severity of the lesions detected by computerized tomography. In patients febrile for less than 2 weeks healing as assessed by computerized tomography took an average of 76 days. However, in patients with repeated febrile episodes occurring for longer than 2 weeks healing was delayed until an average 232 days after onset. Computerized tomography findings generally correlated well with the erythrocyte sedimentation rate, C-reactive protein level, and presence of pyuria and flank pain. However, in patients with a prolonged course computerized tomography proved to be a more reliable indicator of progress than either the results of laboratory tests or the symptoms. In conclusion, computerized tomography was useful in the diagnosis, assessment of severity and evaluation of healing of acute pyelonephritis.

Hyperthermic intravesical perfusion therapy using peplomycin (40 micrograms/ml) in distilled water at 43 degrees C as a perfusate was performed for 2 to 3 hours in 18 patients with superficial bladder tumors and 2 with deep bladder tumors. The therapeutic efficacy was determined by cystoscopy, ultrasonography and/or CT scan. Complete and partial tumor regression was obtained in 1 and 3 of the 18 patients, respectively. There was no tumor regression in the 12 patients. Most of the patients studied had bladder discomfort such as irritation, urinary frequency and so on, during and/or after perfusion. None of the patients developed acute pyelonephritis.

Primary renal aspergillosis has been reported only five times previously. We report the first case with primary renal aspergillosis where reconstructive and not extirpative surgery was used. Patients who have predisposing factors such as diabetes, malignancies, or immunosuppression represent the primary target for fungal infection. In these patients the clinical picture of nonspecific and fungal pyelonephritis are similar. Therefore fungal infections should be considered in the differential diagnosis of urinary tract infections.

In 201 women with pathologic course of pregnancy (161 women suffering from urinary tract infection, pyelonephritis and colic, 40 women with gestosis) the kidneys and the upper urinary tract were studied by means of ultrasonography. In 86% of the 161 women with colic, urinary tract infection and septic complications a dilation of the renal collecting system was found, in 39.4% of them a marked dilation (sagittal diameter of more than 15 mm). Frequency and grade of dilation have been uncreased from the first to the third trimenon, with a significant preference of the right side. In patients suffering from gestosis a lower frequency and grade of dilation was found. Pregnant women with a sagittal diameter of the pyelon of more than 15 mm should controlled more intensively prae and post partum, but the grade of dilation is not the only criterion of therapeutic strategies.

Twelve of the 25 patients with chronic granulomatous disease treated at our institution between 1957 and 1987 were found to have urinary tract disorders. All 12 patients were male and 22 years of age or younger when chronic granulomatous disease was diagnosed. Six patients had hydroureteronephrosis in association with recurrent episodes of pyelonephritis, retroperitoneal lymphadenitis, and granuloma formation. The other six patients had genital lesions or dysuria. Among the six patients with hydroureteronephrosis, a nephrectomy was performed in two, ureterolysis was used to relieve obstruction in one, and hydroureteronephrosis resolved after antibiotic therapy alone in three. We conclude that complications involving the genitourinary system occur frequently in patients with chronic granulomatous disease. Periodic imaging of the urinary tract may detect asymptomatic hydroureteronephrosis or other treatable genitourinary abnormalities in these patients.

In connection with the analyses of 84 post-mortem examinations (47 men, 37 women, average age: 66.3 years) the author dealt with the renal complications of multiple myeloma. The signs of cylinder nephropathy, light-chain nephropathy, amyloidosis, nephrocalcinosis, urate nephropathy, acute renal insufficiency, renal vein thrombosis, acute and chronic pyelonephritis as well as the tumorous infiltration of the renal tissue have been sought for. The severity of the lesions were ranged into minimal, slight, moderate, and severe groups. On the basis of the semiquantitative morphological picture and the clinical data: 1. intact kidney (41 patients), 2. involvement of the kidney without azotemia (10 patients), 3. involvement of the kidney with azotemia (17 patients, serum creatinine level: greater than 177 mumol/l) and 4. renal involvement with chronic renal insufficiency associated with uremia (16 patients) were discerned. In the background of 33 cases (39%) with deteriorated renal function cylinder nephropathy was found most frequently (27 occasions) (32%). Every other complication occurred significantly less frequently e.g. amyloidosis or kappa-light-chain nephropathy occurred in 3 cases each.

The clinical effectiveness of cefteram pivoxil (CFTM-PI) granule, a new oral cephalosporin, was studied in pediatric patients. The results are summarized as follows. 1. CFTM-PI was given orally to 17 children in daily doses of 9.5 to 31.8 mg/kg in 3 to 4 divided portions for 2 to 10 days. Clinical evaluations were made on 14 patients. Clinical effects of CFTM-PI were excellent in 4, good in 5 of 9 patients with tonsillitis or pharyngitis, excellent in all cases of 2 patients with pneumonia, 1 patient with scarlet fever and 1 patient with pyelonephritis, and fair in 1 patient with purulent cervical lymphadenitis. Overall clinical effects were excellent in 8, good in 5, and fair in 1 with an efficacy rate of 92.9%. 2. No side effects were observed in any of the 17 patients. Hematological tests showed a slight elevation of blood platelet counts in 1 patient. 3. The taste and odor of CFTM-PI granule were well accepted by the children. 4. CFTM-PI is a useful oral antibiotic for the treatment of bacterial infections in pediatrics.

We report a rare case of anomalous inferior vena cava (IVC) in a 46-year-old woman hospitalized for the examination of right hypofunctional kidney. She had no history of trauma or pyelonephritis and there were no abnormalities in laboratory findings except serum creatinine value. Excretory urography showed no excretion of contrast medium from right kidney and retrograde pyelography revealed moderate hydronephrosis of right kidney but the obstruction of the ureter was not recognized. Abdominal computed tomographic scan showed a total trace of IVC and inferior venacavography demonstrated complete obstruction of the IVC from its origin with collateralization of upper lumber veins and vertebral veins. Surgical exploration was performed and demonstrated that IVC was a trace from the postrenal segment to hepatic segment. Two right renal veins were draining into the upper lumber vein and the right ureter was compressed slightly by lower renal vein, dilated ovarian vein and fibrotic connective tissue.
