Fetal bradycardia is a well-known response to maternal
hypothermia
, as induced at open-heart surgery, but heretofore has not been reported in conjunction with
hypothermia
from urosepsis. A 24-year-old Vietnamese woman admitted at 33 weeks estimated gestational age with
pyelonephritis
secondary to Escherichia coli developed several episodes of maternal
hypothermia
to 35-36 degrees C. During each episode of maternal
hypothermia
, the baseline fetal heart rate fell to 90-100 bpm, but with retained reactivity. During each episode, maternal vital signs were otherwise stable and oxygen saturation was normal as measured by pulse oximetry. Interpretation of fetal bradycardia during episodes of maternal urosepsis is complex. If seen in conjunction with maternal
hypothermia
, and in the presence of normal maternal cardiac and respiratory function, bradycardia is unlikely to represent fetal distress.

Human patients with renal disease frequently develop disturbed sleep and severe fatigue. To develop a model for studying factors that contribute to these symptoms, we characterized the sleep patterns of various strains of mice after acute challenge with the fungal organism Candida albicans. After intravenous administration to mice, C. albicans typically colonizes the kidney, producing acute
pyelonephritis
. Various strains of inbred mice demonstrate marked variation in the temperature and sleep responses that develop after challenge, with individual strains generally showing increased or reduced somnolence in association with fever or
hypothermia
, respectively. C. albicans-infected mice may be a useful model for identifying the genes and mechanisms that link sleep, temperature, fatigue, and the immune response.

BACKGROUND Extracorporeal membrane oxygenation (ECMO), also known as extracorporeal life support (ECLS), is a technique used to provide prolonged cardiac and respiratory support to persons whose heart and lungs are unable to deliver adequate perfusion or gas exchange to sustain life. It is indicated in patients with severe ARDS, severe
hypothermia
, and cardiac and respiratory failure when other conventional methods fail. CASE REPORT We report the case of a 22-year-old gravid 2 Para 1 woman who presented to the Emergency Department with
pyelonephritis
, who subsequently developed sepsis that progressed to ARDS. She was managed successfully with extracorporeal membrane oxygenation [ECMO] for 5 days, with heparin used as an anticoagulant. After significant improvement, she was successfully de-cannulated and extubated. CONCLUSIONS The use of ECMO in pregnancy and post-partum can be associated with several complications to both mother and fetus. With appropriate patient selection, good knowledge of the procedure, and early initiation, successful outcomes can be attained.
