Severe renin-mediated hypertension was noted in 2 children as a result of selective renal damage from vesicoureteral reflux during the early years of life. In each case the reflux had been corrected successfully long before hypertension developed. In 1 case the late damage involved only 1 kidney and nephrectomy resulted in immediate relief of the hypertension. In the second case, even though both kidneys showed segmental scarring from
calicectasis
and chronic
pyelonephritis
, removal of the atrophied lower pole of 1 kidney made hypertension amenable to medical treatment and reduced excessive renin output to a fraction of the original high levels. The mechanism of renin-mediated hypertension in kidneys with segmental scars of chronic

The occurrence of ureteral fistulas as a complication of obstetrics is very rare, mostly occurring in developing countries. Only 2 cases of ureterouterine fistulas following dilation and curettage (D & C) have been reported. A 3rd case is reported in this paper. Diagnosis was made by an excretory urogram (IVP). After D & C the patient had hemorrhaged profusely and the IVP showed prominent
calicectasis
with hydronephrosis in the right kidney. After clinical stabilization, treatment of the
pyelonephritis
and evaluation of the bleeding disorder, a right percutaneous nephrostomy tube was inserted and vaginal drainage of urine ceased. Later the patient was hospitalized for an elective repair of the ureterouterine fistula. Right ureteroneocystosomy with a psoas hitch was performed to bridge th gap of the ureter to the bladder.

Ninety-eight children have been treated with clean intermittent catheterization (CIC) over a five-year period. Follow-up in 73 of these is presented herin. The complication rate encountered with the technique is low (7 per cent major complications). Only 21 per cent of the patients were able to maintain persistently sterile urine, although no new bouts of
pyelonephritis
were encountered in the group since our report of three years ago. Six of 44 (14 per cent) refluxing renal units stopped refluxing on CIC, but in two units reflux de novo developed while on treatment. Most patients (79 per cent) demonstrated stable upper tracts on CIC, while 14 per cent showed increased
calicectasis
with 7 per cent showing improvement. Thirty-one of the patients were less than three years of age when begun on clean intermittent catheterization. We recommend CIC as the treatment of choice in neurogenic vesical dysfunction where total bladder emptying is a problem and advise that it be started as early as possible.

Basing on the data obtained on percutaneous removal of stag-horn nephroliths in 81 patients (44 females and 37 males aged 10-65), the authors discuss the operative technique and surgical results. The patients were divided into 4 groups depending on the stone size, its position in the pelvicaliceal system, the presence and severity of
calicectasis
. Nephroliths sizes ranged from 3.5 to 10 cm. Better treatment results were obtained in those patients who had the main mass of the stone in renal pelvis and in those with pyelocalicectasis. After the treatment 72 patients (88.8%) were free of the stones. Common among complications were episodes of
pyelonephritis
after the intervention and displacement of the nephrostomy drainage. The authors gained primary experience which supports benefit of percutaneous surgery of stag-horn nephroliths.
